Correlation of cochlear blood supply with mitochondrial DNA common deletion in presbyacusis.

OBJECTIVE: To study the relationships between cochlear hypoxia, mitochondrial (mt) DNA4977 deletion and metabolic features of mtDNA in presbyacusis. MATERIAL AND METHODS: Sixty-seven temporal bones from a presbyacusis group, an age-matched control group and a young and middle-aged control group were involved in the experiment. Nested and tri-nested polymerase chain reactions (PCRs) were applied to test for the presence of the mtDNA4977 deletion. Computer imaging processing was used to measure blood vessel parameters in the internal acoustic meatus (IAM). RESULTS: The mtDNA4977 deletion was detected in 17/34 ears in the presbyacusis group, 4/19 ears in the age-matched control group and 0/14 ears in the young and middle-aged control group. In the presbyacusis group, the lumen of the vasa nervorum of the IAM showed a more severe narrowing in cases with than without the mtDNA4977 deletion. CONCLUSION: The high incidence of the mtDNA4977 deletion in the temporal bones of presbyacusis patients suggests a correlation between the mtDNA4977 deletion and presbyacusis. Hypoxia of the cochlea may cause the mtDNA4977 deletion and other mtDNA mutants and furthermore may cause a reduction in mitochondrial oxidative phosphorylation and decreased function of the acoustic neural system. The symptoms of presbyacusis may occur when the function of the acoustic neural system is impaired as a result of abnormal mtDNA metabolism reaching a particular threshold.

[Pedigree analysis and sequence analysis of mtDNA 12srRNA, tRNA(Leu(UUR)), tRNA(Ser(UCN)) gene in nonsyndromic inherited deafness pedigrees].

OBJECTIVE: To investigate the proportion of mtDNA mutation in the non-syndromic genetic hearing loss (NSHL) pedigrees and the genetics statistical formulae for maternal inheritance, to study the relationship of mtDNA mutation and inherited deafness, to identify the incidence of the mtDNA mutation in such pedigrees and sporadic patients with Sensorineural hearing loss (SNHL). METHOD: Twenty-nine pedigrees with NSHL were collected. Pedigree Investigation was taken. Modal Genetics Analysis. Segregation Analysis were taken. Blood samples were obtained from these pedigrees. DNA was extracted from the isolated leukocytes. The mtDNA 1555G, 7445G, 3243G mutation were examined by multiplex PCR. The sequence of 12SrRNA, tRNA(Leu(UUR)) and tRNA(Ser(UCN)) gene were examined. RESULT: There are 12 pedigrees with mtDNA mutation (i.e. 10 with 1555G and 2 with 7445G) examing by multiplex PCR. Modal Genetics Analysis showed that in irregular dominate genetic pedigrees, the incidence of mtDNA mutation is higher than that of regular dominate pedigrees. Segregation Analysis with Screening for mtDNA mutation showed that maternal inherited pedigrees did not have the segregate ratio that the autosomal inheritance had. Sequence analysis confirmed that the 12 pedigrees carried mtDNA mutation, among them 10 pedigrees with 1555G mutation, 2 pedigrees with 7445G mutation, no pedigrees with 3243G. CONCLUSION: Maternal inherited pedigrees do not have the segregate ratio of the autosomal inheritance, mtDNA mutation have high incidence in NSHL, mostly are 1555G and 7445G mutation. Screening for mtDNA 7445G mutation combined with 1555G examination is of value to clinical use. Multiplex PCR can diagnose mtDNA multi-mutation quickly and facilely.

[Objective multi-parameters analysis of the voice dysphonia].

OBJECTIVE: The purposes of this paper are to develop a multi-parametric protocol, which can evaluate the voice objectively and quantitatively, and to investigate its correlation with the perceptual analysis. METHOD: The voice samples were collected from 83 patients with dysphonia and 40 subjects with normal voices. All the subjects were women. The objective parameters, including fundamental frequency (F0), jitter, Shimmer, fundamental frequency standard deviation (F0SD), fundamental frequency tremor, amplitude tremor, normalized noise energy (NNE), harmonic-to-noise ratio(HNR), signal-to-noise ratio(SNR) and maximal phonatory time (MPT), which were measured mainly on a sustained vowel /a/, were recorded on a software named Dr. Speech for Windows. According to the G component of the GRBAS, the voice was graded from 0 for normal to 3 for severe dysphonia. RESULT: Using the discriminate analysis, a five parameters protocol (MPT, jitter, NNE, HNR and shimmer) was developed and it showed that this protocol allowed 79.8% concordance with jury classification. CONCLUSION: The voice objective analysis should be multi-parameters. Our multi-parameters evaluation protocol is able to reflect the result of perceptual analysis.

Maternally inherited aminoglycoside-induced and nonsyndromic deafness is associated with the novel C1494T mutation in the mitochondrial 12S rRNA gene in a large Chinese family.

