Breast disorders in children and adolescents.

Concerns about problems of the breast are often noted in adolescents and their parents. This review discusses issues and disorders of the breast in children and adolescents, starting with basic principles of embryology and adolescent breast development. Concepts that are covered include congenital breast disorders, abnormal timing of breast development, breast asymmetry, underdeveloped breasts, breast atrophy, tuberous breasts, mammary hyperplasia, fibroadenoma, giant fibroadenoma, cystosarcoma phyllodes, intra-ductal breast papilloma, adenocarcinoma, mastitis, traumatic breast disorders, benign breast disease, fibrocystic change, mastalgia, gynecomastia, and galactorrhea. A number of figures are provided illustrating breast pathologic findings. Clinicians caring for children and adolescents are encouraged to provide careful assessments of this important organ.

Dysfunctional uterine bleeding in adolescents: concepts of pathophysiology and management.

DUB is extremely common in young adolescents, and can be regarded asa part of normal physiologic development. It is essential to have a firm grasp on normal physiologic development of the menstrual cycle to recognize the normal menstrual patterns of adolescents and to manage abnormal patterns that may develop. Specific management depends on the underlying cause, presence and extent of any existing anemia, and duration of the irregular menstrual pattern.

Syndromes and disorders associated with mental retardation.

This article reviews selective comments on the concept of Mental Retardation (MR) in adolescents. Issues covered include the definition, prevalence, and differential diagnosis of MR. Some of the syndromes and disorders associated with MR in the adolescents are also considered with emphasis on the behavioral concerns that may be present in this age group. Finally, concepts of management by the clinician are reviewed. It is recommended that health care professionals caring for adolescents with MR should help these youths maximize their potential as human beings, helping them achieve meaningful functioning in adulthood.

Pharmacologic treatment of attention-deficit hyperactivity disorder.

Attention-deficit hyperactivity disorder (ADHD) is highly prevalent in children and adolescents. Highly effective pharmacological treatments are available that allow the child and the adolescent to function at his/her full potential. Various preparations of methylphenidate and amphetamines have been used for a long time in the treatment of ADHD. This article reviews these and some of the newer drugs used in the treatment of ADHD, including atomoxetine and bupropion.

Kidneys and sports.

With better understanding of the nature of renal disease and its treatment, many more adolescents are now allowed to participate in recreational and competitive sports. The positive physiologic and psychological effects of exercise are increasingly being appreciated in adolescents with chronic diseases. This article reviews relevant aspects of renal disease that have implications for sports participation by adolescents, including hematuria, proteinuria, hyponatremia, hypertension, solitary kidney, exercise-related acute renal failure, and chronic/end-stage renal disease. It also reviews the renal effects of creatine and protein supplementation in athletes.

The college athlete.

Participation in sports is important to many college students. Student athletes come from different levels of previous sport experience as they enter collegiate athletics. The primary source of student medical care is the campus student health center. The health care providers at student health centers attend to many of the sports-related concerns of student athletes. Preparticipation evaluation provides an opportunity to assess the general health of the student athlete and to identify conditions that might increase the risk of further injury. Sudden cardiac death and sports-associated concussions have generated much interest and are reviewed in this article. Other areas reviewed here include use of drugs and supplements, ankle sprains, acute knee ligament injuries, back pain, and shoulder impingement syndrome.

Attention-deficit/hyperactivity disorder psychopharmacology for college students.

This article is written as a practical guide for the use of medications in the management of attention-deficit/hyperactivity disorder in college students. It includes a review of the common medications used and an algorithm of treatment using these medications. Practical guidelines are presented along with comments regarding more common side effects. The article also addresses some concerns about abuse of psychostimulants by college students.

Contraception for college students.

An important issue for human beings is the acquisition of normal sexual health, including the understanding and application of reproductive health when needed. Comprehensive sexuality education is not a topic provided to many children, adolescents, or college students in the United States, however. Students who were sexually active in high school may continue to be at risk for unwanted pregnancies and sexually transmitted diseases in their college life; those who chose abstinence in high school may abandon this concept in college, choosing coital behavior at all levels of university life-freshman through graduate levels. Most American college students are sexually active and many have multiple partners. This article reviews current contraceptive methods available to college students. College health providers and pediatricians are urged to be vigilant about the reproductive health needs of the college students they serve.

