Huge retropharyngeal lipoma causing obstructive sleep apnea: A case report.

Lipoma of the retropharyngeal space is a very rare benign tumor often causing unspecific clinical symptoms. The most common symptoms are dysphagia and/or respiratory disturbances. The clinical diagnosis may be difficult. The radiological imaging techniques (CT and MRI) can provide adequate information with regard to the composition and extension of the tumor, although final histological confirmation is essential. Surgery is the treatment of choice. We present a case of 40-year-old male patient complaining of obstructive sleep apnea symptoms (respiratory disturbances, excessive daytime somnolence, morning headache). The radiological examination (CT) showed a huge (11.7 x 7.2 cm) lipoma of the retropharyngeal space extending from the nasopharynx to the superior mediastinum. The tumor was removed via transcervical approach with complete amelioration of symptoms.

[Rare case of primary sinonasal tuberculosis].

INTRODUCTION: Tuberculosis, considering entry of infection, most frequent concerns the lungs. Rarely we observed extrapulmonary cases of tuberculosis within the bones, joints, urogenital organs and lymph nodes as well as within the head and neck region--larynx, middle ear, pharynx, tonsils, tongue, parotid glands and central nervous system. Sinonasal tuberculosis, either primary or secondary to pulmonary infection, is very rare, but should be considered in the differential diagnosis of sinonasal granulomas. The diagnosis of the head and neck tuberculosis is based on the medical patientis examination as well as on the bacteriological and histological study of the biopsy specimen. AIM: The aim of the study was to show own, very rare case of primary sinonasal tuberculosis. MATERIAL AND METHODS: We described a case of primary sinonasal tuberculosis in an adult female. She was suspected of the neoplastic disease. The diagnosis was based on the histopathology. The treatment was successful and consisted with antituberculous drug therapy and surgery (Functional Endoscopic Sinus Surgery). The patient was followed up for 12 months and during this time we did not notice any symptoms of recurrence. CONCLUSIONS: Tuberculosis of the head and neck region should be take in to consideration during the differential diagnostic of the neoplastic diseases.

[Oxyphilic and follicular thyroid tumors--the correlation between the cytopathologic diagnosis and the histopathologic examination].

INTRODUCTION: Fine needle aspiration biopsy (FNAB) of the thyroid nodules generally allows to make the diagnosis and to choose the proper clinical management. In about 10% of cases cytopathologic features do not differentiate unequivocally benign and malignant lesions. In these cases the cytopathologic diagnosis of follicular tumor (FT) or oxyphilic tumor (OT) is most often made. MATERIALS AND METHODS: From 2001 to 2002 in our Department of Pathology the cytopathologic diagnosis of FT and OT was made in 102 and 25 cases respectively. Histopathologic verification was possible in 39 (38%) patients with FT and in 19 (76%) patients with OT. RESULTS: Histopathological diagnosis of neoplasm was made in 48.7% (19/39) FT and 42% (8/19) OT. The risk of carcinoma was 12.8% in FT and 16% in OT group (surgical treated cases only). CONCLUSIONS: These results show how difficult the diagnostics of follicular lesions in FNAB could be because of the frequent overlapping of the cytological features of benign and malignant lesions. Diagnosis of follicular tumor does not mean carcinoma. Majority lesions are begin non neoplastic on final histopathologic examination. The use of follicular/ oxyphilic tumor in cytological diagnostic instead of follicular neoplasm seems more advisable. However FNAB in the correlation with clinical data may select the patients for the surgical treatment or the further observation.

[Percentage of neoplastic cells in papillary thyroid carcinoma: implications for DNA microarray studies of gene expression profile].

Studies of gene expression profile using DNA microarray technology are usually performed using either tumor-derived sample material or isolated neoplastic cells obtained through microdissection. The scope of information about neoplastic transformation gained from studying profile of gene expression in microdissected samples would be much wider but collection of sufficient amounts of intact RNA is very difficult. A compromise could be reached by relating gene expression profile to percentage of neoplastic cells in the investigated tissue sample. The ratio of neoplastic cells in the investigated sample of papillary thyroid cancer was assessed through evaluation of approximated count of cell number in 10-18 examined image fields. This information was related to gene expression profiles obtained from DNA microarrays. The study involved 40 cases of papillary thyroid cancer; the percentage of PTC cells varied between 20 and 95% and only in half of the cases exceeded 75%. Correct differentiation of tumor and normal sample by means of gene expression profile was possible only when the percentage of tumor cells exceeded 25-30%. Seventeen genes showing the best correlation with the tumor cell numbers were selected and their classification potential was evaluated. Strength of information derived from gene expression profile studies by DNA microarrays in papillary thyroid cancer cells is very reliable and permits distinguishing correctly between normal and neoplastic tissues even when the percentage of cancer cells does not exceed 25-50%. However, the differentiation potential of gene expression profile is not markedly improved by selection of genes showing best correlation with the number of tumor cells.