We report here the characterization of a large Chinese family with maternally transmitted aminoglycoside-induced and nonsyndromic deafness. In the absence of aminoglycosides, some matrilineal relatives in this family exhibited late-onset/progressive deafness, with a wide range of severity and age at onset. Notably, the average age at onset of deafness has changed from 55 years (generation II) to 10 years (generation IV). Clinical data reveal that the administration of aminoglycosides can induce or worsen deafness in matrilineal relatives. The age at the time of drug administration appears to be correlated with the severity of hearing loss experienced by affected individuals. Sequence analysis of mitochondrial DNA in this pedigree identified a homoplasmic C-to-T transition at position 1494 (C1494T) in the 12S rRNA gene. The C1494T mutation is expected to form a novel U1494-1555A base pair, which is in the same position as the C1494-1555G pair created by the deafness-associated A1555G mutation, at the highly conserved A site of 12S rRNA. Exposure to a high concentration of paromomycin or neomycin caused a variable but significant average increase in doubling time in lymphoblastoid cell lines derived from four symptomatic and two asymptomatic individuals in this family carrying the C1494T mutation when compared to four control cell lines. Furthermore, a significant decrease in the rate of total oxygen consumption was observed in the mutant cell lines. Thus, our data strongly support the idea that the A site of mitochondrial 12S rRNA is the primary target for aminoglycoside-induced deafness. These results also strongly suggest that the nuclear background plays a role in the aminoglycoside ototoxicity and in the development of the deafness phenotype associated with the C1494T mutation in the mitochondrial 12S rRNA gene.

Familial auditory neuropathy.

OBJECTIVES/HYPOTHESIS: Auditory neuropathy is a sensorineural hearing disorder characterized by absent or abnormal auditory brainstem responses and normal cochlear outer hair cell function as measured by otoacoustic emission recordings. Many risk factors are thought to be involved in its etiology and pathophysiology. Four Chinese pedigrees with familial auditory neuropathy were presented to demonstrate involvement of genetic factors in the etiology of auditory neuropathy. STUDY DESIGN: Probands of the above-mentioned pedigrees, who had been diagnosed with auditory neuropathy, were evaluated and followed in the Department of Otolaryngology-Head and Neck Surgery, China People Liberation Army General Hospital (Beijing, China). Their family members were studied, and the pedigree maps established. METHODS: History of illness, physical examination, pure-tone audiometry, acoustic reflex, auditory brainstem responses, and transient evoked and distortion-product otoacoustic emissions were obtained from members of these families. Some subjects received vestibular caloric testing, computed tomography scan of the temporal bone, and electrocardiography to exclude other possible neuropathic disorders. RESULTS: In most affected patients, hearing loss of various degrees and speech discrimination difficulties started at 10 to 16 years of age. Their audiological evaluation showed absence of acoustic reflex and auditory brainstem responses. As expected in auditory neuropathy, these patients exhibited near-normal cochlear outer hair cell function as shown in distortion product otoacoustic emission recordings. Pure-tone audiometry revealed hearing loss ranging from mild to profound in these patients. Different inheritance patterns were observed in the four families. In Pedigree I, 7 male patients were identified among 43 family members, exhibiting an X-linked recessive pattern. Affected brothers were found in Pedigrees II and III, whereas in pedigree IV, two sisters were affected. All the patients were otherwise normal without evidence of peripheral neuropathy at the time of writing. CONCLUSION: Patients with characteristics of nonsyndromic hereditary auditory neuropathy were identified in one large and three smaller Chinese families. Pedigree analysis suggested an X-linked, recessive hereditary pattern in one pedigree and autosomal recessive inheritances in the other three pedigrees. The phenotypes in the study were typical of auditory neuropathy; they were transmitted in different inheritance patterns, indicating clinical and genetic heterogeneity of this disorder. The observed inheritance and clinical audiological findings are different from those previously described for nonsyndromic low-frequency sensorineural hearing loss. This information should facilitate future molecular linkage analyses and positional cloning for the relative genes contributing to auditory neuropathy.

[Significance of p53, c-myc and nm23 protein expression in recurrent laryngeal carcinoma].

