New light on the technique and indications for ventriculo-cisternal drainage according to Leksell (interventriculostomy).

Experience with the Leksell drainage (interventriculostomy) in 21 patients is reported. Apart from an account of the technical details and the diagnostic requirements, the indications are also discussed. Apart from the "classic" indications are in a stenosis or restoration of the aqueduct in non-tumourous processes, we see a second reasonable field of application in subtotally resected posterior fossa tumours, when there is the possibility of a recurrence. In these cases the timely use of drainage can save a further operation. In addition, it can be used to forestall an immediate postoperative occlusion of the aqueduct due to oedema in cases in which a tumour was removed from the rostral part of the fourth ventricle. Compared to the common internal shunt procedures, the Leksell drainage shows a definitely greater functional security, equivalent to the Torkildsen drainage.

[Effects of glucosides on a spontaneously active vascular smooth muscle in vitro (author's transl)].

There is no support for the view that cardiac glucosides increase the resting tension in the rat portal vein, although such an effect on vascular smooth muscle might have been expected from the results of investigation on the coronary vessels. Glucosides in a concentration of 10(-4) m inhibit contractility while increasing the frequency of contractions. These changes of spontaneous activity are not accompanied by equivalent changes of the intracellular cation concentrations. There are no significant differences between the effects of g-strophanthine, k-strophanthoside and convallatoxin.

[A new valve controlled system for ventricular drainage. (Technical description and clinical results)].

A new valve controlled system for continuous ventricular drainage has been described. The clinical experiences (114 patients) are very satisfactory, even after a drainage duration of 15 days. The main advantages of the method are the resistance of all connexions against traction, the fixed position of the ventricular catheter, the variable opening pressure of the valve and the insensitivity to air or gas bubbles, at last the protection of the valve against blood and protein clots by use of filters. We have used the system for the continuous monitoring of the ICP and for diagnostic purposes (ventriculography, examination of cerebrospinal fluid) with good results.
