[Lymphogranulomatosis in childhood].

Hodgkin's disease is relatively rare in children. The lymph node biopsies of 66 children with Hodgkin's disease were reclassified and discussed in relation to age and sex. The youngest patient was a 2-year-old boy. Mixed cellularity was the most common histologic type (37.9%-25 cases) showing a slight preponderance of boys (14 cases). Nodular sclerosis comprised 31.8% (21 cases) and the lymphocyte predominant type was found in 19.7% (13 cases) in contrast to the low incidence of the lymphocyte depletion type (6.1%-4 cases). At the age of 2 to 10 years Hodgkin's disease prevailed in boys. Our results are discussed regarding the findings of the literature.

[Malignant non-Hodgkin lymphomas of the orbital region. Histological classification and immunohistochemical studies].

We analysed 15 cases of malignant non-Hodgkin's lymphomas of the orbital region in a retrospective study of the biopsy material from 1965 to 1981. There were 9 cases of lymphoplasmacytic immunocytomas , 1 case of pleomorphic immunocytoma, 2 cases of centroblastic-centrocytic lymphoma and 1 case of plasmacytoma, lymphoblastoma and pleomorphic immunoblastoma . In the majority of cases the orbital region was the primary site of the lymphoma. In a minor part the lymphomas were the expression of a generalized lymphomatous disease. The 15 patients were 12 men and 3 women from 38 to 82 years of age. The peak of incidence was in the septuagenarians . In some of the cases the neoplastic nature of the lymphoproliferative disorder was proved immunohistochemically by the presence of monoclonal cytoplasmic immunoglobulin. The clinical course of the disease was very different and could not be predicted bioptically. Discussing the results we compared our own findings with those in the literature.

Inverted papillomas of nasal cavity and paranasal sinuses. Ultrastructural investigations on epithelial-stromal interface.

10 cases of inverted papillomas of nasal cavity and paranasal sinuses were examined electron microscopically with particular regard to the epithelial-stromal interface. The papilloma cells were clearly demarcated from the stromal tissue by a basement membrane-like material. However, this structure mainly consisted of two or more layers, occasional breaks in the basement membrane were visible. Within the basal epithelial cells resting on the basement membrane accumulations of actin-like microfilaments could sometimes be observed. Furthermore, in the stromal tissue some myofibroblasts were present in the vicinity of the papillomatous cell complexes and capillary vessels also demonstrated several layers of basement membrane. All the changes presented here are considered to be the result of a permanent interaction between aggressive forces of papilloma cells and the defence mechanism of stromal tissue. The results allow the conclusion that inverted papillomas of nasal cavity and paranasal sinuses are true neoplasms with very low malignancy.

[Investigations on the ultrastructural cytology of branchiomeric paragangliomas (author's transl)].

The paragangliomas of the jugular and carotid bodies are growth processes which show varying histological and cytological patterns. The real nature of these lesions is still a matter of discussion. In this study 6 jugular and 3 carotid body tumours were investigated using the electron microscope. The main components were irregular cells with short and long cytoplasmic processes closely packed and mostly arranged in complexes. The ultrastructural cytology of the jugular and carotid bodies showed no striking differences. The great majority of the cells exhibits a clear similarity to the cells of normal paraganglia, especially to chief cells of carotid bodies (type I cells). But there were some modifications in the cellular structure including the abnormal structure of mitochondria and a greater variation in the morphology of the dense cored granules. The cytoplasm was dominated by differing populations of mitochondria comprising such of the crista type and such showing tubulus-like structures with a dense matrix and very often intramitochondrial bodies. The characteristic feature was the occurrence of randomly distributed dense cored granules measuring 700-800 A in diameter. These granules display a homogeneous electron dense or moderately dense somewhat granular core which is separated from limiting membrane by a clear electron lucent halo. A further population of granules was of a greater dimension (3,000-4,000 A) revealing an oval or cigar-like shape. In contrast to the former type the homogeneous dense cores of these granules showed a smooth contour and the lucent halo was very narrow. In general, singular exocytotic phenomena could be supposed. More frequently, however, signs of granulolysis were encountered. Free ribosomes, arranged often as polysomes, sparse, disorderly distributed microfilaments of 60-80 A diameter, few 240 A-microtubules and some lysosomal structures are also observed. Usually the cell clusters abutted directly upon the vascular connective tissue. In other cases cytoplasmic processes are intercalated resembling sustentacular cells. Fenestrated capillaries known to be typical for the paraganglionic tissues were only seldom detected. In the interstitial tissue typical collagen fibres are found showing a periodicity of 600-700 A, but occasionally fibres with a periodicity of about 1,200 A could be seen resembling fibrous-long-spacing collagen. Synaptic junctions and nerves or axons were always lacking.