OBJECTIVE: To evaluate the prognosis of postoperative laryngeal cancer recurrence, the study was designed to investigate the relationship of p53, c-myc and nm23 protein expression in recurrent laryngeal carcinoma. METHOD: Five hundred and seventy-four cases of laryngeal carcinoma were retrospectively reviewed. All the patients were observed for 3 years postoperatively, 56 cases with recurrence were diagnosed and 39 cases of normal laryngeal tissue were compared. The tissue microarrays was made in all the cases. The tissue chip was analyzed by the SABC immunohistochemical method. RESULT: p53, c-myc and nm23 expressed in most of the laryngeal carcinoma. The positive ratio is different. There is higher expression of c-myc in recurrent laryngeal carcinoma, and its expression has significant difference with the laryngeal carcinoma, which have no recurrence (P < 0.01). nm23 has lower expression in recurrent laryngeal carcinoma, and it's expression has significant difference with the laryngeal carcinoma which have no recurrence(P < 0.01). The expression of p53 has no significant difference between recurrent laryngeal carcinoma and the laryngeal carcinoma which have no recurrence(P > 0.05). There is the positive correlation between the expression of c-myc and the recurrent laryngeal carcinoma; and negative correlation between the expression of nm23 and the recurrent laryngeal carcinoma. CONCLUSION: c-myc and nm23 could be the prognosis marker of postoperative laryngeal cancer recurrence.

[Clinic feathers of cholesteatoma in children].

OBJECTIVE: In order to identify the different feathers of cholesteatoma of the middle ear between children and adults. METHOD: We investigated retrospectively 35 cases (40 ears) of cholesteatoma in the middle ear of children, as well as 20 cases (21 ears) of adults. RESULT: The results revealed that cholesteatoma in children manifested more extensive distribution of lesion as well as more serious ossicular chain damage than those in adults. A statistically significant relationship was revealed between lesion extent and ossicular chain damage. Hearing level pre-operation and post-operation were similar. CONCLUSION: Cholesteatoma is a more extensive and aggressive disease in the pediatric age group than in adults. Most children with an extensive disease require a CWD procedure. Hearing preservation depends on the extent of the disease, pre-operation hearing level and the surgical procedure chosen.

[A longitudinal study of electrode impedance in nucleus 24M cochlear implant users].

OBJECTIVE: The primary goal of this study was to examine changes that may occur in electrode impedance over time in cochlear implant users. Secondary goal was to determine the timetable of mapping for patients who were cooperative quite well in psychophysical test. METHOD: Sixteen patients implanted with Nucleus CI24M participated in this study. The impedance of electrode was measured intra-operatively, at initial stimulation, and at each mapping visits. Statistical analysis was conducted to assess how the changes occurring over time. RESULT: There was a significant increase in electrode impedance from the intra-operative test to initial stimulation, and a significant decrease in impedance from initial stimulation to the following visit. The impedance values remained stable for approximately 2-4 months after initial stimulation. CONCLUSION: The results suggest that electrode-tissue interface gradually tend to remain stable. If a patient cooperates quite well in psychophysics test, the psychophysical levels for the map should be stable after 2 to 4-month mapping.

[Tinnitus retraining therapy: a clinical control study of 117 patients].

OBJECTIVE: To investigate the clinical effects of tinnitus retraining therapy (TRT) on tinnitus. METHODS: 225 tinnitus patients were divided into two groups. 117 patients in the TRT group were treated with TRT and drugs, and 108 patients in the control group were treated with only tinnitus masking and drugs. The TRT is consisted of four strategies: (1) tinnitus masking with low level and broad band noise; (2) deep relaxation of the whole body; (3) diversion of the attention to other things; and (4) psychological counseling and therapy. Drugs, such as vasodilator, neurotrophic drug, and sedative of the similar dose and duration of pharmacotherapy were administered to the 2 groups. Effect evaluation was conducted thrice 2, 6, and 12 months after the beginning of therapy to see if the tinnitus was attenuated or disappeared and if the patients' emotion, sleep, and work were disturbed by tinnitus. RESULTS: The relief rate was 17.09%, 82.05%, and 88.03% in the TRT group 2, 6, and 12 months after respectively; and 2.78%, 26.85%, and 41.6% in the control group 2, 6, and 12 months after respectively (chi(2) = 12.54, 69.30, and 63.64, all P < 0.01). CONCLUSION: TRT is effective in treatment of tinnitus.

[Diagnosis and treatment of glomus jugulare tumor].

OBJECTIVE: To investigate the clinical features, diagnosis, and treatment of glomus jugulare tumor. METHODS: The data of 37 patients of glomus jugulare tumor diagnosed by digital subtraction angiography (DSA) and/or operation and pathology, 14 males (37.8%) and 23 females (62.2%), with an average age of 37.2 (3.5 approximately 66 years) and an average course of 4.3 years (1 month - 19 years), were analyzed. RESULTS: The tumor was located in the left ear in 19 cases, in the right ear in 17 cases, and in both ears in 1 case, totally 38 ears. The tumor was chromaffin in one case with hypertension. One case was complicated by ipsilateral carotid body tumor, and 2 cases were complicated by ipsilateral cholesteatoma. Thirty cases (81.1%) presented pulsatile tinnitus and hearing loss as the main symptoms. Thirteen cases (35.1%) were diagnosed as glomus jugulare tumor at the first visit, and 24 cases (64.9%) were diagnosed as other diseases with an average misdiagnosis period of 4.4 years. Eighteen tumors originated from typanum and 28 tumors from glomus jugulare. According the Fisch classification 1981, the tumors in 11 ears were type A, in 7 ears were type B, in 7 ears type C, in 5 ears type D1, and in 8 ears type D2. All cases, except the one with bilateral lesions, underwent operation. The tumor was completely resected in 30 cases. Postoperatively, facial paralysis was seen in 6 cases, dizziness, hoarseness, and subauricular necrosis was seen in one case respectively. Of the 25 patients followed up with a mean follow-up time of 5.2 years (1.1 - 16.1 years), 3 died of lung cancer, neuroblastoma, or extensive involvement of glomus jugulare tumor respectively, 19 survived without tumor, and 3 survived with the tumor. CONCLUSION: Glomus jugulare tumor is likely to involve surrounding important tissues, and to be misdiagnosed. Early surgical removal of the lesion is important.