The radiation-induced changes in rectal mucosa: hyperfractionated vs. hypofractionated preoperative radiation for rectal cancer.

PURPOSE: The purpose of the study was the qualitative and quantitative evaluation of acute radiation-induced rectal changes in patients who underwent preoperative radiotherapy according to two different irradiation protocols. PATIENTS AND METHODS: Sixty-eight patients with rectal adenocarcinoma underwent preoperative radiotherapy; 44 and 24 patients underwent hyperfractionated and hypofractionated protocol, respectively. Fifteen patients treated with surgery alone served as a control group. Five basic histopathologic features (meganucleosis, inflammatory infiltrations, eosinophils, mucus secretion, and erosions) and two additional features (mitotic figures and architectural glandular abnormalities) of radiation-induced changes were qualified and quantified. RESULTS: Acute radiation-induced reactions were found in 66 patients. The most common were eosinophilic and plasma-cell inflammatory infiltrations (65 patients), erosions, and decreased mucus secretion (54 patients). Meganucleosis and mitotic figures were more common in patients who underwent hyperfractionated radiotherapy. The least common were the glandular architectural distortions, especially in patients treated with hypofractionated radiotherapy. Statistically significant differences in morphologic parameters studied between groups treated with different irradiation protocols were found. CONCLUSION: The system of assessment is a valuable tool in the evaluation of radiation-induced changes in the rectal mucosa. A greater intensity of regenerative changes was found in patients treated with hyperfractionated radiotherapy.

Protective effect of local temporary ischemia depends on applied dose of radiation.

The aim of this study was to verify hypothesis that protective effect of local temporary ischemia depends on dose of radiation. 56 male WAG-strain rats were used. Total body irradiation with 3 x 3 and 3 x 5 Gy was performed. Local temporary ischemia was induced by clamping the tail base. The biochemical parameters were the thiobarbituric acid-reactive substances (TBA-RS). In bone marrow smears the polychromatic erythrocyte (PCE) numbers were counted and the numbers of micronucleated PCEs were analyzed. In small intestines the numbers of crypts were calculated. The levels of TBA-RS in the serum of the animals irradiated with a 3 x 3 Gy dose were significantly different (P < 0.002). Also in animals irradiated with a dose of 3 x 3 Gy the numbers of intestinal crypts were different (P < 0.05). In animals irradiated with dose 3 x 5 Gy, for analyzed parameters differences did not achieve statistical significance. Local temporary ischaemia provides general protection against radiation damage for lower dose. This protective effect disappeared after applications of a higher dose of radiation.

Gene expression profile of papillary thyroid cancer: sources of variability and diagnostic implications.

The study looked for an optimal set of genes differentiating between papillary thyroid cancer (PTC) and normal thyroid tissue and assessed the sources of variability in gene expression profiles. The analysis was done by oligonucleotide microarrays (GeneChip HG-U133A) in 50 tissue samples taken intraoperatively from 33 patients (23 PTC patients and 10 patients with other thyroid disease). In the initial group of 16 PTC and 16 normal samples, we assessed the sources of variability in the gene expression profile by singular value decomposition which specified three major patterns of variability. The first and the most distinct mode grouped transcripts differentiating between tumor and normal tissues. Two consecutive modes contained a large proportion of immunity-related genes. To generate a multigene classifier for tumor-normal difference, we used support vector machines-based technique (recursive feature replacement). It included the following 19 genes: DPP4, GJB3, ST14, SERPINA1, LRP4, MET, EVA1, SPUVE, LGALS3, HBB, MKRN2, MRC2, IGSF1, KIAA0830, RXRG, P4HA2, CDH3, IL13RA1, and MTMR4, and correctly discriminated 17 of 18 additional PTC/normal thyroid samples and all 16 samples published in a previous microarray study. Selected novel genes (LRP4, EVA1, TMPRSS4, QPCT, and SLC34A2) were confirmed by Q-PCR. Our results prove that the gene expression signal of PTC is easily detectable even when cancer cells do not prevail over tumor stroma. We indicate and separate the confounding variability related to the immune response. Finally, we propose a potent molecular classifier able to discriminate between PTC and nonmalignant thyroid in more than 90% of investigated samples.

[Anaplastic thyroid carcinoma in a 14-year-old boy].

We report a case of anaplastic cancer of thyroid in 14-year-old male. The diagnosis was supported on: pace of changes, clinical image, disease course, no effect of therapy, histopathological examination.

