Zofran (Ondansetron hydrochloride) injection.

About 30% of patients undergoing surgical procedures in the United States experience nausea and vomiting. Zofran injection has been used to control and eliminate nausea and vomiting in cancer patients since 1991. Clearance has been received from the Food and Drug Administration for marketing of Zofran injection for the prevention of postoperative nausea and vomiting, at a dose of 4 mg IV over 2 to 5 min. Zofran has been evaluated in clinical studies with good results in the reduction or prevention of postoperative nausea and vomiting with minimal adverse reactions.

Levsin (hyoscyamine sulfate USP).

Levsin is an anticholinergic drug used in the treatment of irritable bowel syndromes, functional intestinal disorders, and as adjunct therapy in peptic ulcer disease. Levsin is also used parenterally to reduce duodenal motility to facilitate diagnostic studies. Levsin is readily absorbed through the gastrointestinal tract, reaches peak effects in 2-2.5 hr, and is excreted in the urine within 24 hr.

Propulsid (cisapride).

Propulsid is indicated in the treatment of patients with nocturnal heartburn due to gastroesophageal reflux disease. Propulsid administration increases lower esophageal sphincter pressure and increases esophageal acid clearance.

Flagyl (metronidazole hydrochloride).

Flagyl (metronidazole) is an anti-infective agent used in the treatment of anaerobic bacteria and protozoa. The most common uses of Flagyl in the patient undergoing gastroenterology evaluation and interventions are generally for anaerobic invasion of the gastrointestinal tract causing conditions such as giardiasis. Flagyl is also commonly used in the preoperative treatment of patients prior to colorectal surgery or those at high risk for bacterial contamination during the operative intervention (e.g., patients having emergency abdominal surgery with an unprepared colon). Flagyl is a highly potent anti-infective agent that must be administered and monitored appropriately for safe and effective use.

Unasyn (ampicillin sodium/sulbactam sodium).

Unasyn is a combination drug used for the treatment of infections as well as a prophylactic intervention prior to surgical or endoscopic interventions. Unasyn is a broad spectrum antibiotic of the penicillin family that inhibits beta-lactamases, ensuring activity of the ampicillin against resistant organisms. Peak serum levels of Unasyn are reached in 15 min to 1 hr after an IV infusion of 15 min.

Pentasa (Mesalamine).

Pentasa (mesalamine) is a newly released oral 5-aminosalicylic acid capsule marketed by Marion Merrel Dow Inc., for the induction of remission and for the treatment of mildly to moderately active ulcerative colitis. Ulcerative colitis is a chronic inflammatory disease and the most common form of inflammatory bowel disease, which is a debilitating and unpredictable group of illnesses affecting the gastrointestinal tracts of over 2 million Americans. Pentasa causes few side effects and is generally well tolerated.

A clinical pathway for endoscopy.

The purpose of this article is to describe the development and use of a clinical pathway in an endoscopy setting. The goal was to incorporate all of the established practices of each discipline--nursing, medicine, admissions, laboratory, radiology, and others--without changing their practice models. The clinical pathway is the framework for all the activities in the management and practice of the unit, including the nursing standard of care, nursing competency-based practice, hospital policy and procedures, quality improvement, risk management, and physician preference.