Value of sports pre-participation examination in health care for adolescents.

Though adolescence is typically perceived as a time of good health in many places in the world,the reality is that adolescents have many health problems, including medical and behavioral disorders. The ideal location for health maintenance visits is the office of a clinician who has an established relationship with the adolescent patient. The clinician can use the interest of youth for sports to improve the health care of these youth in various ways, as reviewed in this article. A sports pre-participation examination (PPE) is recommended before the adolescent engages in organized sports. During this evaluation, various issues can be covered, including anticipatory guidance for sports participation, stress involved with organized sports, drug or supplement abuse in sports, eating disorders, and weight control practices, in-jury prevention, principles of sports participation, the importance of regular physical activity and normal growth and development. The pre-participation examination is reviewed in this article, including taking a proper history, performing the physical examination, and ordering appropriate laboratory tests. Youth with medical illnesses can be clinician-directed to the sports activities that are best and safest for them. Keeping youth active in some type of physical activity is important and thus, the examination can be used to allow full or partial participation in all or only some sports. This paper summarizes the clinician's approach to using sports activities as a way of improving the health of youth.

Substance abuse in adolescents: a complex conundrum for the clinician.

Substance abuse remains a complex and pervasive conundrum for society and for clinicians seeking to improve the lives of their pediatric patients. Substance abuse is linked to the human instinct for pleasure at any cost and is fueled by enticing encouragement of the media teaching society to seek drug-induced pleasure without fear of negative consequences. Other complications are the limited education about psychoactive substances provided to youth and the health care profession pledged to serve them. Primary care clinicians must provide their adolescent patients with adequate screening and counseling about substance abuse. Treatment of the substance-abusing patient is often a combination of behavioral interventions (including family therapy), and, in limited situations, addiction-specific medications. Research suggests that female drug addicts have a better outcome in female-only drug treatment programs. In addition, new drugs are being developed that target specific brain mechanisms involved in drug addiction; these drugs will have less toxicity and less abuse potential than illicit drugs such as cocaine. Vaccines are being developed that will block the effects of such drugs as cocaine and PCP. Medications developed for the treatment of depression and epilepsy will be a source of medications for the treatment of drug addiction. The study of endorphins and the neurobiology of stress and substance abuse promise to develop potent anti-addiction chemicals, greatly aiding in the war on drug abuse.

Attention-deficit/hyperactivity disorder in children and adolescents: interventions for a complex costly clinical conundrum.

Management of a child or adolescent with attention-deficit/hyperactivity disorder (ADHD) is reviewed, including psychological and pharmacologic approaches. Psychological treatment includes psychotherapy, cognitive-behavior therapy, support groups, parent training, educator/teacher training, biofeedback, meditation, and social skills training. Medications are reviewed that research has revealed can improve the core symptomatology of a child or adolescent with ADHD. These medications include stimulants, antidepressants, alpha-2 agonists, and a norepinephrine reuptake inhibitor. Management of ADHD should include a multi-modal approach, involving appropriate educational interventions, appropriate psychological management of the patient (child or adolescent), and judicious use of medications. Parents, school officials, and clinicians must work together to help all children and adolescents with ADHD achieve their maximum potential.

Violence: concepts of its impact on children and youth.

Although nations continue to remain involved in ongoing armed conflicts, the threat of direct exposure to violence for American children and youth is more likely to be from the interpersonal violence that occurs in homes, neighborhoods, and schools. Exposure to interpersonal violence has a very serious impact on most youth. Focusing on violence prevention remains a vital component of providing comprehensive health care for all youth.

Behavioral aspects of children's sports.

This article considers some of the numerous behavioral aspects that are related to children's sports. The pediatrician, during various encounters with the athlete and his or her parents in the office, or on the field, can positively influence the sport participation experience of the athlete, by screening and identifying potential problem areas as well as providing anticipatory guidance on various issues.
