Accidental monensin sodium intoxication of feedlot cattle.

Of 1,994 yearling and 2-year-old cattle in a winter feeding program, 117 died within 42 days of being fed toxic amounts of monensin sodium in a liquid protein supplement. Death losses commenced on the third day after ingestion of a toxic amount in the feed. Clinical signs in cattle that died in less than 9 days included anorexia, pica, diarrhea, depression, mild hindlimb ataxia, and dyspnea. Gross necropsy findings in cattle dying in the acute phase of the illness included hydrothorax, ascites, and pulmonary edema, as well as petechial hemorrhages, edema, and yellow streaking in skeletal and cardiac muscle. Cattle dying after 9 days had gray streaks in heart and skeletal muscle, generalized ventral edema, enlarged, firm, bluish discolored liver, and enlarged heart. Microscopic changes in cattle dying in the acute phase (less than 9 days) consisted of pulmonary edema, congestion, and hemorrhage. Cardiac and skeletal muscle had localized areas of edema, hemorrhage, and coagulative necrosis. In cattle dying after 9 days of illness, the changes included lymphocytic infiltration, sarcolemmal nuclear proliferation, and fibrosis in skeletal and cardiac muscle. Lungs contained increased alveolar macrophages and a few neutrophils. Centrilobular necrosis and mild fibrosis were found in the liver. Changes varied somewhat according to the area of heart or skeletal muscle that was affected. Active muscles, eg, those in the heart ventricles and diaphragm, were altered most severely. Intoxication appeared to be a result of sedimentation of monensin in the molasses carrier to give remarkable concentrations of the substance at the bottom of the holding tank.

[Measurement of flow physiology of the large vessels in retinal circulation in type 1 and type 2 diabetics].

Segmental blood flow, velocity, and vessel diameter were measured in 21 eyes of diabetic individuals with diabetes type 1 and 2. The results for arterial blood flow, velocity, arterial and venous vessel diameter in diabetes show significant differences from the results measured in normal individuals. These flow magnitudes, excluding arterial diameter, significantly depend on the duration time of diabetes. The retinal blood flow magnitudes alter before the marks of diabetic retinopathy are visible. There are differences between the results in diabetes type 1 and type 2 regarding dependence on the duration time of diabetes.

[Evaluation of fluorescein angiograms using the multitemporal technic].

If there are differences in the density of black-and-white photographs of the fundus oculi taken at different times, they may be transformed into color differences by means of the multitemporal technique. A multitemporal picture is obtained by additive color mixing of black-and-white photographs transilluminated at the same time with one primary color each. Since, in a serial angiogram, the interrelation between the film density and the passage of the fluorescein concentration in the fundus oculi is known, it is possible to draw conclusions from the multitemporal picture regarding phase differences and changes in the curve shape of the fluorescein concentration.

[Principal possibilities and limitations of fluorescent angiographic procedures for the measurement of the flow-physical magnitudes of the retinal circulatory system (author's transl)].

Flow-physical magnitudes are presented that describe the retinal circulatory system. The principal possibilities and limitations of fluorescent angiographic procedures for the measurement of these magnitudes are discussed. On this basis, we compare fluorescentophthalmoscopic measurements of time, serial angiography, and cinematography with densitometric evaluation of the negatives, integral fluorophotometry, 'two point' photometry, and the application of television with evaluation of the video signal.

Avascular necrosis of the distal fibular epiphysis: a new condition?

A 5-year-old boy was seen because of a right ankle sprain; radiographs showed a marginal anterior avulsion of the distal fibular epiphysis. After plaster immobilization, the patient continued with pain and subjective instability. New radiographs showed a lytic area in the same zone of the avulsion with surrounding sclerosis and a detached bone fragment. Biopsy showed bone necrosis. Two years later, without any traumatic event, the patient began to have a similar pain in the left ankle. Radiographs showed fragmentation of the anterior portion of the distal fibular epiphysis with surrounding sclerosis. Surgical treatment was performed by curettage and cancellous bone grafting; biopsy showed connective tissue, mucoid degeneration, and necrotic bone. After excluding any general disease, we believe that this bone necrosis could be considered an epiphyseal idiopathic avascular necrosis in children.

Multisubstance analysis of reflection spectra before and after laser photocoagulation for proliferative diabetic retinopathy.

PURPOSE: The aim of the study was to provide new data that add to the understanding of diabetic retinopathy and its treatment by photocoagulation. PATIENTS AND METHODS: Using a spectrometric method, the reflection spectra of the macula in six type II diabetics with proliferative diabetic retinopathy were measured before and after laser photocoagulation and compared with normals. The investigations were done one day before laser photocoagulation and again the day after. Further measurements were made 14 days, 6 weeks and 4 months after coagulation. The reflection spectra were assessed by multisubstance analysis. RESULTS AND CONCLUSIONS: The contents of xanthophyll, melanin and choroidal oxyhemoglobin and the intensity of scattered light were significantly altered after laser coagulation. For example, in patients with proliferative diabetic retinopathy the content of oxyhemoglobin in the choroid was lower than normal, but it rose after laser treatment. This suggests an improved oxygen supply of the choroid after laser coagulation, implying reduced retinal hypoxia.

[Extra-abdominal desmoid tumors. Diagnostic-therapeutic considerations and review of the literature].

The authors review the literature, discuss clinical aspects and options in the management of desmoid tumors, analyze recent advances in the understanding of its biology and how they affect therapeutic choices, and outline future treatment perspectives. We conclude that although recurrence rates are high, wide excision is the best firs approach. If it is not possible, marginal surgery and radiation therapy can be performed. Radiation therapy can be also use in the attempt to control recurrent disease. Although many pharmacologic agents have been advocated there is no clear proof of their usefulness. Systemic therapy may be considered for selected patients that are not amenable to local treatment.
