Serum sperm antibodies unrelated to mumps orchitis.

In order to determine whether there is an association between mumps orchitis and serum sperm antibodies, we tested patients at the time orchitis was diagnosed (n = 7) and individuals who had had orchitis at least 1 month previously (n = 14). Data were compared with the results for a control group of blood donors (n = 20). Sperm antibodies were detected by the gelatin agglutination test of Kibrick, the tray agglutination test of Friberg and the ELISA. Clinically significant sperm antibody levels were not found in patients in the early stages of the disease. Four subjects tested post-disease were positive in the Friberg test and one was positive in the ELISA. One control serum was also positive in the latter test. Significant differences were not found between levels in patients in the early stages of the disease and levels in individuals post-disease, although the results of the Friberg test differed significantly between controls and former mumps orchitis cases. These data do not support the assumption of an involvement of humoral immunity against spermatozoa in mumps orchitis patients.

[Lipid and mineral metabolism and morbidity with temporary loss of work capacity in miners].

Examination is performed on 87 miners from mine for copper production subject to intensive local vibrations with average rate of increase (K) of the norms for the individual octave frequencies 2.4 times for 3 h exposure of the working shift, as well as to intensive noise--113-115 dB/A equivalent level, general dust--23 mg/m3, fine dust--3.5 mg/m3 and fine quartz--0.43 mg/m3. There are data for vibration effect--changes in the cold test, vibration sensoriness, ultra sound sonometry, in all miners. In a significant part of them are established dyslipoproteinemia--increase of triglycerides, total and LDL cholesterol, values beyond reference (mainly above the norm) of 10 electrolytes and microelements in blood serum (blood). An extremely high prevalence of the cases and the days of total morbidity with temporary disability (respectively 189.7 and 2935 per 100), with predominance in its structure of diseases of the central and peripheral nervous systems, hypertension disease, etc. The importance of vibrations for disorder in the metabolic processes in the organism and development of temporary diseases are underlined.

[Gamma-chamber testicular scintigraphy in the combined diagnosis of Morris' syndrome].

The authors examined 15 patients with the syndrome of Morris by gamacamarated scintigraphy and they succeeded in depicting the gonads, found in the abdomen and their topographic localization. Grave structural changes were established in the testes during the performance of gamacamerated scintigraphy, but in 3 patients only one testis was depicted, which suggested atrophy and lack of functional parenchyma in the gonad, which was not discovered. Conclusive scintigraphic examination of topographic localization as well as of structural disturbances of the parenchyma of the testis replaced testicular biopsy, which requires laparotomy. Gamacamerated scintigraphy could establish malignant growth of the gonad and could help to remove it in time by operation.

[The combined treatment of male infertility in patients with idiopathic varicocele].

The author reports on 1066 patients with idiopathic varicocele and fertility disturbances, in whom surgical intervention was made by the method of Ivanissevich--a high resection of the internal testicular vena. Stimulating hormonal therapy of spermatogenesis with gonadotropic preparation Pregnyl was used according to a scheme in 246 patients, nonaffected by surgical treatment with a number of spermatozoa under 40 millions/cm3. Improvement of spermatozoa parameters was found in 31.70% of the operated patients; pregnancies of their wives occurred in 15.85% of the examined men.

[Clinico-morphological studies of epididymal cysts].

A clinico-morphologic study of epididymis cysts was made in 52 patients at mean age 39.4 years, observed over a period of 2 ys. 4 mos. In approximately half of the cases (49.2 per cent) the cysts were localized in the left epididymis. Four types of histologic structure were distinguished: 1. smooth outlines of the lumen, filled with spermatozoa, and wall built up of fibrous tissue and different proportion of smooth- muscle, elastic and reticular fibers, and lined with one-row ciliary columnar or cubic epithelium; 2. similar to the former structure, but without spermatozoa in the lumen; 3. stellate-outlined lumen without spermatozoa in it, with a wall built up of connective tissue and lined with one-row ciliary columnar epithelium; 4. the wall is stellate-folded, built up of loose connective tissue and lined with one-row cubic epithelium, forming micropapillary structures, without presence of spermatozoa. Considerable difficulties are pointed out in determining the types of cysts according to the nature of their epithelial lining and the structure of their wall. Precise classification is possible when thorough clinico-morphologic parallel is used.