Malignant fibrous histiocytoma of bone. Light microscopic and electron microscopic examination of four cases.

Malignant fibrous histiocytoma (MFH) of bone is a well-defined tumor by light microscopy but no agreement has been achieved concerning its histogenesis. We present the light and electron microscopic findings of four cases of MFH of bone. In case 1 multiple bone tumors were observed and in case 4 the tumor developed after irradiation. It was our aim to document the cytological variability and to arrange the findings in a histogenetic concept of primary intraosseous MFH. We observed some undifferentiated cells but mainly histiocyte- and fibroblast-like cells including intermediate forms, and several types of giant cells. We should emphasize the fact that there were also some large cells with a light microscopic resemblance to rhabdomyoblasts and with electron microscopic characteristics of myoblastic differentiation. From the ultrastructural point of view, therefore, MFHs seem to derive from a primitive mesenchymal stem cell rather than from the ordinary histiocyte. It is suggested that osteosarcoma and MFH of bone may have a common progenitor cell but it is important to make a clear clinico-pathological distinction between the tumors because of differing biological behavior.

[Juvenile malignant lymphomas. 1. Hodgkin's lymphoma].

From 1970 to 1978 22 children with Hodgkin's lymphomas at the age of 4-15 years were treated at the university children's hospital of Jena. There were 16 patients with the first appearance of the disease and 6 with relapses. The stage classification was carried out after the Ann-Arbor-classification. A "staging" operation with laparotomy and splenectomy was performed in 17 children. The histological material was classified after the Rye-modification of the Lukes-Butler-classification. The clinical staging showed 8 patients in stage I, 7 in stage II, 6 in stage III and one child was in stage IV. The therapy consisted of a telecobalt irradiation extended field irradiation, total nodular and local irradiation) with a focal dose of 4500 rad and a chemotherapy (6 cycles COPP). The life-table-analysis for those patients who were primarily treated in Jena showed a complete five-year-remission rate of 92 per cent. The five-year-survival-rate for all patients (with the first appearance of the disease and with relapses) amounts to 77 per cent. After splenectomy we observed two overwhelmingly progressing aetiologically not clear infections and a pneumococcal meningitis.

[Juvenile malignant lymphomas. 2 Non-Hodgkin lymphomas].

Between 1970 and 1978 33 children with Non-Hodgkin-lymphomas at the age of 2-15 years were treated at the university children's hospital of Jena. 27 patients showed the first appearance of the disease, 6 patients had already been treated in other hospitals and were admitted with relapses. The biopsy material was classified or re-classified after the Kiel-classification. Beside the histological classification the surface markers of the malignant cells of NHL-patients were determined. 20 of 33 children were already in stage IV (Ann-Arbor-classification). Among our patients were 6 lymphoblastic NHL of Brukitt type, 10 of the convoluted cell type and 16 unclassified and one lymphoblastic lymphoma. The main localization of the NHL were mediastinum [15] and the gastrointestinal tract [10]. The therapy consisted of irradiation and chemotherapy (2 protocols) and, in case of an abdominal localization, in the attempt at a radical operation. Patients of stage I and stage II showed a complete remission rate of 50 per cent for 3 years; patients of stages III and IV of 20 per cent only. NHL of the convoluted cell type and of the Burkitt-type proved to have worse three-year-remission rates (16 per cent and 27 per cent) than unclassified lymphoblastic NHL (42 per cent).

[Experiments concerning the value of immunoglobulin-determinations (IgG, IgM, IgA) with respect to the immunological reactions of patients suffering from malignant tumors (author's transl)].

