[Immunohistochemical detection of thyroglobulin].

An antiserum against human thyroglobulin isolated by gelfiltration from the human thyroid gland was raised in rabbits. The specificity of the antiserum to detect human thyroglobulin was determined by an immunoperoxidase technique on formalin-fixed and paraffin-embedded tissue sections of 17 primary thyroid carcinomas and 80 carcinomas of other origins, such as pancreas, colon, breast, bladder, ovary, testis, and larynx. Positive reactions to immunological staining were recorded only from differentiated thyroid carcinomas, while no reactions were exhibited by the other malignant tumours tested in this study.

[Radiation pneumonitis. Morphology and pathogenesis].

Radiation-induced pulmonary damage is an inflammatory process which progresses in stages. The quantity rather than quality of pulmonary reactions depends on the given radiation dose. Damage to endothelial cells of capillaries and of respiratory epithelium is the initial event in the development of radiation pneumonitis. Release or activation of cytogenic and serum-derived mediators, increased permeability of the capillary wall, and damage to pneumocytes II are the most important factors in pathogenesis. There is no morphological latent stage. The latency between exposure to irradiation and manifestation of clinical symptoms of pneumonitis may be explained by the morphologic findings. Several non-radiation-induced lung diseases are predisposing factors. There is a tendency to bacterial, viral or mycological superinfections in patients with radiation pneumonitis. Radiotherapy leaves behind a latent pulmonary lesion with continuously changing reactions to most various irritations.

[Pericardial mesothelioma. Apropos of a case].

A 74 year old woman without previous cardiovascular disease presented with pericardial effusion and tamponade. Two-dimensional echocardiography displayed a pericardial mass, but a diagnosis of tumour was not envisaged. Pericardiocentesis produced an exudative fluid with lymphocytes but no malignant cells. Pericardial biopsies through axillary thoracotomy indicated chronic progressive pericarditis. As the patient's haemodynamics deteriorated, sternotomy was carried out showing diffuse pericardial sclerosis corresponding, at histology, to a mixed, predominantly fibroblastic pericardial mesothelioma. The authors describe the main clinical characteristics of pericardial mesothelioma, always discovered belatedly at surgery or necropsy. They insist on the value of non-invasive methods (two-dimensional echocardiography, computed tomography) to diagnose a pericardial tumour, and on the poor prognosis of mesotheliomas, treatment still being uncertain due to the small number of cases and to the early occurrence of cardiac complications.

Detection of Dirofilaria immitis microfilariae in peripheral blood. A quantitative comparison of the efficiency and sensitivity of four techniques.

Four techniques for the detection and quantification of Dirofilaria immitis microfilariae in peripheral blood were compared, namely the conventional thick smear, thick films prepared by cytocentrifugation, filtration of blood through polycarbonate membranes, and density gradient centrifugation followed by membrane filtration. The efficiency of the methods and their sensitivity was assessed by determining quantitatively the recovery of known numbers of microfilariae from defined volumes of blood. Polycarbonate membrane filtration either alone or in combination with density gradient centrifugation showed an efficiency of 0.9 and 0.8, respectively, and reached 100% sensitivity with microfilariae densities of greater than or equal to 10 microfilariae per ml of blood. Conventional thick smears and cytocentrifugation were of considerably lower efficiency (0.02 and 0.03, respectively) and reached 100% sensitivity with more than 200 microfilariae per ml.

[The importance of autopsy for quality assurance in medical care].

For all persons who died in the city of Dresden and 7 surrounding counties from 1967 to 1978, documents relating to the clinical and pathologic diagnosis were examined and findings compared. The study covered nearly two thirds of all deaths which occurred in the district of Dresden during this time period. The autopsy rate overall was found to be 22.8%. In 57.3% the clinical diagnoses were found to have been correct, in 19.7% they corresponded in part, but in 23% there was no concordance whatsoever between the clinical and autopsy diagnoses. The influence of age, place of death and disease group on the agreement between clinical and underlying pathoanatomic diagnosis was also examined. The analysis highlighted the degree to which death statistics based on the death certificate are misleading. In up to 28.8% of cases there were formal errors in the underlying disease diagnosis listed by the certifying physician on the death certificate. The value of regular comparison between the diagnoses, and its usefulness for training and continuing medical education are emphasized. The results of the study underline the importance of making available more prospectors in the district of Dresden to meet the expanding tasks of the clinically active pathologist in autopsy and biopsy diagnostic efforts.

[Results of microscopic image analysis of the thyroid gland fine needle biopsies].

In previous cytomorphometrical studies applying a microscopic image analysis system it was shown that follicular adenomas and highly differentiated follicular carcinomas of the thyroid gland can be distinguished. Basing on these results, the usefulness of the method was examined in selected cases of routine materials. 143 fine needle biopsies were analysed by means of the image analysis system. Moreover, in 52 cases the results could be checked by histological investigations of surgical samples. In comparison with the cytomorphological findings, the results of the cytomorphometrical analysis demonstrate a higher grade of assurance of the diagnostic statements. It was confirmed, that it is possible to diagnose highly differentiated follicular carcinomas of the thyroid gland with the method demonstrated above.

Circulating immune complexes and the prognosis of acute myeloid leukemia.

We studied the relation between the clinical course and the presence of circulating immune complexes at diagnosis and/or during complete remission in 186 patients with acute myeloid leukemia. Patients with immune complexes at diagnosis had significantly fewer complete remissions (32 vs. 94 per cent), remissions of shorter duration (median, 4.3 vs. 15.0 months), and shorter survival times (median, 1.8 vs. 22.3 months) than patients without such complexes (all comparisons, P less than 0.01). All patients with immune complexes during the first two months of remission remained in remission for less than six months, whereas only 11 per cent of patients without complexes within this period had such early relapse. Of 23 patients who relapsed after long remissions, 18 (78 per cent) had immune complexes that preceded hematologic evidence of relapse by three weeks to six months (median, 3.7 months). These findings suggest that circulating immune complexes may reflect an important aspect of the pathophysiology of acute myeloid leukemia, and that measurement of these complexes can provide useful prognostic information at diagnosis and during remission.

Antigen-Antibody Complex↗

Combination of techniques for concentration and identification of microfilariae from peripheral blood.

Human blood containing a known number of Dirofilaria immitis or Brugia malayi microfilariae was incubated with Giemsa-stain or methylene blue solution and then subjected to density gradient centrifugation in Percoll-solution at a density of 1.090 g/ml. This resulted in complete separation of microfilariae from cellular components. Subsequently the microfilaria-containing layer was altered through a polycarbonate membrane. The stained parasites are then easily recognized and identified on the completely transparent membrane. The average recovery was 80%. This technique proved useful as a preparatory method and as a diagnostic tool for the detection of microfilariae in patients with low microfilaraemia.

[The differential diagnosis of follicular adenomas and carcinomas in fine needle biopsies of the thyroid gland by means of automatic image analysis (author's transl)].

Problems in discriminating follicular adenomas from carcinomas in fine needle biopsy material of the thyroid gland by means of conventional microscopy led us to perform morphometric analyses. Applying a microscopic image analysis system for automatically recording of smears of the thyroid gland that were obtained from fine needle biopsies, it seemed possible to analyse "follicular neoplasias". However, in order to get statistical assurance, a larger number of samples remains to be investigated. The usefulness of the method is to be confirmed by its routine application.
