Neural cograft stimulates the survival and differentiation of embryonic stem cells in the adult mammalian auditory system.

Mouse embryonic stem (ES) cells were transplanted into the cochlea of adult guinea pigs in order to explore their survival, differentiation, and possible integration with the host tissue. With the purpose of investigating the possible effect of manipulating the local embryonic microenvironment, ES cells were transplanted into the cochlea with or without an embryonic neuronal cograft consisting of dorsal root ganglion (DRG) tissue. To detect the survival and differentiation of ES cells, cells expressing green fluorescent protein (GFP) were used in combination with immunohistochemical detection of a neuronal marker, neural class III beta-tubulin (TUJ1 antibody). At 4 weeks following transplantation implanted ES cells were found close both to the sensory epithelium, and the spiral ganglion neurons (SGNs) with their peripheral dendritic processes projecting to the organ of Corti. There was a significant difference in the number of surviving TUJ1 (+) ES cells between the DRG cograft group and the non-cograft group (P < 0.01, ANOVA). Neurite-like projections were also identified between TUJ1-positive ES cells and the peripheral dendritic processes from SGNs. The results suggest that an embryonic neuronal microenvironment may be one of the key factors in the survival and differentiation of ES cells in the adult auditory system.

[Misdiagnosis analysis of Wegener's granulomatosis in otorhinolaryngology].

OBJECTIVE: To investigate the early clinical features and development of Wegener's granulomatosis (WG), and to reduce the rate of misdiagnosis. METHOD: Retrospective analysis of 32 WG patients who consulted in otorhinolaryngology department primarily was carried out. RESULT: Among 32 cases, twenty-seven had been misdiagnosised (84%). Most of the cases were misdiagnosised as inflammation of otorhinolaryngology. CONCLUSION: Many cases of WG were consulted at otorhinolaryngology doctors primarily. Otolaryngological presentation may be as rhinitis, otitis media, facial palsy or laryngeal obstruction and so on. Patients with these presentations not responding to the standard treatment should be further investigated for a diagnosis of WG. Demanding history detailed, thinking highly of the status of lung and kidneys, analyzing the abnormal examinations overall are the key points to reduce the rate of misdiagnosis of WG.

[Dermatomyositis/polymyositis associated with head and neck malignancy].

OBJECTIVE: To observe the relation between dermatomyositis/polymyositis and head-neck malignancy. METHOD: All 136 cases of dermatomyositis/polymyositis admitted to PUMCH during 10 years (1994-2003) were studied. The clinical features of 6 patients with an associated head-neck malignancy were analyzed. RESULT: There were 17 cases of malignancy in 136 patients with dermatomyositis/polymyositis, of which, 6 cases (6/17, 35.29%) were found to have head-neck malignancy: 4 cases of nasopharyngeal carcinoma, 1 case of non-Hodgkin lymphoma and 1 case of hypopharynx carcinoma. CONCLUSION: Nasopharyngeal carcinoma represents the first neoplasm among head-neck malignancy associated with dermatomyositis/polymyositis. Nasal endoscopic and/or fibrolaryngoscopic search for potential head-neck malignancy should be performed in every patient with dermatomyositis/polymyositis.

[Study on distortion product otoacoustic emissions and expanded high frequency audiometry in noise exposure workers].

OBJECTIVE: To analyse the relationship of distortion product otoacoustic emissions(DPOAE), conventional pure tone audiometry and expanded high frequency audiometry, and discuss the originated mechanism of DPOAE and value in early diagnosis and detection noise-induced deafness. METHOD: DPOAE, conventional pure tone audiometry and expanded high frequency audiometry were performed in 42 young adults with normal hearing of control group and 20 noise exposure workers of test group. DPOAE amplitudes, conventional pure tone hearing thresholds and expanded high frequency hearing thresholds were compared. RESULT: In noise exposure workers against young adults with normal hearing, the pure tone hearing thresholds at 6.0 kHz and expanded high frequency area declined significantly(P < 0.05), DPOAE amplitudes at frequency from 4.0 to 6.0 kHz and 11.2 kHz declined significantly too(P < 0.05), but there existed no significantly difference at frequency from 12.5 to 16.0 kHz between the two groups(P > 0.05). CONCLUSION: DPOAE is potential implement to early diagnose and detect noise-induced deafness. It seems likely that the origination of DPOAE not limited to the outer hair cell in corresponding cochlear area. The originated area and mechanism of DPOAE should be solved by further studies.

