[Microwave effect on immunological response of chronic limb lymphedema].

OBJECTIVE: To observe the character of local and systemic immune responses in chronic limb lymphedema, and to investigate the effect of microwave on immunological response. METHODS: From November 1996 to February 1998, 27 patients with chronic limb lymphedema were adopted in this study. Among them, there were 11 males and 16 females, the average age was 36.6 years. These patients were classified as the experimental group and 10 healthy volunteers as the control group. Before and after microwave heating and bandaging treatment, T and B lymphocyte in peripheral blood and skin infiltrating cells in the patients and volunteers were detected and phenotyped with alkaline phosphatase-anti-alkaline phosphatase(APAAP) and avidin biotin peroxidase complex (ABC) immunohistochemical methods respectively. RESULTS: There were decreases of CD4+ T lymphocyte and the ratio of CD4+/CD8+ in peripheral blood and predominant dermal perivascular T lymphocyte infiltration in chronic limb lymphedema patients. After two courses of microwave heating and bandaging treatment, it was found that the number of CD4+ T lymphocytes increased and the ratio of CD4+/CD8+ was restored to normal levels, and dermal perivascular T lymphocyte infiltration decreased greatly. CONCLUSION: Microwave heating and bandaging treatment can modulate the systemic and local immunological imbalance of chronic limb lymphedema.

[Treatment of nonunion in childhood by allogeneic decalcified bone graft].

OBJECTIVE: To investigate the effect of allogeneic decalcified bone graft in the treatment of nonunion in children. METHODS: From April 1990 to September 1997, 17 children with nonunion were adopted in this study. Among them, there were 10 boys and 7 girls, the age ranged from 2 to 13 years. The allogeneic decalcified bone graft taken from fresh corpse by aseptic manipulation were used to repair 3 cases of congenital nonunion and 14 cases of acquired nonunion. RESULTS: All cases were followed up 2 to 9 years after operation, 9 cases were cured successfully and 7 cases were cured after twice operations. One case of congenital pseudoarthrosis of the tibia was operated twice and there were callus grown half years after the second operation, but reoccurred after one year. Although there were 1.5 cm to 3 cm shortening deformity of extremity including another 2 cases of congenital pseudoarthrosis of the tibia and 5 cases of nonunion caused by chronic osteomyelitis, the function of joint was normal. CONCLUSION: Treatment of nonunion in children by allogeneic decalcified bone graft is a valuable technique in clinical practice. It is characterised by high capacity of osteoinduction, low antigenicity, rapid bony union, plentiful source of bone graft and convenient utilization.

[Clinical value of mediastinoscopy in the diagnosis of mediastinal mass: an analysis of 60 cases].

OBJECTIVE: To study the value of mediastinoscopic examination in the accurate diagnosis of mediastinal mass. METHODS: Standard cervical mediastinoscopy (SCM) or extended mediastinoscopy was applied to patients with mediastinal mass on chest CT or MRI, in order to obtain suspicious tissues at peritracheal, retrosternal, subcarinal, or bilateral hilar areas for pathological study. RESULTS: Of 60 cases in this series, 58 got definite pathological diagnoses. Of the 58 cases, 30 were diagnosed as malignancies, accounting for 51.7% of the cases examined. Twenty-eight of the 58 were diagnosed as benign accounting for 48.3% of them. Small-cell undifferentiated carcinoma was the most common type of malignancy, followed by lymphoma, metastatic adenocarcinoma and squamous cell carcinoma in decreasing frequencies. Over one half of the patients with benign lesions were tuberculosis, followed by sarcoidosis. In this series, 93% of the cases received SCM examination. The most common sampling sites were 2R and 4R areas. CONCLUSION: Mediastinoscopy is a valuable procedure which helps clarify diagnosis of mediastinal mass.

[Characteristics of lymph node metastasis of squamous cell carcinoma of thoracic esophagus and its clinical significance].

