Myocardial fibrin deposition in experimental viral myocarditis that progresses to dilated cardiomyopathy.

Myocardial fibrosis is a characteristic late feature in cases of viral myocarditis that progress to dilated cardiomyopathy. However, the pathogenesis of the myocardial fibrosis in such cases is unknown. Prior studies have shown that in healing wounds and tumor stroma generation, interstitial fibrin deposition precedes the development of fibrosis. Therefore, interstitial fibrin deposition in the myocardium was investigated in a murine model of myocarditis in which dilated cardiomyopathy develops. Inbred male C3H/He mice inoculated with coxsackievirus B3 were killed 0, 3, 7, 14, 21, 30, and 60 days after infection. Paraffin sections of hearts were stained with hematoxylin-eosin, Masson's trichrome stain, and antibodies to fibrinogen/fibrin by use of an immunoperoxidase technique. Pretreatment of all mice with anticoagulants and antifibrinolytics 5 minutes before death was used to prevent artifactual fibrin deposition and fibrinolysis during tissue manipulation. Tissue fixation in formalin supplemented with acetic acid served to extract non-cross-linked fibrin, fibrinogen, and fibrinogen and fibrin degradation products, thus ensuring that clotted and cross-linked fibrin was the major immunoreactant. Myocardial fibrin deposition and fibrosis were each quantitated by computer-assisted image analysis. Myocardial fibrin deposition first appeared on day 3, was maximal on day 14, and disappeared by day 30. Conversely, myocardial fibrosis was not detectable until day 14 and was maximal at day 60. Thus, as in healing wounds and developing tumor stroma, fibrin deposition preceded fibrosis in this murine model of myocarditis that progresses to dilated cardiomyopathy.

How thoroughly should needle localization breast biopsies be sampled for microscopic examination? A prospective mammographic/pathologic correlative study.

To investigate the relationships between mammographic calcifications and pathologic lesions, and to develop guidelines for specimen sampling, we prospectively studied 157 consecutive needle localization breast biopsies (NLBB) using a technique in which radiographic findings in sectioned specimens were correlated with the histologic findings. All NLBBs were performed because of mammographic microcalcifications without a soft tissue density, and macroscopic examination in each case failed to reveal a gross lesion. All specimens were submitted in their entirety. Microscopically, 32% of the cases showed carcinoma, 12% demonstrated atypical hyperplasia, and 56% consisted of benign breast tissue without atypia. Forty-nine of the 50 cancers (98%) and 14 of the 19 atypical hyperplasias (74%) were associated with the radiographic calcifications. If histologic examination had been restricted to areas of the specimens containing the radiographic microcalcifications and the remaining tissue submitted only for cases of carcinoma or atypical hyperplasia on the initial sections, 38% fewer tissue blocks would have been processed, but one case of noncomedo ductal carcinoma in situ and five cases of atypical hyperplasia would have gone undetected. If microscopic examination had also included all additional tissue consisting of fibrous parenchyma, all 50 carcinomas and 17 of the 19 atypical hyperplasias would have been detected and 20% fewer tissue blocks would have been submitted. We conclude that restricting histologic examination of NLBB to areas of radiographic calcifications and fibrous parenchyma results in a high level of detection of clinically significant lesions and a considerable reduction in the amount of tissue processed.

Autologous blood donations prior to elective cardiac surgery. Safety and effect on subsequent blood use.

Blood transfusions are frequently necessary for patients having open heart surgery. Although autologous blood is free of such risks as transfusion-transmitted diseases, the safety and effectiveness of autologous donations by cardiac patients are debated. We analyzed the records of 291 consecutive patients undergoing elective open heart surgery. One hundred seven (36.8%) donated between 1 and 6 U of autologous blood (mean, 3.0 +/- 1.5 U) before undergoing surgery. First-time elective coronary artery bypass surgery was the most common indication for autologous donation (90.6% of autologous donors). Problems following donations were infrequent (2 of 326 donations) and without long-term consequences. Among patients having coronary artery bypass surgery who donated autologous blood, 27% required homologous blood transfusions as compared with 82% of other patients undergoing the same procedure. We conclude that autologous blood donations are safe for patients with cardiac disease and effective in significantly reducing homologous blood needs in patients undergoing elective coronary artery bypass surgery.

Passive transfusion of human chorionic gonadotropin from plasma donated during pregnancy.

Although fresh frozen plasma (FFP) prepared from autologous blood donated during pregnancy has frequently been given to homologous recipients at our institution, one transfusion resulted in an unanticipated diagnostic dilemma. A 31-year-old woman with disseminated intravascular coagulation of unclear etiology was transfused with multiple units of FFP, including 2 from pregnant autologous donors. A serum human chorionic gonadotropin (HCG) assay, performed because of the possibility that the patient's illness was a complication of unrecognized pregnancy, was positive using a blood sample drawn 7 h after the transfusions. An extensive evaluation was completed before the possibility of passive transfer of hormone from blood products was considered. Retrospective testing of serum samples established that HCG appeared in the patient's serum only after the first FFP transfusion from a pregnant autologous donor. In 8 other recipients of 1 or 2 units of FFP from pregnant autologous donors, post-transfusion HCG levels ranged between 96 and 1,750 mIU/ml. Of 15 recipients of packed red blood cells from pregnant autologous donors, only patients with renal failure or recipients of multiple units developed positive HCGs, which were always less than or equal to 85 mIU/ml. The differential diagnosis of a positive pregnancy test in a recently transfused individual should include the possibility of passively acquired hormone.

Increased subepithelial collagen deposition is not specific for collagenous colitis.

To refine the pathologic diagnosis of collagenous colitis and to determine whether increased subepithelial collagen deposition (SCD) is specific for collagenous colitis (CC), we reviewed histologic sections of 1549 colonic mucosal biopsy procedures obtained from 1332 patients and 157 consecutive colonic resections. Quantitative evaluation of SCD was performed for those patients who showed thickness of SCD that was 5 microns or greater in either biopsies or resections, and the overall histologic and clinical features were correlated. A focal or diffuse increase of SCD greater than or equal to 5 microns was noted in 22 (1.7%) of the patients with biopsies and in 11 (7%) of the cases with resections. An otherwise unexplained, prolonged watery diarrhea was present in 6 (27%) of the 22 biopsy patients but in none of the patients with resections. These 6 patients with unexplained watery diarrhea had significantly greater amounts of SCD than those without this clinical feature both in terms of average thickness (21 microns versus 11 microns; P less than 0.02) and of the percentage of surface epithelium involved (47% versus 15%; P less than 0.02). Also, biopsies and resections with increased SCD from patients without watery diarrhea were all from the rectum and rectosigmoid region. These results lend further support to the nonspecificity of increased SCD and to the importance of the quantity of SCD, rather than its mere presence. In summary, increased SCD is not specific for CC; however, a greater amount of SCD is associated with a greater probability of associated watery diarrhea.(ABSTRACT TRUNCATED AT 250 WORDS)
