[Improvement of zoster therapy by adamantine].

In the course of 3 years we observed a considerable improvement of herpes zoster in 44 patients being treated with amantadine. The periods of pain and efflorescence were shortened to 1/3 of the values usually experienced and painful post-zoster complications did not occur. The therapeutic effect depends on a) beginning treatment with high doses as early as possible, b) combination of local and systemic administration of adamantine, c) continuation of treatment for several weeks with gradually reducing doses. Harmless side-effects which are easily controlled are dryness of mouth, slight fall in blood pressure and insignificant general stimulation. In old people half the standard dose should be given in the beginning and particular attention paid to symptoms of restlessness on account of a possible delirium. Severe disorders of renal function are a contraindication.

[Concomitant depressions in non-psychiatric diseases. Identification and therapy of depressive components].

We introduce the new term "concomitant depression" and define it purely phenomenologically as the simultaneous appearance of a depressive mood and physical disease. Its place within the framework of previously current nomenclatures and classification according to causal principles are discussed. Neurology and internal medicine in particular are greatly confronted with the problem of concomitant depression. The main fields are the cerebral insult, radicular pain syndromes and internal medical conditions which tend to chroncity. The proportion of depressive component amounts to 1/5 to 1/3 of the total number of patients. By means of an anterospectively programmed medical record, we were able to evaluate the concomitant depression quantitatively. Suggestions for rational pharmacotherapy are given.

[Cerebral circulation (measured by isotopes) and acid-base equilibrium of the cerebrospinal fluid during migraine].

In migraine patients a considerable metabolic disturbance characterised by a decrease in bicarbonate and an increase in lactate was demonstrated during the migrainous state. Measurements of the cerebral circulation were taken by means of radioactive isotope and showed a distinct decrease in blood supply to the entire brain during the migraine attack. Following forced hyperventilation during the attack-free interval a pathological reactivity with decreased blood flow was found in the hemisphere which had been indicated as the usual site of the migrainous pain.