OBJECTIVE: To study the characteristics of lymph node metastases of squamous-cell carcinoma of thoracic esophagus and its influence on patients' prognosis and find reasonable dissecting approaches of lymph nodes. METHODS: To analyse retrospectively the clinical materials of 243 patients who had undergone resection of squamous-cell carcinoma of thoracic esophagus. RESULTS: Lymph node metastases was found in 110 of the 243 treated patients (45.3%). In 3,813 lymph nodes dissected, metastases existed in 400 (10.5%). The cancer cells disseminated upward and downward to lymph nodes along the esophagus. Tumor differentiation and the depth of tumor invasion were factors influencing lymph node metastases, but the length of tumor was not. The 5-year survival rate of the patients with lymph node metastasis was 16.4%, being much lower than that of patients with no lymph node metastasis (51.9%). The numbers of lymph node metastases had little influence on patients' prognosis. CONCLUSION: Because of the different upward and downward spreading of esophageal cancer cells to the lymph nodes, different lymph node dissecting approaches should be taken. The thoracic, and abdominal regional (two fields) lymph nodes dissection could be appropriate to esophageal carcinoma of the lower thoracic segment, and the cervical, thoracic and abdominal (three fields) lymph node dissection should be done for patients with earlier esophageal carcinoma of upper or middle thoracic segments.

[The role of granulocyte colony-stimulating factor in pathogenesis of acute lung injury].

OBJECTIVE: To investigate the role of granulocyte colony-stimulating factor (G-CSF) in pathogenetic mechanism of acute lung injury (ALI) by endotoxin. METHODS: A rat model of ALI was established by intraperitoneal injection of Escherichia coli LPS with a dose of 2 mg/kg. Thirty SD rats were randomly divided into the normal group and four endotoxin treated groups at different time intervals (2 h, 4 h, 6 h, 8 h after LPS injection), 6 rats each group. RT-PCR method was used to determine the expression level of G-CSF mRNA in lung tissue. The concentrations of serum TNF-alpha were determined by RIA. The number of neutrophils fixed in lung and peripheral blood neutrophils were counted, and pathological changes of lung tissue were examinted. RESULTS: The level of G-CSF mRNA expression in lung tissue increased rapidly at 2 h after LPS injection, peaked at 4 h. Although decreased a little at 6 h and 8 h, it was still significantly different from the normal group and the 2 h group (P < 0.01, P < 0.05). The level of TNF-alpha peaked at 2 h after endotoxin injection, which was significantly different from the normal group (P < 0.05). The number of neutrophils fixed in the lung also increased in endotoxin-treated groups, and the level of G-CSF mRNA expression in the lung tissue was positively correlated with the number of neutrophils fixed in the lung, the lung coefficient and the pathological signs of lung injury. CONCLUSIONS: The increase in the level of G-CSF mRNA expression in the lung tissue after endotoxin injection resulted in neutrophil sequestration in the lung, which may aggravate lung injury and increase lung coefficient. This process may be an important link in the pathogenetic mechanism of ALI induced by endotoxin.

[Anomalous junction of pancreaticobiliary duct in congenital bile-duct cystic and cylindrical dilatation].

OBJECTIVE: To study the relationship between anomalous junction of pancreaticobiliary duct (APBD) and congenital bile-duct cystic as well as cylindrical dilatation. METHOD: Cholangiography was performed in 51 patients with bile-duct dilatation. RESULTS: APBD was diagnosed in 43 patients. In 8 patients (72%), the bile duct was drained into the pancreatic duct (B-P) in the cystic group, whereas in 14 patients (78%), the pancreatic duct was drained into the bile duct (P-B) in cylindrical group. There were significant differences in the internal diameter of the distal bile duct (P < 0.01), the maximal diameter of the pancreatic duct (P < 0.05) and the average angle of pancreaticobiliary junction (P < 0.01) in cystic and cylindrical groups. CONCLUSIONS: Different types of APBD after lead to different pathological changes of the biliary tract. The narrowed distal bile duct joins the pancreatic duct at a right angle in most cystic cases and the pancreatic duct joins the bile duct at an acute angle in most cylindrical cases.

[Troponin T during cardiopulmonary bypass].

