A randomized trial on the use of ultrasonography or office hysteroscopy for endometrial assessment in postmenopausal patients with breast cancer who were treated with tamoxifen.

OBJECTIVE: Our purpose was to compare the screening effectiveness and acceptability of transvaginal ultrasonography (with sonohysterography if endometrial thickness was >4 mm) with office hysteroscopy. STUDY DESIGN: This randomized crossover study comprised 53 consecutive asymptomatic (without vaginal bleeding) postmenopausal patients with breast cancer who had taken tamoxifen (20 or 40 mg/day) for at least 6 months. RESULTS: Two patients had endometrial cancer (1 primary, 1 breast secondary), both detected only by transvaginal ultrasonography. Twenty-six patients had at least 1 polyp (total 47, all benign). The women who had polyps were not significantly different in their age, body mass, months of tamoxifen intake, or the cumulative dose. The sensitivity and specificity of transvaginal ultrasonography were 85% and 100%, respectively. The corresponding values for office hysteroscopy were 77% and 92%. It was significant that more patients preferred transvaginal ultrasonography to hysteroscopy (P < .001). CONCLUSION: Transvaginal ultrasonography (plus sonohysterography) may be more effective and acceptable than office hysteroscopy for detecting endometrial abnormalities in women taking tamoxifen.

Anaplastic nodules in an ovarian mucinous tumor. Case report and literature review.

The case of a 38-year-old woman with an ovarian mucinous cyst that contained solid nodules of anaplastic carcinoma is described. Hitherto, only 17 cases of this aggressive type of ovarian tumor have been reported. The patient underwent a total abdominal hysterectomy with bilateral salpingo-oophorectomy and a para-aortic and pelvic lymphadenectomy. The tumor was classified as FIGO Stage Ic. On flow cytometry the tumor was DNA diploid. Also, on image analysis the regions with well- differentiated carcinoma and the regions with anaplastic nodules were DNA diploid. No adjuvant treatment was given. There is no evidence of disease 30 months after diagnosis.

Meigs' syndrome with elevated serum CA 125 levels: two case reports and review of the literature.

Two cases of Meigs' syndrome in association with elevated serum CA 125 levels are reported. The significance of Meigs' syndrome lies in the fact that neither ascites nor pleural effusion is necessarily an ominous sign in women with a pelvic tumor. Although there is a strong correlation between ovarian malignancy and elevated serum CA 125 levels, several benign conditions have been found to cause a rise in CA 125 levels. It is important to remember that a pelvic neoplasm in a woman presenting with hydrothorax, ascites, and elevated CA 125 levels might be benign and that this condition can rapidly be resolved with surgical removal. Neither ultrasound examination nor computed tomography can reliably offer a preoperative diagnosis.

Renal transport of netilmicin in the rat.

We examined the renal transport of [3H]netilmicin in the rat using clearance, microinjection and free flow micropuncture techniques. Netilmicin was constantly infused at rates ranging from 1.92 to 192 nmol/min and resulted in plasma concentrations of 0.4 to 40 micrograms/ml and fractional excretion rates of 94.5 +/- 3.3 to 107.9 +/- 1.7%. Although the mean fractional excretion rate of netilmicin was equal to or exceeded the filtered load, the drug continued to accumulate in renal cortex albeit in a curvilinear manner and at the highest rate of infusion attained a peak level of 447 +/- 26 nmol/g of renal cortex. The renal cortical uptake of netilmicin was inhibited in a dose-dependent fashion by the simultaneous infusion of gentamicin. In response to microinjection of [3H]netilmicin (390 +/- 56 pg) 82.0 +/- 1.8% of the dose was recovered in the urine after early proximal tubular injections, 90.5 +/- 1.1% of the dose was recovered after late proximal tubule injections and 98.5 +/- 1.3% of the dose was recovered after early distal tubule injections. In free flow micropuncture experiments the fractional delivery of netilmicin to the early proximal tubule measured 1.16 +/- 0.03, a value significantly greater than 1.0 (P less than .005). No significant change in fractional delivery was evident to the late proximal tubule (1.13 +/- 0.04), whereas it decreased to 1.02 +/- 0.04 (P less than .01) at the early distal tubule.(ABSTRACT TRUNCATED AT 250 WORDS)

Predicting the clinical behavior of ovarian cancer from gene expression profiles.

We investigated whether prognostic information is reflected in the expression patterns of ovarian carcinoma samples. RNA obtained from seven FIGO stage I without recurrence, seven platin-sensitive advanced-stage (III or IV), and six platin-resistant advanced-stage ovarian tumors was hybridized on a complementary DNA microarray with 21,372 spotted clones. The results revealed that a considerable number of genes exhibit nonaccidental differential expression between the different tumor classes. Principal component analysis reflected the differences between the three tumor classes and their order of transition. Using a leave-one-out approach together with least squares support vector machines, we obtained an estimated classification test accuracy of 100% for the distinction between stage I and advanced-stage disease and 76.92% for the distinction between platin-resistant versus platin-sensitive disease in FIGO stage III/IV. These results indicate that gene expression patterns could be useful in clinical management of ovarian cancer.
