Macluralisin--a serine proteinase from fruits of Maclura pomifera (Raf.) Schneid.

A serine proteinase was isolated from fruits of Maclura pomifera (Raf.) Schneid. by affinity chromatography on bacitracin-containing sorbents and gel-filtration. The enzyme, named macluralisin, is a glycoprotein with a molecular mass of 65 kDa; its protein moiety corresponds to a molecular mass of 50 kDa. The substrate specificity of macluralisin towards synthetic peptides and insulin B-chain is similar to that of cucumisin, a subtilisin-like proteinase from melon fruit. The enzyme is completely inhibited by diisopropylfluorophosphate. Its amino-acid composition resembles that of a serine proteinase isolated from the Cucurbitaceae. The N-terminal sequence has 33% of its residues identical to those of the sequence of fungal subtilisin-like proteinase K. Hence, Maclura pomifera serine proteinase belongs to the subtilisin family, which seems to be broadly distributed in the plant kingdom.

[Evaluation of the long-term results of the surgical treatment in patient with vaginal aplasia in the presence of functioning uterus].

Results of surgical treatment of 28 adolescent girls with vaginal aplasia and functioning uterus are analyzed. The uterus had to be removed in all the patients 1.5 to 6 months after abdominal colpopoiesis with preservation of functioning uterus; after sigmoidal colpopoiesis the uterus was preserved in 85% of patients. Analysis of the contents of the uterine cavity showed endometritis, which was evidently caused by the absence of the cervix uteri, a natural barrier preventing the penetration of intestinal flora of neovaginal mucosa into the uterine cavity. The studies permitted a new interpretation of the results of abdominal and sigmoidal colpopoiesis with preservation of functioning uterus in adolescent girls with vaginal aplasia.

[The role of echography in the complex dynamic examination of patients with retardation of sexual development].

Seventy-nine patients with sexual development retardation were examined, 24 of these suffered from ovarian genesis condition and 55 from central genesis condition. The findings evidence that detection of the uterus and gonads presenting as cords is one of the diagnostic criteria indicating gonadal dysgenesis. Echographic examinations carried out over the course of therapy yield a more accurate picture of ovarian function. No increase in uterine size on the echogram after discontinuation of hormonal therapy and the appearance of follicles in the ovaries after treatment point to normally functioning ovaries and helps specify the origin of sexual development retardation. In sexual development retardation of a central origin ultrasonic scanning helps assess the therapy efficacy and predict its outcome.

[Synchronization effects in the pacemaker during high-frequency atrial stimulation].

The succession of intra-impulse intervals in venous sinus of isolated frog heart was studied during suprathreshold atrial stimulation at a frequency exceeding the initial pacemaker rate by l5-200%. Near to the initial and multiple frequency pacemaker synchronization takes place with the stimulator rhythm. With the stimulation of frequency increase atrial impulses begin to conduct retrogradely and regular ectopic rhythm is established. A unilateral pacemaker acceleration with the unilateral frequency drawing together is revealed in intermediate areas. Different effects appear to accompany it including asymptotic and cyclic synchronization proving in the form of arrhythmias.

[Methodological approach to the study of gynecologic diseases among children and adolescents].

Guidelines for the assessment of gynecologic morbidity in children and adolescents have been developed by the authors. A methodology of elucidation of gynecologic morbidity in children and adolescents was obtained and a classification of gynecologic diseases suggested. Efficiency of current therapy in childhood and adolescence as a determinant of reproductive health in womanhood is shown.

[Tall stature and its correction in girls].

The paper presents a classification of tall stature and clinical and hormonal findings in 35 girls in the age range of 11 to 18 years with constitutional tall stature. They showed a normal decline in growth hormone (GH) concentrations in the second phase of pubertal maturation. Concentrations of GH clearly correlated with the menstrual status. They were low in regularly menstruating girls, higher in non-menstruating girls and highest in tall girls with irregular menstrual cycles. Estrogen therapy in 7 girls decelerated growth rates and promoted epiphyseal ossification. Regular menstrual cycles established after withdrawal of therapy in all patients.

[The clinical picture, diagnosis and therapy of duplication of the uterus and vagina with partial aplasia of one vagina in adolescents].

Fifteen girls aged 12 to 18 years have been evaluated and operated for unilateral hematocolpos related to uterus didelphys with a partially aplastic vagina. It most commonly occurred at puberty and characteristically presented as the onset of algomenorrhea at a few months following menarche, with deterioration of it during every menstruation, and suppurative discharge refractory to any therapy. The most revealing diagnostic approach was the gynecologic examination supported by pelvic and renal ultrasound scans. Surgical approach to this malformation consists in dissection of the walls of the rudimentary vagina.

[Possibilities of surgical treatment of lesions of the organs of the reproductive system and disorders of sexual development].

The authors describe the data on clinico-laboratory and instrumental examinations and surgical treatment of about 5,000 children with different reproductive organ injuries including complex patterns of abnormal sexual differentiation. The designed diagnostic programs made it possible to distribute patients into groups and according to the disease entities in conformity with the classification suggested. It is emphasized that in the developing science concerned with sex, it is necessary to distinguish an area pertaining to childhood, namely pediatric andrology, and to delineate its subject matter and tasks. Skilled assistance can be rendered by a specialized center. A broad spectrum of improved masculinizing (including the design of an artificial phallus) and feminizing (including the design of an artificial vagina from the rectosigmoid segment of the colon) is provided. The catamnestic data are evaluated.

[Use of difenin in the treatment of constitutionally tall girls].

Thirteen constitutionally tall girls marked by certain retardation of sexual development, confirmed by clinical, instrumental and laboratory studies were examined. The girls' age ranged within 11 to 15 years. The examinations were performed before and during phenytoin treatment. The data obtained indicate that during phenytoin treatment, increment of the body length decreased to 1-3 cm over one year; skeletal maturation was found to be accelerated by 2-5 years. In 8 patients, menstruations occurred by themselves and in 5, the menstrual cycle returned to normal. The rise of the content of follicle-stimulating and somatotropic hormones as well as of estradiol together with a tendency towards cortisol content lowering were recorded. Based on the studies performed it may be ascertained that the use of phenytoin inhibits the growth rate, accelerates the closing of the zones of bone growth, promotes normalization of menstrual function, and expedites menarche.

[The role of environmental and genetic factors in the development of pathology of the reproductive system in girls].

Genetic and environmental effects on reproductive development of girls are described. Possible genetic involvement is postulated in girls with opsomenorrhea, second-degree amenorrhea and juvenile bleedings. Abnormal development of the menstrual function is more commonly associated with chronic tonsillitis, stress or weight loss. Environmental effects are particularly frequent and pronounced one year before or during the menarche. Central menstrual disorders were associated with parental reproductive disorders in 47% of the cases.
