Keeping the "care" in managed care: the importance of the therapeutic relationship and psychosocial factors in the managed care setting.

There is much truth in the adage that "the more things change, the more they stay the same." Nowhere does this seem more apparent than in health care where, amidst monumental reconfiguration, basic foundations of physician-patient relationships and attention to the impact of psychosocial factors on health and health care delivery remain as critical influences. While the importance of the therapeutic relationship and the influence of psychosocial factors in medical care has been clear in traditional systems of delivery, these factors may be even more critical in managed care systems. These emphases must be incorporated by design, however, and not left to default.

Dealing with pain-related disabilities: non-organic components of pain.

This article briefly introduces and reviews the "non-organic" components of pain-related disability situations, discusses the effect of the complexity of the problem on physician attitudes and behaviors, provides a description of the non-organic influences and their assessment, and makes suggestions for appropriate goals for intervening in pain-related disability conditions.

The chronic pain syndrome: a physical and psychologic challenge.

The chronic pain syndrome differs from chronic pain. The syndrome is a psychologic stress state that can produce physical-type illness, often with musculoskeletal manifestations. It tends to strike the "worried well," by nature hysterical, depressed, hypochondriacal, and dependent. Standard medical, surgical, and psychologic treatments do not alleviate the pain. The chronic pain syndrome progresses only with enablement by the physician or indiscriminate use of ancillary services. An early complete history and physical examination are the basis from which the physician draws logical conclusions about the patient to prevent progression. The long-range solution lies in a dynamically structured, progressive physical and mental reactivation program implemented by a highly trained interdisciplinary team of professionals using a holistic approach.
