Determination of saterinone enantiomers in plasma samples with an internal standard using a Chiralcel OD column, fractionation and reversed-phase chromatography.

A specific and validated high-performance liquid chromatographic method was developed for the determination of the S-(-) and R-(+) enantiomers of saterinone. 1-[(4-cyano-1,2-dihydro-6-methyl-2-oxopyridin-5-yl)phenoxyl] -3-[4-(2- methoxyphenyl)piperazin-1-yl]propan-2-ol, in plasma at the low ng/ml level. The enantiomers of saterinone and an internal standard, 1-[(4-cyano-1,2-dihydro-6-methyl-2-oxo-pyridin-5-yl)phenoxy]-3-[4-(2- ethoxyphenyl)piperazin-1-yl]propan-2-ol, were chromatographed on a chiral Chiralcel OD stationary phase. However, the S-(-) enantiomers of saterinone and the internal standard were unresolved, as were the R-(+) enantiomers of both substances. Therefore, the two fractions were collected and each was separately resolved on an achiral Polyencap A reversed-phase column and quantified. The detection limit was 0.5 ng/ml of enantiomer, allowing the determination of plasma levels up to 36 h after oral administration of 90, 150 and 180 mg of saterinone to twelve subjects.

Cytofluorometric analysis of the DNA content in ovarian carcinoma and its relationship to patient survival.

Forty-one patients with epithelial malignancies of the ovary treated at the Medical College of Wisconsin Affiliated Hospitals from 1976 to 1984 had paraffin embedded tissue available for review. Of the 41 patients, 40 had adequate material to provide 50 micron sections that were evaluated with flow cytometry to determine DNA content. Tumor- and patient-related parameters were then correlated with the results of flow cytometry. Overall survival at 5 years in these patients was 43%, and relapse-free survival was 50%. Forty percent of the tumors were diploid, and 60% were aneuploid. Five-year survival for diploid patients was 74% with a relapse-free survival of 71%. Corresponding overall and relapse-free survivals for aneuploid patients were 22% and 35%, respectively. Distribution of the patients by histology, stage, and grade was equal between the diploid and aneuploid groups. All patients were treated with whole abdominal plus concomitant pelvic boost irradiation; total doses to the whole abdomen ranged from 10 Gy to 46 Gy (1000 to 4600 cGy) with the median dose being 37.5 Gy. Approximately 40% of the patients received chemotherapy, which usually consisted of a single agent (Alkeran, Burroughs Welcome, Research Triangle Park, NC). This retrospective study of patients with ovarian cancer treated with radiation therapy suggests the possibility that determinations of DNA ploidy may be useful in selecting patients whose poor prognosis dictates that a more aggressive therapy be used.

[Neopterin--a prognostic marker in HIV infection].

28 HIV-infected persons (19 stadium II, 5 stadium III, 4 stadium IV) were investigated for CD4+ lymphocytes, HIV p24 core antigen and urinary neopterin. Monitoring the course of infection repeated estimations were done. Urinary neopterin was found elevated in 26 out of 28 infected people (p less than 0.001). There is an increase in urinary neopterin levels associated with clinical progression in the course of HIV-infection. An inverse correlation to CD4+ lymphocyte count exists. High levels of neopterin early indicate a clinical progression to disease, as well as a decrease of CD4+ lymphocytes below 0.4 or 0.2 Gpt/l, respectively, and a reappearance of HIV p24 core antigen. The course of 5 cases is presented. In HIV-infection urinary neopterin is a suitable marker of activity and progression.

[Syphilis in HIV infection. 2 cases of neurosyphilis].

In HIV-infected people the course of syphilis is altered and aggressive. Besides seronegativity Neurosyphilis can manifest premature and as a outcome of failed penicillin-therapy in secondary syphilis. Two cases of neurosyphilis are reported and discussed. The usefulness of spinal fluid analysis is pointed out. Recommendations for diagnostics and therapy are given.