OBJECTIVE: To evaluate the effect of cardiopulmonary bypass (CPB) on myocardial troponin T and its clinical significance and relationship to myocardial cell injury. METHODS: Thirty-seven patients underwent open heart surgery under CPB. The procedures included correction of congenital heart disease in 15 patients, valve replacement in 15, and coronary artery bypass grafting in 7. Blood samples were taken at four different time intervals for assessing a troponin T. Correlative factors including duration of clamping of aorta, dosage of cardioplegic solution, and metabolic acidosis were studied. RESULTS: When clamping time > 60 minutes, dosage of cardioplegia solution < 10 ml/kg and existing metabolic acidosis, the level of troponin T was significantly increased (P < 0.01). CONCLUSION: The elevation of troponin T is closely related to cardiopulmonary bypass, especially the duration of clamping of aorta, insufficiency of cardioplegia, and metabolic acidosis.

[Re-surgical treatment for postoperative local recurrence of lung cancer].

OBJECTIVE: To evaluate the feasibility of re-surgical treatment for postoperative recurrence of lung cancer and its indications. METHOD: The clinical data of 20 patients treated again surgically after recurrence of lung cancer were analyzed retrospectively. RESULTS: The resectability, radical resectability, morbidity, and mortality were 75.0% (15/20), 80.0% (12/15), 20.0% and 0.0% respectively. The mean operative blood loss was 945 ml. The percentages of patients with pleural adhesion and intrapericardial dissection were 100.0% and 53.3% separately. The 1-year, 3-year and 5-year survival rates were 94.1%, 41.7% and 40.0%. The mean survival time of the patients receiving exploration or palliative resection was 15.0 months, shorter than the 56.9 months of the patients having radical resection. The mean survival time of the patients with N(0) was (105.3 +/- 82.0) months, much longer than (11.4 +/- 6.0) months of the patients with N(1) or N(2). CONCLUSION: Because of extensive adhesion, operations for postoperative recurrence of lung cancer are difficult, but if recurrence is found in time and operative indication is appropriate, re-surgical treatment can prolong the patients' survival time.

[A dynamic study on the surgically induced astigmatism after phacoemulsification through three different-sized scleral tunnel incisions].

OBJECTIVE: To compare the changes in the values and directions of surgically induced astigmatism (SIA) after phacoemulsification with three different-sized scleral tunnel incisions and to evaluate their clinical results. METHODS: The patients were divided into three groups; the length of the scleral tunnel incision was 3.2, 5.5 and 7.0 mm respectively in A, B and C groups. Keratometry measurements were obtained preoperatively, at postoperative 1 week, 1 month and 6 months respectively. SIAs were analyzed by "Cravy vector method". The mean SIA and uncorrected visual acuity were compared among the groups up to 6 months postoperatively. RESULTS: Eyes in A group displayed significantly less mean SIA and better uncorrected visual acuity than B and C groups at 1 week postoperatively (P < 0.05), but there were no significant differences between any groups at 1 month, 3 months and 6 months postoperatively (P > 0.05). There were no significant differences in the mean SIA and uncorrected visual acuity between B and C groups throughout the whole study period (P > 0.05). CONCLUSIONS: SIAs in eyes of 3.2 mm incision group of phacoemulsification are less and more stable; a better and quicker rehabilitation of uncorrected visual acuity can be obtained in the early postoperative period.

[The susceptible alleles on HLA-DRB 1 of type I diabetes in children in Harbin].

OBJECTIVE: To evaluate association between type I diabetes in children and HLA-DRB1 alleles in Harbin. METHODS: Case-control method was employed, and the PCR-SSP technique was used to identify HLA-DRB1 alleles. RESULTS: The frequencies of DRB1 * 0301 and DR3/4 heterozygosity were significantly higher but the frequency of DR2 was significantly lower in cases than that in controls. In DR4 subtypes, the frequency of DRB1 * 0405 was higher and the frequency of DRB1 * 0406 was lower in cases than that in the controls. The difference between cases and controls in DRB1 * 0901 was not significant. CONCLUSION: DRB1 * 0301 allele and DR3/4 heterozygosity were the important susceptibility and heterozygosity genes while DR2 was the most important protective gene in Harbin. The DR4 possessed double-tendency. DRB1 * 0405 conferred susceptibility and DRB1 * 0406 conferred little protection. The susceptibility conferred by DRB1 * 0901 was not certain in type I diabetes in childhood in China. This study provided useful genetic data on the study of etiology of type I diabetes mellitus.

