Efficacy of acyclovir combined with immunopotentiating agents in the treatment of varicella-zoster.

In-vitro and in-vivo evaluation of cellular immune reactions (T-cell subset, E-rosette formation, 2,4-dinitrochlorobenzene, were made in patients with varicella-zoster infection (VZ) and with other intracellular infections. The data demonstrated depressed cellular immunity including a decrease in the number of T-helper cells and a fall in the T-helper/T-suppressor lymphocyte subset ratio to less than 0.7. Three groups of patients with VZ infections were randomly assigned to three regimens: nine patients were given acyclovir alone for five days; ten patients received acyclovir for five days and isoprinosine for ten days; and twelve patients acyclovir for five days and levamisole twice weekly for three weeks. In patients with VZ infections treated with acyclovir and either levamisole or isoprinosine cellular immunity improved faster, while VZ infections in those treated with levamisole healed more rapidly. One patient treated with acyclovir alone had recurrent VZ infection and required a further course of therapy.

Ofloxacin, a new quinolone in the treatment of genitourinary and enteric infections.

During 1984 to 1986, 121 patients with gonorrhoea were treated with ofloxacin. This report is to summarize the results of our trial in patients treated with three different dose regimens of ofloxacin: A) a single 800 mg dose of ofloxacin (32 patients); B) a single 400 mg dose of ofloxacin (55 patients) or C) a twice a day dose of 600 mg ofloxacin (34 patients). In addition, 19 patients with urinary tract infection and eight patients with enteric fever were treated with 400 mg ofloxacin twice a day for seven to ten days. Out of 121 patients with gonococcal urethritis, only 107 patients were evaluable. Cure was obtained in 96.5%; 93.2% and 94.5% of the patients in groups A, B and C respectively. 16 out of 17 patients with urinary tract infection achieved clinical cure or improvement. The 16 patients were bacteriologically accessible; pathogens were eradicated in all of them. Ofloxacin was also effective in the treatment of enteric fever, in which eradication was achieved in all seven patients. 28% of Neisseria gonorrhoea and 29% of Salmonella spp. isolated in our community were beta-lactamase-positive.

Use of temocillin in typhoid fever, hepatobiliary disease and other infections.

During 1980-1984, 51 patients with enteric fever (typhoid/paratyphoid fever and salmonellosis) biliary tract sepsis, and other Gram-negative infections were treated with temocillin at 3 medical centres in Bangkok, Thailand. This article summarises the results in the 42 evaluable patients (14 males, 28 females); 10 patients with enteric fever, 2 patients with salmonellosis; 12 patients with biliary tract sepsis, and 18 patients with other Gram-negative infections. Overall, 86% of the patients were clinically cured or improved. Bacteriological eradication or marked reduction in the number of organisms was achieved in 95% of the patients. In 10 cases of enteric fever and 2 cases of salmonellosis all bacterial pathogens were eradicated, but 1 case of Salmonella paratyphi infection failed clinically. Eleven of 12 biliary tract infections were clinically cured or improved, and organisms were eliminated from all 10 bacteriologically assessable patients. The majority of patients received 1 to 2g of temocillin given by intravenous infusion twice daily for 5 to 14 days. Temocillin was well tolerated by all patients.

A case comparison of acquired immune deficiency syndrome (AIDS) in homosexual males with spindle-endothelial cell abnormalities and with recrudescent melioidosis.

The AIDS syndrome includes cases of biopsy-proven Kaposi's sarcoma in persons under 60 years of age, or biopsy- or culture-proven Pneumocystis carinii pneumonia, or either of the life-threatening opportunistic infections in young previously healthy persons with no underlying cause of immunodeficiency (Center for Disease Control criteria). Here we described the first case of AIDS with early Kaposi's sarcoma-like lesions in homosexual male drug addict and have compared the clinical and laboratory findings with those of another homosexual male having recrudescent melioidosis due to Pseudomonas pseudomallei.

Cellular immunity (T-cell subset using monoclonal antibody) in tuberculosis, melioidosis, pasteurellosis, penicilliosis; and role of levamisole and isoprinosine.

In vitro and in vivo evaluation of cellular immunity (T-cell subset--using monoclonal antibody--immunofluorescent staining technique; E-rosette formation to 2.4-Dinitrochlorobenzene reaction) were made in tuberculosis, melioidosis, pasteurellosis and penicilliosis. The data demonstrate depressed cellular immunity especially a significant decrease in numbers of T-helper cells and T-helper/T-suppressor subset ratio fell to less than 1.0. Of greater importance, in tuberculous patients and patients with melioidosis treated with levamisole, 150 mg/day, twice a week, along with a specific antitubercular or antimicrobial agent for appropriate duration, improved faster both clinically, microbiologically and in cellular immunity.

Cellular immunity in typhoid fever, Legionnaires' disease, amebiasis: role of transfer factor and Levamisole in typhoid fever.

Typhoid fever is an infectious disease commonly seen in the tropics, with multisystem involvement and a high morbidity and mortality rate. Legionnaires' disease: a newly described acute respiratory infection by unusual aerobic gram-negative micro-organisms namely Legionella pneumophila. Cellular immunity: in vitro and in vivo evaluations of cellular immunity using E-rosette formation (E) and 2.4-Dinitrochlorobenzene (D) reaction were made in typhoid fever, amebiasis and Legionnaires' disease. Results will be presented. Three patients with relapsing typhoid fever were given transfer factor and another group with typhoid fever were given Levamisole with sulfamethoxazole-trimethoprim. Up to 90% of the cases receiving immunopotentiating factors/agents improved faster in both general condition, fever and cellular immunity.

The combination of pivmecillinam and pivampicillin compared to co-trimoxazole in the treatment of enteric fever.

Typhoid fever is an infectious disease with multisystem involvement and is commonly seen in the tropics. Twelve patients with acute typhoid fever were successfully treated with a fixed dose combination of pivmecillinam and pivampicillin. The treatment results were compared to those obtained from the treatment of ten other patients with co-trimoxazole, which is the routine treatment of our Department. The two forms of treatment appeared to be equally effective, suggesting that the combination mecillinam/ampicillin may represent a valuable alternative to the antityphoidal drugs currently available. Eleven patients were infected with strains resistant in vitro to either ampicillin, chloramphenicol, or both. All clinical isolates were sensitive to co-trimoxazole and to the combination of mecillinam and ampicillin. MIC values for the combination ranged from 0.16 to 2.5 mg/l. No side-effects were recorded.
