Congenital cystic adenomatoid malformation associated with ipsilateral eventration of the diaphragm.

A two-month-old girl was operated for a left-sided eventration of the diaphragm. At surgery, a mass representing an extralobar pulmonary sequestration (EPS) was found inferior to the left lobe of the lung and was excised. Histopathological examination of the resected specimen showed presence of a type II congenital cystic adenomatoid malformation (CCAM) within it. A review of the relevant literature is presented.

Lumbosacral parasitic rachipagus twin.

A rare case of a newborn girl presenting with a mass representing an aborted parasitic twin attached to the back in the midline over the lumbosacral region (parasitic rachipagus) is reported. Rudimentary digits were attached to the mass, and in one area, the overlying skin resembled scrotal skin. A depression resembling an anal dimple was also present on the surface of the mass. Investigations showed a well-developed long bone and other small bones within the parasite, and sacral spina bifida in the autosite. Successful surgical excision of the parasitic mass was performed. A review of the literature pertaining to parasitic rachipagus twinning is presented.

Application of GC-EI-MS for the identification and investigation of positional isomer in primaquine, an antimalarial drug.

A major impurity associated with primaquine drug samples obtained from European Pharmacopoeia (EP) and other commercial sources was detected and identified using HPLC, photo diode array (PDA), LC-MS/MS and gas chromatograph-electron impact-mass spectrometer (GC-EI-MS). PDA and LC-ESI-MS/MS data provided an evidence for it being isomeric in nature. However, spectral data obtained from the newly developed GC-EI-MS method has been utilised for structural elucidation and found to be conclusive to characterize this impurity as positional isomer, i.e. 8-(4-amino-4-methylbutyl amino)-6 methoxyquinoline. The structure of this impurity has been confirmed by its synthesis. Precursor of primaquine was also investigated using GC-EI-MS. The data obtained confirmed the origin of isomeric impurity in precursor.

Isolated liver tuberculosis: a case report.

A case of liver tuberculosis resembling a hemangioendothelioma on computed tomography and clinically masquerading as a tumor is reported. The cause of isolated liver tuberculosis and the importance of an open liver biopsy is discussed, with a review of the literature.

In situ hybridization in the histological diagnosis of early and clinically suspect leprosy.

The present study tests the utility of the in situ hybridization procedure for M. leprae rRNA in the histological diagnosis of early leprosy and clinically suspect leprosy, both diagnostically demanding situations. The histological confirmation obtained with routine histopathology (Haematoxylin-Eosin staining for studying morphologic alterations and Fite-Faraco staining for demonstration of acid-fast bacilli) were 32% for early leprosy and 25% for clinically suspect leprosy. With performance of the in situ hybridization on the histologically unconfirmed cases, the positivity rates obtained were 58.8% and 55%, respectively. The results of the study confirm the utility of the procedure in the diagnostically difficult situations of early and suspect leprosy, and it is proposed that the procedure be employed in situations of clinical doubt.

Coagulopathy associated with Wilms' tumour: a rare complication.

Coagulopathies associated with malignancies are not extremely rare, but are uncommonly associated with Wilms' tumour (WT). A 6-month-old female with a WT and an abnormal coagulation profile is reported. Due to the Coagulopathy and elevated D-dimers in the blood, surgery was deferred and chemotherapy was given, after which the coagulation profile became normal.

A Simple Solution to the Age Old Problem of Regioselective Functionalization of Guanine: First Practical Synthesis of Acyclic N(9)- and/or N(7)-Guanine Nucleosides Starting from N(2),N(9)-Diacetylguanine.

Regioselective alkylation of guanine, a long-lasting challenge, has been overcome by understanding the role of acids as catalyst in the coupling reaction of DAG (10) with OBDDA (11). The acid-catalyzed and noncatalyzed reactions of 10 with OBDDA which mainly give N(7) and N(9) isomers, respectively, appear to follow different mechanisms. The practical utility of the noncatalyzed reaction, which gives almost quantitative yields of N(9) derivatives, is demonstrated by synthesizing acylovir/gancyclovir in high yields.

Total gastric necrosis in a neonate.

Necrosis of the entire stomach is extremely rare in neonates. We report a case of total gastric necrosis in a full-term neonate. The possible pathophysiology, diagnostic features and therapeutic options for gastric necrosis in a newborn are discussed.
