Effects of pattern of ventilation on pulmonary metabolism and mechanics.

Effects of three patterns of mechanical ventilation on pulmonary mechanics, lung phospholipid and surface activity were studied in the normal closed-chest dog. The patterns were continuous mechanical ventilation with: 1) tidal volume (VT)=15 ml/kg; 2) VT=15 ml/kg with 10 cm H2O positive end-expiratory pressure (PEEP); 3) VT=50 ml/kg. THE DOGS IN EACH GROUP WERE VENTILATED FOR 24 HOURS, WITH CAREFUL ATTENTION PAID TO MAINTENANCE OF NORMAL BLOOD GASES, FLUID BALANCE, AND CARDIAC OUTPUT. The animals were sacrificed and the lungs studied to determine pressure-volume curves, dry lung weight/wet lung weight ratios, phospholpid contents and surface activities. The results were compared with control values in acutely sacrificed unventilated dogs. No significant change from controls was found with any pattern of ventilation employed with the exception of the tendency for lungs ventilated with PEEP to retain fluid (decreased dry lung weight/wet lung weight ratio).

Program accreditation and correctional treatment.

In the correctional field, treatment program accreditation requires the support of correctional administrators and program providers for successful introduction. How accreditation criteria are developed and a support structure for the process in corrections is achieved is in itself an interesting story. Her Majesty's Prison Service has, in 3.5 years, accelerated the effectiveness of correctional treatment programming, established a platform for program integrity, obtained acceptance by the institutional leadership, and increased pride and morale among prison officers. In this article we describe the development, structure, content, and benefits of correctional treatment program accreditation as it has occurred in England and Wales.

The plantar transverse incisional approach for heel spur syndrome. A retrospective study.

The efficacy of the transverse plantar incisional approach for the treatment of recalcitrant heel spur syndrome or plantar fasciitis was investigated by evaluating cases of this procedure performed by the authors from 1991 to 1998. Patient records were reviewed for conservative treatment rendered prior to surgical intervention as well as for the perioperative course of the patient. All patients were asked to complete questionnaires regarding their heel spur syndrome or plantar fasciitis and their opinion of both the conservative and the surgical treatments received.

The split peroneus longus lateral ankle stabilization procedure.

The authors present an interesting approach to lateral ankle stabilization using the peroneus longus tendon. The more commonly utilized procedures for instability of the ankle complex, rationale for use of the peroneus longus tendon, and the details of the split peroneus longus lateral ankle stabilization procedure are presented. When evaluating patients with lateral ankle joint instability, the subtalar joint (STJ) complex needs to be assessed. In addition, the role of STJ instability and its relationship to lateral ankle joint instability are discussed.

Intraosseous ganglion of the cuboid bone.

Intraosseous ganglia are benign cystic lesions usually located in the end of long bones, especially the distal tibia. Patients typically present with mild, localized, aching discomfort in a joint that is aggravated by weightbearing. Pathogenesis is currently unknown with the literature suggesting many valid theories. Surgical excision and curettage is the treatment of choice. A case of a ganglion cyst within a cuboid bone is presented along with a review of the literature and discussion of operative technique.
