Adenocarcinomas in the cervicovaginal Papanicolaou smear: analysis of a 12-year experience.

We attempted to reassess the significance of cervicovaginal Papanicolaou smear(s) in identifying adenocarcinoma (AC) by comparing Papanicolaou smears with histopathology in a pool of 188 patients. The first part of study dealt with 95 patients, seen over a 12-yr period (1975-1986), who had tissue diagnosis of cervical or endometrial AC, all with prior papanicolaou smears available. Of the 95 patients, Papanicolaou smears were positive and/or inconclusive for uterine AC in 37 (38.9%) and falsely negative in 58 (61.1%) patients. Of the latter, six were either positive and/or inconclusive for squamous carcinoma and moderate to severe dysplasia; thus, the effective detection rate (sensitivity) for malignancy and related condition of Papanicolaou smears was 45.3% (43 of 95). The second part of the study concerned the analysis of 130 patients with 148 Papanicolaou smear in whom cytologic diagnosis positive and/or inconclusive for AC was made during the same study period. Tissue diagnoses were available in 107 of these as follows: AC and glandular hyperplasia in 66 patients (61.7% predictive value), with 11 (10.3%) from extrauterine sources; squamous carcinoma in 12 and moderate to severe dysplasia in 5, totalling 17 patients (15.9%); miscellaneous conditions (polyps, radiation change, and glandular atrophy) in 11 patients (10.3%); and no pathologic change in 13 patients (12.1%). The effective predictive value of positive and/or inconclusive Papanicolaou smears for all malignancy and related states is 77.6%.

The C region of human insulin-like growth factor (IGF) I is required for high affinity binding to the type 1 IGF receptor.

We have produced and characterized the binding properties of three structural analogs of human insulin-like growth factor I (hIGF-I). These analogs are [1-62]hIGF-I, an analog lacking the carboxyl-terminal 8-amino acid D region of hIGF-I; [1-27, Gly4, 38-70]hIGF-I, an analog in which residues 28-37 of the C region of hIGF-I are replaced by a 4-reside glycine bridge; and [1-27,Gly4,38-62]hIGF-I, an analog with the C region glycine replacement and a D region deletion. The removal of the D region of hIGF-I has little effect on binding to the type 1 and type 2 insulin-like growth factor (IGF) receptors. [1-62]hIGF-I has 2-fold higher affinity for the insulin receptor and 4-fold higher affinity for IGF serum-binding proteins. The replacement of the C region of hIGF-I with a four-glycine span results in a 30-fold loss of affinity for the type 1 IGF receptor. However this analog has near normal affinity for the type 2 IGF receptor, the insulin receptor, and IGF serum-binding proteins. Incorporating the C region glycine replacement and the D region deletion into one analog does not affect binding to either the type 2 receptor or to IGF serum-binding proteins. As predicted from the single deletion analogs [1-27,Gly4,38-62]hIGF-I has reduced affinity for the type 1 IGF receptor (approximately 40-fold) and increased affinity for the insulin receptor (5-fold). These data indicate that determinants in the C region of hIGF-I are involved in maintaining high affinity binding to the type 1 IGF receptor and that neither the C region nor the D region are required for high affinity binding to the type 2 IGF receptor or to IGF serum-binding proteins.

Myocardial infarction and normal coronary arteriography: a 10 year clinical and risk analysis of 74 patients.

Myocardial infarction with normal coronary arteries was identified in 74 patients with a mean age of 43 years (range 19 to 66). A mean follow-up period of 10.5 years after documented myocardial infarction and 8.6 years after cardiac catheterization was obtained. The survival rate was 85% (n = 63). There were no statistical differences in age or clinical risk factor prevalence between survivors and nonsurvivors. Moderate (55%) to severe (27%) left ventricular impairment was more common in nonsurvivors. Nine of 11 deaths were cardiovascular, 6 were sudden and 8 occurred in patients with moderate to severe global left ventricular impairment. Seventy-six percent of survivors were asymptomatic and 86% were fully active at follow-up. Two survivors and three nonsurvivors experienced a second myocardial infarction. The clinical risk factors of the study group (Group I) were compared by age, sex and year of catheterization with risk factors in two matched groups. Group II consisted of 74 patients with coronary occlusive disease and myocardial infarction and Group III consisted of 148 patients with normal arteriograms. Group I differed from Group II in having fewer clinical risk factors (p = 0.01 to less than 0.0001). Cigarette smoking did not differ significantly between Group I (72%) and Group II (69%) but was less common in Group III (45%) (p less than 0.001). Hormone therapy or the peripartum state was more common in women in Group I (34%) than in women in Group III (14%) (p = 0.03).(ABSTRACT TRUNCATED AT 250 WORDS)

Evaluation of slow-release formulations of temephos (Abate) and Bacillus thuringiensis var. israelensis for the control of Aedes aegypti in Puerto Rico.

Formulations of temephos (Abate) and Bacillus thuringiensis var. israelensis (B.t.i.) on corncob and dried coconut husk carriers were tested for slow-release insecticide properties against Aedes aegypti larvae in Puerto Rico. Granular formulations of 5% and 10% temephos gave continuous larval control in used automobile tires for 27 to 124 days and 34 to 162 days, respectively. Temephos on radial sections of corncob and coconut husk chips gave good larval control in tires for 27 to 63 and 61 to 134 days, respectively, depending on the size of the carrier. Small (2-3 g) and large (3-5 g) coconut husk chips tested in 167 liter drums provided continuous control for 55 to 105 days, respectively. Granular formulations of B.t.i. controlled Ae. aegypti in tires for 19 to 33 days, and B.t.i. briquets exhibited larvicidal activity in large containers for 26 to 78 days.
