[Interdisciplinary surgical therapy of tumors of the frontal skull base].

Most tumors of the anterior cranial base invade both the intra- and extracranial regions at the borderline of the cranial and facial skeleton. In surgical treatment of these lesions, combined craniofacial approaches are applied in accordance with the anatomical conditions. Surgery is performed by an interdisciplinary team of neurosurgeons and maxillofacial surgeons. To evaluate the effectiveness and the complications of this surgical concept, the data of 58 patients treated over a 10-year period were collected. In these patients, 60 operations were performed, including two operations for tumor recurrence. In 38 cases, complete tumor removal was achieved. One patient died in the early postoperative period because of pulmonary embolism. Significant neurological deficits occurred in three patients. In most cases, postoperative complications and functional disability were both due to intradural invasion of the tumor. Nevertheless, in the majority of the cases radical tumor removal is achieved with acceptable morbidity when microsurgical techniques are applied for the resection of the intradural tumor. Both the meticulous repair of dural defects and the reconstruction of the anterior cranial base and orbit contribute distinctly to a reduction in the rate of postoperative complications and to acceptable cosmetic results.

Characterization of the periodontal microflora by the fatty acid profile of the broth-grown microbial population.

The applicability of fatty acid analysis to the characterization of periodontal microflora was investigated using gas-liquid chromatography (GLC) and the software of the Sherlock Microbial Identification System (MIS) from MIDI Inc. Sulcus fluid was collected with paper points and anaerobically cultured in broth at 35 degrees C for four days. The broth-grown microbial population was extracted and the fatty acid methyl esters (FAME) were separated by GLC. The investigation of 67 specimens from asymptomatic sulci and of 32 specimens from inflamed sulci showed that the patterns of FAME profiles, the clustering of FAME profiles by computerized 2-D plot procedure, and the determination of the peak area index (PAI) of the FAME profiles differentiate between normal and pathological sulcus flora. Comparison of the clinical sulcus rating and the FAME data indicated that a pathological FAME profile may precede manifest periodontitis, and the normalization of the FAME profile may precede healing. It is concluded that the FAME analysis of sulcus fluid is a diagnostic aid for periodontological surveillance, for the initiation of preventive treatment of periodontitis, and for controlling the antimicrobial efficiency of therapeutic measures.

[3-dimensional photogrammetry assessment of facial contours].

In Germany, three-dimensional non-invasive measurement techniques are not in routine use for medical purposes. Completely integrated applications of photogrammetric technology are lacking. The results of clinical examination, X-rays and pre- and postoperative photographs from different angles have been used for medical analysis. In an interdisciplinary research project we tested the general applicability of photogrammetric measurement systems. We examined patients with malformations of the mandible-maxilla complex by taking pictures of the face. In order to assess the surface structure we projected regular patterns onto the surface. We calculated about 500 points on the surface with accuracy better than 0.2 mm. Graphical analyses of measurement results are presented in clinically relevant form. We produce representations of the faces in auto-CAD by means of regular meshes which allow views from any perspective, longitudinal and lateral sections. In addition to calculating angles, distances, surfaces and volumes, visualisation of shape is a useful aid in documentation and quantification of changes of soft tissue of the human face under surgery treatment.

[Long-term outcome after intraoral miniplate osteosynthesis in mandibular angle fractures].

The success or failure of intraoral miniplate osteosynthesis is mainly influenced by right indication. More than 90% of the fractures of the mandible were treated successfully by intraoral noncompressive miniplate osteosynthesis. Hence a postoperative intermaxillar fixation seems not to be necessary. Considering the range of indication this is an effective method of no complications. Nevertheless an immobilisation for one week is accepted by our patients without any problems and is recommended according to normal healing. The collective of our patients in Essen is selected as unilateral multiple fractures, comminuted fractures and infected fractures of the mandible were not treated by miniplate osteosynthesis.

[Dental treatment before radiotherapy of carcinoma of the mouth floor].

Two collectives of patients who required radiation therapy were built. In order to determine the adequate occasion and extension of surgical teeth restoration in dependence on radiation therapy, both collectives were compared. The first group implicated 21 manifest ORN. The triggers of these ORN were evaluated. In the second collective the ORN occurrence after 225 generous surgical teeth restorations before radiation therapy were prospectively fixed and analysed. In comparison of both collectives the concept of generous removing of all potential dentogene inflammations before radiation therapy was successful in prevention of the septic ORN.

[Osteoplasty of osseous defects of the frontal bone and orbital roof--indications, technique and results].

From 1972 to 1992, 65 patients with traumatic or postoperative defects of the orbitocranial skeleton were operated on at the department for maxillofacial surgery, University of Essen. Until 1989 alloplastic reconstruction with PMMA implants or autogenous bone grafts of the iliac crest were preferred. Since 1989 we have utilized autogenous calvarial bone grafts (17 cases). CT based models of the defect helped to select calvarial donor sites of appropriate size and contour. The calvarial autografts provided high resistance against resorption. Until now, we have seen no major donor site morbidity. On the whole the results substantiate these autografts to be the superior material for orbitocranial reconstruction.

[Long time results of the Sandwich-technique for mandibular alveolar ridge augmentation].

During six years twenty-five patients have been operated upon using a "sandwich technique" for mandibular alveolar ridge augmentation. This technique was advocated by Schettler in 1974. In 3 cases, autologous rib cartilage was used, in 20 Cases an autologous transplant from the iliar crest and in two cases, autologous "cialit" conserved bone chips were inserted. All transplants healed without complications and less resorption was found in all transplants at the late follow-up examinations.

[Indications for the surgical treatment of midfacial fractures in children].

A study of midfacial fractures in 28 children under 16 years of age treated at the Medical School Essen is reported. All children underwent surgical treatment. 18 patients with fractures of the zygomatic complex were treated by open reduction and miniplate osteosynthesis. 10 patients with blowout fractures were treated with "Lyodura"-implants. To prevent persistent diplopia an early surgical treatment is recommended. An adverse effect on the growth of facial bones has not been seen.

The surgical approach to orbital space-occupying lesions.

From January 1985 to April 1987, 60 patients with orbital space-occupying lesions were treated surgically. Orientation and spatial localization of the lesion had already been achieved preoperatively by means of imaging methods. The decision on whether to use a neurosurgical transcranial, or a facial surgical approach, was made on this basis. The less elaborate procedure in each case was given preference to obtain a representative tissue sample. The definitive planning of the operation was then carried out with knowledge of the histology: a transcranial operation alone or as an additional measure was only necessary in six patients. Either definitive tumour therapy could be performed with or without temporal osteotomy in the remaining 54 patients exclusively via an anterior orbitotomy, or further oncological treatment was commenced after histological verification of the diagnosis.

Choice of surgical approach in orbital space occupations--an interdisciplinary problem.

During the last two and a half years 60 patients with orbital space occupying neoplasms were treated surgically at the Department of Maxillo-facial Surgery of the University Clinic of Essen. With modern imaging techniques exact information on the location and extent of space occupation was available before the operation. Therefore the surgical approach could be optimized. Despite the various and sometimes inaccessible tumor locations, transcranial operation was only necessary in six patients. In the remaining 54 patients anterior orbitotomy allowed either a definitive tumor therapy or histological verification of the diagnosis.
