[Sustained clinical response of large hepatocellular carcinoma after chemoembolization with pirarubicin, amiodarone and Lipiodol].

Although chemoembolization is known to be an effective palliative treatment in hepatocellular carcinoma, it has a limited effect in large tumors. We report the case of a patient with a large hepatocellular carcinoma of the left liver who had a significant and sustained clinical response after six sessions of chemoembolization with a pirarubicin/amiodarone/lipiodol emulsion. Pirarubicin is an anthracycline which penetrates faster than doxorubicin into cancer cells. Amiodarone is a multidrug resistance inhibitor. Polysorbate 80, an excipient of injectable amiodarone stabilizes the anthracycline/lipiodol emulsion. The clinical efficacy of this new formulation could be evaluated in a phase II clinical trial.

[Adrenal metastasis from follicular thyroid carcinoma with intra-cardiac thrombus].

The authors report the case of a 57-year-old patient operated for adrenal metastasis from follicular thyroid carcinoma with intracardiac thrombus, who subsequently developed disseminated metastases leading to death 9 months after the operation. Adrenal metastases of thyroid carcinomas are rare and can be treated surgically when they are isolated. These metastases may appear many years after initial thyroid surgery requiring regular surveillance, especially by serum thyroglobulin assay in the case of well differentiated carcinoma.

Magnetic resonance imaging in the assessment of synovial inflammation of the hindfoot in patients with rheumatoid arthritis and other polyarthritis.

OBJECTIVES: To describe the localisation of synovitis and tenosynovitis of the hindfoot observed on magnetic resonance imaging (MRI) in patients with chronic polyarthritis, and to correlate the findings of physical examination and MRI. METHODS: Patients with chronic polyarthritis, and one or two painful hindfoot were included. On physical examination and on MRI, the tibio-talar, talo-calcaneal, and talo-navicular and calcaneo-cuboidal joints were adjudged to have or not synovitis, and the tibialis anterior and posterior, the peroneus longus and brevis, the flector digitorum and hallucis longus tendons to have or not tenosynovitis. Criteria for synovitis and tenosynovitis were a high signal intensity on T2-weighted images, a low signal intensity on T1-weighted images, and enhancement after Gd-DTPA injection, in the joint area, and around the tendon, respectively. The correlation between the findings of physical examination and those of MRI were evaluated using the Kappa statistics. RESULTS: 12 patients (three men, nine women, mean age of 55.5 years+/-11.4 S.D.) with chronic polyarthritis (rheumatoid arthritis (RA): nine, ankylosing spondylitis: one; psoriatic arthritis: one, unclassified: one) were included. All presented with one (7 patients) or two (5 patients) painful hindfeet (and swelling for 16 out of 17 hindfeet). On physical examination, 25 joints and eight tendons were adjudged to have synovitis and tenosynovitis. MRI showed synovitis in 12 out of 25 of these joints (48%), and tenosynovotis in three out of eight of these tendons (37.5%). Moreover, MRI showed ten and seven clinically unsuspected synovitis and tenosynovitis, respectively. The proportion of agreements between physical examination and MRI were 54.9% (kappa=0.1) and 88.2% (kappa=0.27) for synovitis and tenosynovitis, respectively. CONCLUSION: A weak correlation was observed between the findings of physical examination and MRI in patients with chronic polyarthritis and a painful hindfoot. MRI might be used to localise synovitis in the area before performing some intra-articular injections. However, other studies are needed to address this question.

[Management of the emphysematous pyelonephritis].

We report two cases of emphysematous pyelonephritis in diabetic patients. The first suffered from emphysematous pyelonephritis and emergency nephrectomy was performed. The second patient suffered from emphysematous pyelitis and was successfully treated by nephrostomy. We present a comprehensive review of the literature on emphysematous pyelonephritis and discuss the indications and the results of treatments of this rare and severe infection.

Cervical Chymopapain Chemonucleolysis.

Cervical chemonucleolysis probably represents the treatment of choice in case of a C6-C7 radiculalgia. This technique must be performed by specialists, given the particular anatomy which is in front of the cervical disk, within the soft tissues. For the future it is necessary to delete the manufacturer's restrictions concerning chymopapain use at the cervical level.

MR cholangiography in the evaluation of hepatic and biliary abnormalities in autosomal dominant polycystic kidney disease: study of 93 patients.

PURPOSE: The purpose of this work was to conduct an MR cholangiography study of hepatobiliary abnormalities in autosomal dominant polycystic kidney disease (ADPKD) and to correlate these abnormalities with the risk of infection. METHOD: Cystic and intrahepatic bile duct (IHBD) abnormalities identified by MR cholangiography in 93 ADPKD patients were studied retrospectively. A blind study of liver function tests, renal insufficiency, and infectious episodes was also carried out. Correlations among these data were looked for using univariate analysis. RESULTS: Intrahepatic cysts were present in 84 of 93 patients and peribiliary cysts in 59 of 90 patients. IHBDs were abnormal in 25 of 90 patients (tubular dilatation in 15 cases and bead-like dilatation in 16). IHBD abnormalities were correlated with hepatobiliary infections (p = 0.0012), gamma-glutamyltranspeptidase elevation (p = 0.018), and terminal renal failure (p = 0.006). CONCLUSION: This study confirms the complexity of hepatobiliary involvement in ADPKD. Various types of cystic lesions can arise, and patients with IHBD abnormalities are at increased risk for hepatobiliary infection.

Liver tumor characterization: comparison between liver-specific gadoxetic acid disodium-enhanced MRI and biphasic CT--a multicenter trial.

OBJECTIVE: In our multi center trial we compared the potentials of biphasic contrast-enhanced computed tomography (CT) and a novel tissue-specific magnetic resonance imaging (MRI) contrast agent gadoxetic acid disodium in liver lesion characterization. METHODS: A total of 176 patients with 252 liver lesions were analyzed. There were 104 malignant and 148 benign lesions. High-field strength (1.0 T or 1.5 T) MR systems with T1-and T2-weighted sequences were used with and without fat suppression. After gadoxetic acid disodium injection, dynamic imaging and hepatocyte phase MR imaging were performed. Biphasic with 150 mg I/kg of body weight (100-200 mL) spiral CT was also performed. Image reading consisted of on-site (by study investigators) and fully blinded off-site (by E.S.P; C.R; and A.S) evaluations. The classification (benign or malignant) and characterization (lesion type) outcomes of both techniques were assessed. All imaging results were verified against a standard of reference. RESULTS: Both on-site and off-site evaluations demonstrated increases in the lesion classification accuracy with gadoxetic acid disodium-enhanced MRI when compared with spiral CT. This improvement was also shown for characterization. Gadoxetic acid disodium was well tolerated. CONCLUSIONS: Gadoxetic acid disodium offers a safe and diagnostically powerful tool for the evaluation of patients with focal liver lesions with a reliable assessment of lesion classification and characterization.