[The level of total estrogens in the urine of cryptorchid patients].

The authors carried out studies on total estrogens in urine of healthy men as well as in men with cryptorchidism. These studies were performed on 79 men, divided into the following groups: a control group of healthy men--31; nonoperated patients--5; operated patients with cryptorchidism up to the seventh years of age--7; operated patients with cryptorchidism between 7 and 12 years of age--15; operated patient with cryptorchidism over 12 years of age--21. The hormonal analysis of total estrogens in urine were made by the modified fluorometric method of Braun. The highest level of total estrogens was found by the authors in the men operated men with cryptorchidism, presenting mean values of means = 42, 1 M/mol for 24 hours. The results make strong impression that in all groups with of patients with cryptorchidism, operated and nonoperated, the total estrogens are increased twice in comparison with the control group of healthy men. These results question the role of the increased level of estrogens in the pathogenesis of distributed spermatogenesis and lacking fertile capability in men with cryptorchidism.

[Fertility after the surgical treatment of cryptorchism].

The authors carried out a study on the ejaculates of 85 men at the age of 16 to 38 years, who underwent surgical intervention for treatment of unilateral and bilateral cryptorchidism. The patients were divided into two basic groups: orchidopexy was made in 66 patients with cryptorchidism and unilateral orchidectomy was performed in 19 patients with atrophy of the cryptorchidic testis. Surgical treatment of cryptorchidism improved fertile capability in 15.30% of the patients. The high percentage of 84.70% of the infertile patients after surgical treatment of cryptorchidism raises the question for earlier surgical treatment of cryptorchidism up to 5 years of age. Orchidectomy is indicated in abdominal or highly retained in the inguinal canal testis with very short blood vessels. Orchidectomy is also indicated in patients with atrophic testes, in whom perspective for fertile capability lacks and presents high risk of malignization.

[Congenital anorchism--the diagnostic and treatment problems].

The authors prove congenital anorchism by operative way as they recommend to think of congenital anomaly of testes--anorchism in all patients with lacking testis in the scrotum and inguinal canal. This anomaly is established in three patients. They propose a double hormonal stimulation with choriongonadotropin according to a fixed scheme as a basic diagnostic method in men with congenital anorchism. It is necessary to begin timely substitutive androgenic therapy in men with established anomaly. Usage of testicular prosthesis for correction of the cosmetic defect is one of the therapeutic stages of this congenital state. The prosthesis is of great psychological advantage in men with anorchism. Transplantation of testes is a future therapeutic method in patients with anorchism.

[Spermatological disorders in patients with postmumps orchitis].

The author studied spermatozoa parameters in 27 patients, undergone postparotitic orchitis with fertile problems. Unilateral localization of the damaged testis was established in 21 men, but bilateral localization--in 6 men. The author divided the patients into two groups--the first group included patients, who underwent orchitis before 10 years of age and the second group of patients, who underwent the complication orchiepididymitis after 10 years of age. After examination of the ejaculate he found disturbances following disturbances with unilateral localization of orchitis in the first and second group in respect to the number of spermatozoa mil/ml in the following ratio 32:12, the ration of spermatozoa motility, described in percentage, was 39:10, but the ratio of velocity mic/sec was 8:5. Spermatogram showed azoospermia with definitive sterility in both groups of patients, who had bilateral localization of postparotitic orchitis.

[The incidence of idiopathic varicocele among andrological patients].

