Measurement of absolute dimensions in endoscopic sinus surgery.

OBJECTIVE: To develop and evaluate a simple, noninvasive technique for the precise measurement of antrostomy dimensions using standard endoscopic views. METHODS: We produced an antrostomy probe and devised a method, on the basis of techniques of aerial planimetry, for measuring antrostomies using standardized endoscopic views. Ten shapes of known surface areas were measured independently by three experienced endoscopic sinus surgeons using 30 and 70 degree endoscopes. The accuracies of surgeon assessment and interobserver concordance were calculated. RESULTS: The three surgeons were able to calculate the surface areas with great precision (Pearson's coefficient of correlation > 0.85 in all cases) using both 30 and 70 degree endoscopes. An intraclass correlation coefficient of 0.92 was obtained, indicating a high degree of concordance between the measurements obtained by different surgeons. CONCLUSION: This technique shows considerable promise as a tool for precise measurement of antrostomy dimensions in both clinical and research settings.

The diagnostic value of barium swallow in globus syndrome.

Serious lesions presenting as globus syndrome appear to be extremely rare, but many clinicians routinely request barium swallow to exclude these. No data yet exist on the diagnostic yield of contrast radiography. We reviewed 2854 barium swallows from one department over 7 years. The false-positive rate (37%) depended on presenting symptoms and differed between radiology departments, suggesting substantial subjectivity. No serious radiological abnormalities occurred in the estimated 2011 patients with globus syndrome. The 95% confidence interval for the likelihood of finding a significant abnormality on routine barium swallow in a patient with globus symptoms is 0-0.00145. The 2854 barium swallows cost an estimated pounds sterling 685,000 and 0.22-0.48 deaths. As the risk of missing a serious lesion in a patient with globus is very unlikely to exceed one in 700, we support the opinion that barium swallow should be reserved for those with risk factors or atypical features.

Is electronystagmography of diagnostic value in the elderly?

We aimed to determine the diagnostic value of electronystagmography (ENG) in a community-based sample of dizzy subjects over 65 years old. A total of 96 asymptomatic controls and 149 dizzy subjects underwent ENG. Clinical diagnoses were made on standardized criteria. ENG results were classed as normal or abnormal, according to reference ranges derived from the controls. Rates of ENG abnormality in different diagnostic categories, sensitivities, specificities and predictive values were calculated. Central vascular disease was common (105 out of 149 subjects); peripheral vestibular disease was not (14). Spontaneous nystagmus had a positive predictive value of 95% for central vascular disease, but was only 18%-sensitive, and was usually detectable clinically. ENG had no other significant diagnostic value. ENG failed to discriminate dizzy subjects from controls and failed to differentiate various dizziness syndromes. ENG was of no practical value in this community-derived sample of dizzy elderly subjects.