[Predictive value of sleep nasendoscopy and the Müller maneuver in uvulopalatopharyngoplasty for the obstructive sleep apnea syndrome].

OBJECTIVE: To improve the therapy effect of uvulopalatopharyngoplasty (UPPP) for the obstructive sleep apnea syndrome (OSAS) with the use of sleep pharyngolaryngoscopy and the Müller maneuver to localize pharyngeal airway collapse. METHOD: Twenty-eight patients with OSAS were divided into two groups respectively based on findings of Müller maneuver and sleep pharyngolaryngoscopy: Type I had only oropharyngeal(soft palate and palatine tonsil) collapse and type II had oropharyngeal collapse with hypopharyngeal collapse. All outcome of PSG after UPPP was analyzed. RESULT: 1. In Type I, the change of the average apnea hypopnea index (AHI) and the lowest SaO2 before and after UPPP was 52.7 to 19.7 (P < 0.01), 63.1% to 80.0% (P < 0.01) using Müller maneuver, and 48.6 to 18.7(P < 0.01), 65% to 80%(P < 0.01) using sleep pharyngolaryngoscopy respectively; In type II, the change was respectively 54.7 to 38.6(P > 0.05), 60.7% to 67.0%(P > 0.05) using Müller maneuver, and 59 to 34(P < 0.01), 59% to 71%(P < 0.05) using sleep pharyngolaryngoscopy. 2. Surgical response rate in type I was compared with type II: 68.4% VS 22.2% (P < 0.01) using Müller maneuver and 73.3% VS 30.8% (P < 0.05) using sleep pharyngolaryngoscopy, respectively. CONCLUSION: Endoscopic pharyngeal assessment of OSAS patients has clinical value for the improvement of UPPP outcome.

[Study on auditory brainstem response to bone conducted clicks].

OBJECTIVE: To observe how different vibrator placement and different stimulate rates affect the results of bone conduction ABRs. METHOD: Bone conduction ABRs with different vibratory placement and different stimulate rates were recorded in 20 normal hearing young adults and compared with their air conduction ABRs. RESULT: The results show that all subjects were recorded reproducible and reliable air and bone conduction ABR waveforms under higher intensities(above 40 dBnHL). The ABR thresholds from air and bone conduction were similar, they are both about 5-10 dB higher above their behavioral thresholds to clicks. The frontal placement produces significantly longer ABR wave V latencies. Bone conduction ABR wave V latencies from different stimulate rates were also observed. The results show that temporal placement ABR wave V latencies became longer significantly when the stimulate rate increased from 20/s to 52/s. CONCLUSION: Although it is affected by different vibrator placement and different stimulate rate, bone conduction ABR is a valuable and effective method to detect hearing, it plays an important role in newborn auditory screening, assessment of cochlear reserve, and identification of hearing loss.

[Bilirubin induced apoptosis of human neuroblastoma cell line SH-SY5Y and affected the mitochondrial membrane potential].

OBJECTIVE: To study whether bilirubin induces apoptosis of human neuroblastoma cell line SH-SY5Y and affects the mitochondrial membrane potential. METHODS: SH-SY5Y cells were exposed to bilirubin, then morphological changes of nucleus were assayed by Hoechst 33258 staining. The percentage of apoptotic cells was measured by flow cytometry. The mitochondrial membrance potential(MMP) was detected by flow cytometry through Rhodamine123 staining. RESULTS: MMP of SH-SY5Y cells significantly decreased from (16.6 +/- 1.556) U to (7.11 +/- 0.701) U (P < 0.01) after exposed to 0.02 g/L bilirubin for 1 h. The percentage of apoptotic cells was 19.4% after exposed for 3 h, and significantly increased to 76.4% after 4 h, but typical apoptotic nuclear changes occured after 6 h. CONCLUSION: Bilirubin could early cause disruption of MMP, then induce the apoptosis of SH-SY5Y cells.
