Use of activated carbon and natural zeolite as support materials, in an anaerobic fluidised bed reactor, for vinasse treatment.

In Cuba, the alcohol distillation process from cane sugar molasses, produces a final waste (vinasse), with an enormous polluting potential and a high sulfate content. Applying the anaerobic technology, most of the biodegradable organic matter can turn into biogas, rich in methane but with concentrations of sulfide above 1%. The present work develops two experiences with anaerobic fluidized bed reactors (AFBR) using both Cuban raw material, activated carbon and natural zeolite, as support media, with the purpose of obtaining high organic matter removal rates and keeping sulfide and ammonium concentrations in the permissible ranges. The reactors were operated during 120 days, achieving an organic loading rate of 10 kg COD/m3 day, with COD removal above 70%, and a methane production of 2 L/d. The activated carbon and natural zeolite used support materials in anaerobic fluidized bed reactors, and showed good results of distillery waste removal.

[The severity of airport malaria].

Report of a new case of airport malaria with renal failure. The evolution of the thirty cases previously described is reviewed. Most of the time, airport malaria seems to be a severe infection.

[Polyneuropathy and solitary bone plasmacytoma. A new case].

The authors report a case of acute polyneuropathy revealing a solitary osseous plasmacytoma with osteo-dense and osteolytic bone lesions. Initially, the rapid progression of the sensory and motor loss led to treatment by plasma exchanges and irradiation of the plasmacytoma. Four months later, despite a significant improvement of the neurological condition, serum protein electrophoresis continued to show a peak of monoclonal immunoglobulin. Chemotherapy with cyclophosphamide and prednisone was administered for one year whilst the neuropathy continued to regress. This case, which presents many classical features of plasma cell dyscrasia (polyneuropathy with albumino-cytological dissociation, radiological osseous condensation, low concentrations of lambda light chain protein), illustrates some unusual features of solitary plasmacytomas associated with peripheral neuropathy: the young age of our patient, an acute progression of the neuropathy in the early stages, tumoral localisation in the diaphysis of a long bone.

Open heart surgery in patients with end-stage renal disease.

Seventeen patients on maintenance dialysis therapy or with severe impaired kidney function required correction of cardiovascular diseases by open heart surgery. Ten aortic, two mitral valvular replacements and five coronary artery bypass grafts were performed. Special attention was taken for these patients, including pre-operative dialysis, cardiopulmonary bypass with complete haemodilution, composition of extracorporeal circuit volumes, arterial and venous access and myocardial protection. Prosthetic valves were preferred to porcine heterografts for valve replacements and autogenous saphenous veins were utilised for coronary artery bypass grafts. Follow-up of 14 patients was available from one to 84 months. Carefully planned open heart surgery can be successfully carried out in uraemic patients without an increased operative risk.

Side effects of therapeutic plasma exchange during treatment of polyarteritis nodosa. Comparison of filtration and centrifugation. 718 sessions in 63 patients.

Between 1981 and 1984, 72 patients (22-75 years) were included in a prospective study for treatment of Polyarteritis Nodosa (PN) which associated therapeutic plasma exchanges (TPE), corticosteroids (CS) 1 mg/kg/day, and a randomized trial of cyclophosphamide (CP) 2 mg/kg/day. We observed the incidence and analysed side effects (SE) occurring during TPE considering the technology of plasma removal. Full data were available for 63 patients. Seven hundred and eighteen TPE were performed. Centrifugation was used in 594 PE (82.7 per cent) with intermittent flow centrifugation (IFC) 320 times (44.5 per cent) and continuous flow centrifugation (CFC) 274 times (38.2 per cent). Filtration (F) was used in 124 sessions (17.3 per cent). Replacement fluid was 4 per cent albumin in 650 TPE and fresh frozen plasma (FFP) in 89 TPE. A total of two hundred and eleven SE were reported in 53 patients (84.1 per cent) during 173 TPE (24.1 per cent). Forty-six TPE were temporarily stopped because of SE (6.4 per cent). The mean volumes of removed plasma (ml/kg/TPE) were: 60.7 +/- 8.7 ml/kg when CFC was used 54.5 +/- 20 ml/kg with F and 52.5 +/- 9.6 ml/kg with IFC (n.s.) Main SE were: technical difficulties in 80 TPE, moderate or severe hypotension in 47, allergy to replacement fluid in 39. Hepatitis B antigen appeared in one patient. No death occurred during TPE and SE were usually minor and transient.(ABSTRACT TRUNCATED AT 250 WORDS)
