Educating interprofessional learners for quality, safety and systems improvement.

Most health professionals in training, as well as those in practice, lack the knowledge and skills they need to play an effective role in systems improvement. Until very recently, these competencies were not included in formal (or informal) educational curricula. Interprofessional collaboration - another core competency needed for successful systems improvement - is also inadequately taught and learned. Achieving Competence Today (ACT) was designed as a new model for interprofessional education for quality, safety and health systems improvement. The core of ACT is a four-module active learning course during which learners from different disciplines work together to develop a Quality Improvement Project to address a quality or safety problem in their own practice system. In this paper we describe the ACT program and curriculum model, discuss our strategies for maximizing ACT's interprofessional potential, and make recommendations for the future.

Transform residency training in chronic illness care -- now.

Identification of the drivers of change that hasten improvement of chronic illness care in resident practices serves as an effective strategy for transformative change. The Academic Chronic Care Collaborative (ACCC) is an ongoing network of 48 teams from 22 teaching hospitals and systems that have committed to implementation of the evidence-based Chronic Care Model in their residency practices. The teams track the results of their efforts by monthly reports of both clinical and educational outcomes. The authors' experience with these self-selected teams shows that the two most effective drivers of change have been organization-wide leadership -- from the CEO to the clinical manager -- and the academic culture, e.g., competitiveness, the aspiration for excellence, focus on the research mission, comfort with data, and commitment to the education mission. These ACCC teams and their sponsoring health systems have successfully harnessed these drivers for the implementation of evidence-based chronic illness care in resident practices.

High mixing rates in the abyssal Southern Ocean.

Mixing of water masses from the deep ocean to the layers above can be estimated from considerations of continuity in the global ocean overturning circulation. But averaged over ocean basins, diffusivity has been observed to be too small to account for the global upward flux of water, and high mixing intensities have only been found in the restricted areas close to sills and narrow gaps. Here we present observations from the Scotia Sea, a deep ocean basin between the Antarctic peninsula and the tip of South America, showing a high intensity of mixing that is unprecedented over such a large area. Using a budget calculation over the whole basin, we find a diffusivity of (39 plus minus 10) x 104[?]m2[?]s-1, averaged over an area of 7 x 105[?]km2. The Scotia Sea is a basin with a rough topography, situated just east of the Drake passage where the strong flow of the Antarctic Circumpolar Current is constricted in width. The high basin-wide mixing intensity in this area of the Southern Ocean may help resolve the question of where the abyssal water masses are mixed towards the surface.

Knowledge for improvement: who will lead the learning?

If we wish to improve the results of a system, what is needed to help health professionals learn about the redesign of that system? To create learning experiences that will attract good health professionals, a special blend of practical insight and critical thinking is required. To enjoy good standing in the academy, these leaders must be able to design, conduct, and disseminate knowledge-building activities subject to peer review. This is the story of the development of the United States Veteran's Health Administration Quality Scholars Program from those who designed and formed it to prepare health professional teachers and academics.