[A very rare case of low-grade angiosarcoma of the nose and paranasal sinuses].

Angiosarcomas of the nose and paranasal sinuses are extremely rare. Histologically angiosarcomas are divided into two groups: low and high-grade. We present a case of angiosarcoma of apparent low-grade malignancy localized within the nasal cavity and paranasal sinuses in a 74-year old woman treated by radiotherapy due to middle ear chemodectoma 21 years ago. She was surgically treated via the lateral rhinotomy. During the 23 months follow-up no recurrence or epistaxis have been observed.

Analysis of cyclin D1 and retinoblastoma protein immunoreactivity in follicular thyroid tumors.

Protein products of cyclin D1 and retinoblastoma (Rb) genes play crucial roles in regulation of G1/S transition in the cell cycle. In this study we analyzed, using immunohistochemical methods, the expression of cyclin D1 and Rb proteins in material from medical archives (12 cases of follicular thyroid carcinoma, 57 cases of follicular adenoma and 17 nodular goiter cases). A positive nuclear reaction for cyclin D1 was observed in 83.3% (10/12) of the follicular carcinomas, in 96.5% (55/57) of the follicular adenomas and in 23.5% (4/17) of nodular goiters. Overexpression of cyclin Dl (more than 50% of positively staining cells) was noted in 25% (3/12) of the follicular carcinomas and in 22.8% (13/57) of the follicular adenomas. No overexpression of cyclin D1 was noted among nodular goiters. The number of carcinoma cases with cyclin D1 overexpression did not differ statistically in any significant way from the follicular adenoma group (p = 1.000). A positive nuclear reaction for Rb protein was noted in 100% of the follicular carcinomas (12/12), in 96.5% of the follicular adenomas (55/57) and in 47.1% of the cases (8/17) of nodular goiter. Rb protein overexpression (more than 50% of positively staining cells) was found in 83.3% (10/12) of the follicular carcinomas, in 68.4% (39/57) of the follicular adenomas and in 11.8% (2/17) of the nodular goiters. The number of cases with Rb protein overexpression in the follicular carcinoma group did not differ significantly from that in the follicular adenoma group (p = 0.486). A positive correlation was found in the groups studied between the expressions of Rb protein and cyclin D1. However, the correlation was statistically significant only in the nodular goiter group (Rs = 0.567; p = 0.018). In the follicular carcinoma group, that correlation was borderline (Rp = 0.437; p = 0.072) and, in the follicular adenoma group, it was statistically insignificant (Rs = 0.217; p = 0.105). Our results confirm the existence of mutual regulation mechanisms of Rb and cyclin D1 protein expressions, which are observed in cells from various carcinomas.

[Pleomorphic adenoma of the nasal septum].

The authors described the case of mixed salivary gland tumour in the nasal septum. This localisation is extremely rare. Only several cases were described in Polish literature.

Loss of heterozygosity in 73 human thyroid tumors.

OBJECTIVES: The aim of the study was to establish the LOH frequency of selected polymorphic markers in different histological types of thyroid tumors: 18 colloid goiters (CG), five follicular adenomas (FA), nine follicular carcinomas (FTC), 40 papillary carcinomas (PTC), and one anaplastic carcinoma (ATC). For PTC, tumors negative for RET/PTC rearrangements were preferred. METHODS: LOH studies were performed using 14 highly polymorphic markers previously described as frequently lost in thyroid tumors. RESULTS: In 20 cases (27%) the loss of at least one marker was found. No difference between the frequency of the LOH in FTC and PTC tumors was revealed (33% v. 33%). No differences between histopathological subtypes of PTC in LOH were found. Papillary thyroid carcinomas showed a tendency to higher LOH frequency from patients older than 45 years of age compared to younger ones (9/23 v. 4/17) although it was not statistically significant. CONCLUSIONS: We conclude that papillary thyroid cancers, particularly those diagnosed in patients older than 45 years of age, do exhibit LOH at least with the same frequency as follicular cancers. This increased number of LOH events may contribute to the clinical aggressiveness of cancer in older patients.

Diagnostic problems concerning epithelioid sarcoma--case analysis.

We discuss here five cases of epithelioid sarcoma (ES) with final diagnosis established after reexamination of initial findings. Problems with differential diagnosis of these neoplasms arise since their microscopic picture may simulate several other pathological conditions such as non-neoplastic granulomatous reactions, squamous cell carcinomas and adenocarcinomas, melanomas and soft tissue sarcomas with epithelioid component. Final ES diagnosis requires presence of cytokeratin, EMA and vimentin in neoplastic cells, as confirmed by immunohistochemical reactions. Differential diagnosis is also helped by concurrent cytology assessment that allows recognizing more easily such characteristic features as presence of plasmacytoid or spindle-shaped cells.