The present paper is a report on the determination of G, M and A serum immunoglobulins of 177 patients (98 men and 79 women) at the age of 19 to 78 years with malignant tumors and of 100 healthy controls of the same setup with respect to age and sex. The results were evaluated by cross-section and longitudinal examinations dependent upon localization, TNM classification, histological differentiation and the therapy of the tumor. The results show that the G, M and A immunoglobulins are not suitable for statements on the situation of the reaction of the organism of tumor bearers and on the prognostic course. Neither is it possible to use the determination of the concentration of serum immunoglobulins for the regulation of a therapy.

[Occurrence of naphthol-AS-D-chloracetate-esterase-activity in mast cells with special references to cutaneous mastocytoses (author's transl)].

The enzyme naphthol-AS-D-chloracetate esterase (NASDCE) was demonstrated histochemically in mast cells of tissues of laboratory animals and men under normal and pathological conditions using the method after Leder (1964). Investigating the possible action of different fixations as Bouin's and Carnoy's fluid as formaldehyde also cryostate sections were studied for comparison. After tissue fixation the activity of the NASDCE is localized strongly in the granules of mast cells. The type of fixation is without influence on the histochemical reaction and localization. Differences concerning the investigated species could not be demonstrated. In benign neoplasias of mast cells such as diffuse (urticaria pigmentosa) and localized cutaneous mastocytosis as well as benign systemic mastocytosis with skin involvement (Sagher) the granule bound reaction is very intensive. The NASDCE can be recommended as a marker for the microscopical localization of mast cells, especially in the histological diagnosis of their benign and malignant neoplasias.

Synovial sarcoma of the abdominal wall. Light microscopic, histochemical and electron microscopic investigations.

A synovial sarcoma of the abdominal wall in a 56-year old woman showed the typical features of this tumor type. Histologically a characteristic biphasic cellular pattern with epithelium-like cell complexes and sarcomatous spindle cell areas was found. The histochemical examination revealed that tumor cells synthesize glycoproteins and weakly acid glycosaminoglycans (mainly hyaluronic acid). Electron microscopically the tumor cells in epithelium-like cell islets were sometimes arranged in gland-like formations with microvilli at the luminal side, specialized intercellular junctions and a peripheral basement membrane-like condensation of the ground substance. There was no fundamental cytological difference between cells of epithelium-like and spindle cell areas. Generally the tumor cells imitated cells of the synovial membrane and we found no evidence for origin from cells of the nerve sheath. Because of the submicroscopic relationship and histochemical similarities of synovial sarcomas and mesotheliomas we suggest that they should be united in a group of sarcomas with possible biphasic cellular pattern, while preserving their clinicopathologic definition.

Unusual leiomyoma of vulva with fibroma-like pattern and pseudoelastin production.

A unique case of a 71-year old woman with an unusual fibroma-like leiomyoma of the vulva in the region of the left Bartholin's gland is reported. Light microscopically the tumor resembled a fibroma, but electron microscopically the cells corresponded to modified smooth muscle cells. The great number of vessels and their intimate relation to the tumor cells is remarkable. In the intercellular space granular and fibrillar pseudoelastic material was found and thus the presence of collagen type III in addition to type I is suggested. In the region of the right Bartholin's gland a mesenchymal proliferation with similar histological features was seen but there was more resemblance to a conventional leiomyoma. The histogenesis remains obscure so that the designation of these lesions should include light microscopic and electron microscopic appearances as well as localization.

Myofibroblasts in connective tissue capsules around implanted dental materials.

For testing the biocompatibility of dental materials in an experimental animal model the capsular tissues around the implanted dental materials were examined light and electron microscopically. The exudative inflammation and fibrosis at different times after implantation were thought to be the indicators of the degree of the compatibility. Starting from the knowledge of the occurrence of myofibroblasts in reparative granulation tissue we looked for such cells in the capsular tissues. Myofibroblasts were especially found at three weeks after onset of the experiments. Structural peculiarities of this fibroblast modulation are emphasized. Myofibroblasts are considered as an essential link between the early exudative reaction and the late fiber formation in tissue reactions after mechanical injury.