The author carried out a study on 9753 andrologic patients, as idiopathic varicocele was established in 2102 (21.55%). In respect to age, the disease involved men most frequently between 21-30 years of age and this comprised 58.00% of the cases. In respect to family status he established that 77.00% of the cases were married without children, 6% of them were married with children and 17% of them were unmarried. In respect to profession the disease involved men with technical profession on the first place (34.00%), employees on the second place (14.70%) and chauffeurs on third place (12.00%). In conclusion the author indicates that the frequency of idiopathic varicocele is over 20% as 86% of men during fertile age are affected, but 77.00% of the examined cases suffer from disturbed fertility.

[Incidence of hypertension and ischemic heart disease with temporary disability among workers exposed to vibration].

The article reveals the pathologic changes in man caused by vibration and detects morbidity with temporary disability (MTD), hypertension disease (HD), and ischemic heart disease (IHD) in vibration-related professions in ore extraction, metallurgic industries, etc. The morbidity data used are established on vascular tension, neuro-humoral regulation and lipid metabolism changes, and the results obtained are given in comparison with the nation-wide computerized MTD-data for 1982-1986 and those for the Communication branch. Considerably high HD and IHD incidencies have been revealed in vibration-related professions and compared to the sample branch. Computerized systems' possibilities are evaluated in MTD occupational risk-factors assessment.

[Immunological studies of patients with chronic nonspecific prostatitis and infertility].

The authors studied carried out immunological studies on 20 men with chronic nonspecific prostatitis at reproductive age as well as on 10 healthy men included into the control group. Correlative investigations were made on the morphology of the ejaculate as well as on immunological parameters of seminal plasma aiming to establish the degree of the disturbed fertility. The immunological investigations included determination of albumin, transferrin and immunoglobulins in the seminal plasma. In men with chronic prostatitis there was a reduction in the number of spermatozoa up to 39 mln/cm3 combined with lower motility and an increased percentage of teratoforms. The immunological studies showed statistically significant nonincreased values in all men with chronic prostatitis in comparison with healthy men of the control group. These results show that increased permeability of barriers of blood serum proteins participates in the pathogenesis of the disturbed fertility. It is apparent that the cause for sterility in the married couples is due to autoimmune process occurring in men with chronic nonspecific prostatitis.

[The current concept of tumor markers in oncologic urology--cytoplasmic markers].

The cells of testicular tumors and other malignant processes synthesize specific proteins which may be demonstrated intracellularly and may be quantitated in the patient serum. Of major value as markers in testicular tumors are alpha-fetoprotein and human choriongonadotropin. Radioimmune methods allow detection of minimal amounts of these substances at a moment when there are on other criteria for the presence of malignant process in the organism. This helps to establish early diagnosis and detection of relapses, precise determination of the stage and monitoring of treatment. Methods have also been developed for intracellular localization of the tumor markers, which allow precise diagnosis at cell level and retrospective studies on fixed biopsy specimens. The immunohistochemical classification which has been established and the experience being gained allowed the approach to the treatment of testicular tumors to be based in a major extent to the tumor markers.

[The treatment of male infertility with the preparation vitaton].

Thirty men with fertility disorders, 21 to 42 years of age (mean 29 years) and mean duration of sterile matrimony 4 years 7 months received Vitaton treatment. The patients were divided in three groups: 8 with cryptorchidism, 12 having undergone resection of v. testicularis interna sin for varicocele and 10 with idiopathic oligospermia. In the first group of patients with bilateral cryptorchidism and azoospermia the control spermatograms showed persistence of azoospermia, i.e. in none of these patients was fertility reestablished. The second group of patients who had been operated for idiopathic varicocele experiences significant improvement in spermatozoid motility. In the third group (with idiopathic oligospermia) spermatozoid viability was improved--more than 50 per cent motility and more than 10 mu/sec speed of the spermatozoa. Application of Vitaton in andrologic practice gives justifiable hopes for improvement of spermatozoid motility and speed, which are the most important parameters for male inseminating capacity.