[Screening for mitochondrial DNA mutation in two pedigrees with nonsyndromic inherited sensorineural hearing loss].

OBJECTIVE: To investigate the genetic mechanism of maternal nonsyndromic inherited sensorineural hearing loss(SNHL), to identify the incidence of the 7445(G) mutation in such pedigrees and sporadic patients with SNHL, and to provide the theoretical evidence for the diagnosis of this disease. METHODS: Blood samples were obtained from 2 pedigrees and 14 sporadic patients with SNHL. DNA was extracted from the isolated leukocytes. The mitochondrial DNA (mtDNA) fragments were amplified by PCR. The 1555(G), 3243(G) and 7445(G) mutation was detected by Alw 26 I, Apa I and Xba I restriction endonuclease digestion respectively. The sequence of 12S rRNA, tRNA(Leu(UUR)) and tRNA(Ser(UCN)) was examined. RESULTS: Restriction endonuclease digestion analysis showed that 12 individuals from 2 pedigrees carried homoplasmic 7445(G) mutation, which was of maternal inheritance. Six individuals from 2 pedigrees and 14 sporadic patients did not have 7445(G) mutation. All individuals did not have 1555(G) and 3243(G) mutation. The sequence analysis further showed that none of them carried homoplasmic 1555(G) and 3243(G) mutation, 12 individuals had (nt)7445 A--> G substitution in tRNA(Ser(UCN)) gene. CONCLUSION: The incidence of 7445(G) mutation in such pedigrees is higher than that in sporadic patients. Screening for mtDNA 7445(G) mutation combined with 1555(G) examination is of much value to clinical use.

Deletions are easy detectable in cochlear mitochondrial DNA of Cu/Zn superoxide dismutase gene knockout mice.

OBJECTIVES: To investigate the tissue specificity of reactive oxygen species (ROS) damage to mitochondrial DNA (mtDNA) and to determine whether cochlear mtDNA is a sensitive target for ROS damage. METHODS: 10 Cu/ZnSOD gene (Cu/Zn superoxide dismutase gene, Sod1) knockout mice and 16 wild-type mice were analyzed by nested polymerase chain reaction (PCR). RESULTS: Three deletions were detected in various tissues of Sod1 knockout mice. MtDNA3867bp and mtDNA3726bp deletions were the most visible, and mtDNA4236bp deletion was barely detected in these tissues. There were obvious differences in the ratio of deleted mtDNA/total mtDNA in different tissue. Deleted mtDNA was most abundant in the liver and kidney and less in cochlea, heart and brain. The lowest was in spleen and skin. The ratio in various tissues was 3 - 20 times in Sod1 knockout mice over wild-type mice. In cochlea, the ratio was about 15. CONCLUSIONS: Without the protection of Sod1, ROS can lead to mtDNA deletions in various tissues with significant tissue specificity. Cochlear mtDNA is a sensitive target for ROS damage.

Cochlear mitochondrial DNA3867bp deletion in aged mice.

OBJECTIVES: To study the status of cochlear mitochondrial DNA (mtDNA) and to determine the location of mtDNA deletion in aged mice. METHODS: We detected cochlear mtDNA in 2, 7 - 10 and 17 - 19 month old mice by nested polymerase chain reaction (PCR) and DNA sequencing. RESULTS: mtDNA3867bp deletions were found in the cochleae of aged mice. The deletion occurred within nt9103-nt12970 and were flanked by 15 base pair direct repeats. Comparing the incidence of mtDNA3867bp deletions, 17 - 19 month old mice (7/8) were significantly higher than 7 - 10 month old mice (4/16). The deletion was not observed in 2 month old mice (0/7). The ratio of deleted mtDNA/total mtDNA in 17 - 19 month old mice was higher than in 7 - 10 month old mice (P < 0.001). CONCLUSION: Cochlear mtDNA 3867bp deletion in aged mice may be related to presbycusis.