[Characterization and phylogenic analysis on a new isolate of genus Rhodocista].

A strain of purple non sulfur bacteria was isolated from waste water plant in Beijing. It is phototaxic, and can converted the vibrioid cells into thick-wall cyst. Internal synthetic membrane is present as lamellae. It has a low absorption maxima at 798 nm. and require thiamine and vitamin B12 as growth factors. It can use succinate as carbon and energy source, but glucose can not be used. It can not grow in 3% NaCl and had Q-9 as major quinone. The 16S rRNA gene was amplified, cloned and sequenced. A phylogenic tree was constructed on the basis of the 16S rDNA sequences. It showed that the previously known member of the genus Rhodocista, Rc. centenaria, is the nearest neighbor to strain 3-p. The level of binary sequence similarity between Rc. centenaria and strain 3-p of 95%, and their phenotypic differences and genetic DNA-DNA relatedness of 56%, shows they are different species of Rhodocista. A new species name, Rhodocista pekinense, was proposed for strain 3-p.

[Expression of recombinant human stem cell factor in insect cells].

The cDNA of human stem cell factor(hSCF) containing signal sequence was cloned into the transfer vector pVL941 of AcNPV to construct a recombinant transfer vector pVL941-SCF. Sf9 cells were cotransfected with wild type viral DNA and pVL914-SCF to produce the recombinant virus AcNPV-SCF by homologous recombination in cell. Southern-hybridization analysis suggested that the recombinant viral DNA contained hSCF cDNA fragment. The Sf9 cells infected with the recombinant baculovirus AcNPV-SCF expressed biologically active rhSCF which was secreted into the cell culture. The synergistic activities of SCF in conjunction with human interleukin-3(hIL-3) was measured by MTT colorimetric method and TF-1 cell line. The expression level of Sf9 cells reached its highest at about 1970 units/ml in the 3rd day after the infection with AcNPV-SCF. Three SCF bands with molecular masses of 18 x 10(3), 20 x 10(3) and 22 x 10(3) were detected by immunoblotting.

[Phenotypic features and genetic diversity of strains of Rhodopseudomonas].

14 strains of purple non-sulfur bacteria and 3 type strains of Rhodopseudomonas were investigated by phenotypic characterizing, DNA-DNA hybridizing and DGGE analyzing. The characteristics of their morphology, structure and physiology show that 14 strains of isolates have the features of the genus Rhodopseudomonas: dividing asymmetrically by budding, lamellar intracytoplasmic membranes, presence of bacteriochlorophyll a and carotenoids of the normal spirilloxanthin series. On the base of their differences in morphology, growth, carbon sources 14 strains can be divided into 2 groups: g-c group, T group. Parts of 16S rRNA gene were amplified by using a pair of primers: 341f-906r. Denaturing gradient gel electrophoresis(DGGE) of PCR-amplified fragment of rDNA indicated that they have 3 genetic types: 1 of them, including 6 of 8 strains of T groups is similar to that of Rps. rutila R1, the other 2 types, represented separately by 2 of T group or all the g-c group, were different from that of either of 3 type strains The Data of the DNA-DNA hybridization of 8 representative strains indicated that the differences between different types were enough to separate them into different species including 2 possible new species.

[Biomachining of metal copper by Thiobacillus ferrooxidans].

Thiobacillus ferrooxidans was employed in the biomachining process of metal copper(Cu0). The bacteria growth and the changes of Fe3+ concentration during machining processes have been studied. Biomachining and chemical machining have been compared. The results showed that the concentrations of bacteria and Fe3+ determine the speed of machining copper. The biomachining is more fast that chemical maching because bacteria are able to regenerate Fe3+ oxidizing copper. It was also found that the Cu2+ produced from the machining processes inhibit the growth of bacteria. Cu2+ has to be removed.