[The rare case of sinonasal osteosarcoma in a 22-year-old patient (case report)].

Sarcomas of head and neck are very rare and comprise about 10 to 15% of all malignant nonepithelial neoplasms. A case of 22-year old patient with sinonasal osteosarcoma was presented. The authors stressed the significance of CT in the diagnostic procedures. The preoperative symptoms, histological character of the tumor and postoperative status were shown and discussed. Patient was treated by combined method: the surgical excision and radio-chemotherapy. The 24 months follow-up period was free of symptoms.

[Rare case of primary isolated orbital sarcoidosis in 53 year old woman].

Isolated to one organ cases of sarcoidosis are observed extremely rare. We described a case of 53 year old woman suffer from isolated to orbit form of sarcoidosis. Systemic sarcoidosis (lymphadenopathy, pulmonary infiltrates, skin lesions) are excluded. The patient was treated by surgery (excision of orbital tumor) with following prolonged steroidotherapy. Total retirement of symptoms was obtained. During the 19 months follow-up no recurrence have been observed.

Analysis of P53 and P21WAF1 proteins expression in follicular thyroid tumors.

The expression of P53 and P21WAF1 proteins was analyzed immunohistochemically in archival material derived from 12 cases of follicular thyroid carcinoma, 57 cases of follicular adenoma and 17 cases of nodular goiter. In the follicular carcinoma group 6 out of 12 cases (50%) were positive for P53 protein and 4 out of 12 cases (33.3%) were positive for P21WAF1 protein. In the follicular adenoma group, 18 out of 57 cases (31.6%) were positive for P53 and 16 out of 57 cases (28.1%) were positive for P21WAF1 protein. No positive cases of P53 or P21WAF1 proteins presence were found in the nodular goiter group. Positive correlation between the expression of P53 and P21WAF1 proteins was found for follicular carcinoma and adenoma groups (p = 0.034 and p = 0.002, respectively). The obtained results demonstrate that simultaneous immunohistochemical detection of P53 and P21WAF1 proteins expression may be useful in determining functional status of P53 protein, helping to interpret expression of P53 protein in thyroid follicular carcinoma cells.

Analysis of P161NK4A protein expression in follicular thyroid tumors.

PI6INK4A (P16) protein expression was analyzed immunohistochemically in archival material derived from 12 cases of follicular thyroid carcinoma, 57 cases of follicular adenoma and 17 cases of nodular goiter. Among follicular carcinomas, 11 out of 12 examined cases (91.7%) were positive for P161NK4A protein. Among follicular adenomas the percentage of immunopositivity was 76.5% (45/57) and among nodular goiter cases it was 19.3% (13/17). Overexpression of P16INK4A protein was found in 66.7% (8/12) of follicular carcinomas and in 19.3% (11/57) of follicular adenomas; the values of this parameter were statistically significantly higher in the follicular carcinoma group (p < 0.005). No P16INK4A protein overexpression was noted in nodular goiter cells. High immunohistochemically-detected expression of P16INK4A protein in follicular thyroid carcinoma cells suggests that the altered expression pattern of P16INK4A protein may disturb the regulatory mechanisms of thyreocyte cell cycle and plays a significant role in the formation of benign neoplasms and their malignant counterparts derived from follicular thyroid cell.

Analysis of nm23-H1 protein immunoreactivity in follicular thyroid tumors.

Immunohistochemical analysis employing a monoclonal antibody nm23-H1 (the antibody against nm-23 protein) was performed on archival material, consisting of 12 cases of follicular thyroid carcinoma (FTC), 57 cases of follicular thyroid adenoma (FTA) and 17 cases of nodular goiter (NG). Both cytoplasmic and nuclear immunoreactions for nm-23H1 were observed in cells of FTCs, FTAs and NGs. In oxyphilic adenomas cytoplasmic staining was observed. Eleven (91.7%) cases of FTC, 55 (98.2%) cases of FTA and 14 (82.4%) cases of NG were found to be positive for nm23-H1 protein. There were no statistically significant differences in the mean percentage values of immunopositive cells between carcinomas and adenomas. A significant increase in the number of cases with high percentage (more than 50) of positive cells was found in both carcinomas (FTCs) and adenomas (FTAs)--mainly microfollicular ones, in comparison with nodular goiter. It can be concluded that highly positive immunoreaction for the nm23-H1 protein in the cells of carcinomas (FTCs) and microfollicular adenomas indicates for a high proliferation rate of these tumors.
