Assessment of lymphoedema of the lower limbs by the community nurse.

Community nurses are increasingly at the frontline in supporting patients in the management of long-term conditions. They are therefore likely to be involved in identifying and providing care for people who are affected by lymphoedema. Community practitioners will need to develop the skills to recognize patients who have, or are at risk for developing, lymphoedema. Aspects of the nursing assessment of lymphoedema include taking a detailed history and conducting a comprehensive physical and psychosocial assessment of the impact of this condition. The assessment is multifaceted and will often require intervention from other members of the multidisciplinary team.

Modified pyloric exclusion for infants with complex duodenal injuries.

Duodenal injuries among infants are exceedingly rare. The use of pyloric exclusion in the treatment of infants is previously unreported. We report three cases of duodenal injury in infants who were surgically managed through duodenal diversion via pyloric exclusion with concomitant tube gastrostomy. A subsequent discussion of pyloric exclusion highlights the therapeutic rationale, surgical technique, and previous experience with this procedure in children.

The National Lymphoedema Framework Project.

The National Lymphoedema Framework Project originated from an epidemiology study (Moffatt et al, 2003) carried out in South West London, which identified that lymphoedema is a significant but unrecognised problem. Lymphoedema is a chronic swelling arising from the accumulation of protein-rich fluid that would otherwise drain via the lymphatic system. It can be grossly disfiguring, and is associated with cancer and its treatment, particularly breast cancer, but it can occur for other reasons, such as trauma or parasitic infection, or it may be primary in origin (Fig 1). The main aim of the project is to provide sufficient evidence to persuade policy-makers that lymphoedema is a major problem that is currently under-resourced and for which national guidance for management is required.
