[Potassium substitution in fasting].

Twenty patients, divided in two groups to ten were undergoing a fasting period for 14 days. One group received 80 mEq potassium per day. Despite of identical loss of body-weight, the application of potassium showed the following advantages: 1) The body is able to keep acid-base-balance. 2) The important loss of potassium, induced by starvation can be reduced to a minimum.

[A flow-through method for the O2-sensitive, enzymatic analysis of glucose (author's transl)].

A flow-through glucose-analyser is described based on an oxygen-sensitive enzymatic system. Leak-current-proof welded PTFE-membranes contribute considerably to an easy maintenance of the O2-sensors. Soluble or immobilized glucose oxidase may be used in automatic analysers. As a primary instrument of analysis, the oxygen pO2-electrode lends itself to the measurement of other enzymatic reactions that involve oxygen.

[Fasting values of carbohydrate--and lipid metabolism in healthy subjects and in patients with diabetes mellitus].

In 106 healthy adults and 34 in-patients with diabetes mellitus the venous blood concentration of lactate, pyruvate, hydroxybutyrate and acetacetate was measured. In healthy men we found a lactate concentration of 7.7 mg/100 ml (7.11-8.15), a pyruvate level of 0.37 (0.34-0.44) mg/100 ml, a level of acetoacetate of 0.41 mg/100 ml(0.38-0.47) and a level of hydroxybutyrate of 0.47 mg/100 ml(0.29-0.70).

[Serum immunoglobulin levels in ulcerative colitis and Crohn's disease (author's transl)].

The immunoglobulins G, A and M were determined quantitatively by radial immunodiffusion in 50 patients with ulcerative colitis and 41 with Crohn's disease. 50 clinically healthy probands formed the control group. There was no correlation with duration, severity, and localisation of the disease, or with the treatment used. IgA was significantly lower in ulcerative colitis as compared with the control group. The most marked changes of immunoglobulin levels observed were in Crohn's disease: whereas IgA was increased by 37%, IgM was decreased by the same percentage. A typical immunogram in Crohn's disease is a valuable differential diagnostic feature as compared with ulcerative colitis.

[Electrolyte metabolism in obese subjects with various forms of therapy].

In 30 obese subjects three different methods for weight, reduction were applicated over a period of 14 days. One group (n=10) was treated with total starvation, the other group (n=10) with total starvation and 80 mval potassium in addition and the third group (n=10) with a 700 cal. diet. In total starvation the balance of sodium, potassium and phosphate amounted to -9 mval/d, -34,9/d and -8,8 mval/d respectively. Whereas calcium showed a positive balance of 4,4 mval/d. During addition of 80 mval potassium sodium excretion increased whereas potassium excretion was diminished resulting in a potassium balance less negative. During treatment with the 700 calorie diet the highest negative sodium and a high positive potassium balance were observed,

[Metabolic effect of a parenterally administered fat emulsion with middle-chain triglycerides in healthy men].

Within 3 hours, 10 healthy male volunteers were infused 500 ml of a 5 percent emulsion in which 25 percent of the fat proportion had been replaced by MCT-fats. As expected, the ketone body concentration in the blood rose, while pyruvate remained constant and lactate dropped. The results show that, basically, a MCT-containing fat infusion is suited for parenteral nutrition and, because of their specific properties, medium chain triglycerides may be used as rapid energy donators.

[Essential-fatty-acid deficiency syndrome in man].

The syndrome of essential fatty acid deficiency was first described in rats in 1929. Not before 1958, a corresponding syndrome was observed in infants after administration of cow's milk over longer periods of time. Until recently, the syndrome of essential fatty acid deficiency in adults was completely unknown. In 1970, it was reported in a 44-year-old male patient who had received fatfree parenteral nutrition for 70 days. In its course, a desquamative exanthema developed which completely remitted after administration of linoleic acid. Between 1970-1973, we observed two patients who developed hyperkeratotic dermatoses after long-term administration of a synthetic formula diet (Vivasorb). The lack of essential fatty acids could also be demonstrated in gas-chromatographic studies of the course of the process.

[Ketone bodies in the blood during sorbitol infusion].

The concentration of hydroxybutyrate and pyruvate during infusion of sorbitol was significantly depressed in 12 healthy subjects. The dosage ranged between 0.25 and 0.50 g/kg BW/h. Considering the blood parameter the hunger ketosis was depressed by the infusion rate of 0.25 g/kg/h. Cholesterol concentration, too, was significantly lowered by sorbitol.

[Carbohydrate administration and electrolyte balance in patients under intensive care].

A twenty-four percent solution of carbohydrates 0.2 g/kg/h consisting of fructose, xylit and glucose in a 2:1:1 ratio and an aminoacid solution 0.03 g/kg/h were given to 13 patients of the internal medicine intensive care unit. There was an unimportant loss of carbohydrates, only, with but few exceptions, i.e. a loss of 11% in 3 patients. The results demonstrate that the carbohydrates of the combined solution become utilized independent of each other. The balance of water, sodium, potassium and magnesium were not only negative but also positive. The elimination of calcium was between 1-19 mval/24 h, of phosphate between 0.6-8.7 mval/24 h.
