Pathomorphological findings in thyroid cancers of children from the Republic of Belarus: a study of 86 cases occurring between 1986 ('post-Chernobyl') and 1991.

Recently, an impressive increase in malignant thyroid tumours has been observed among children less than 15 years of age living in the Republic of Belarus at the time of the nuclear accident of Chernobyl in 1986. More than half of these patients lived in the region of Gomel, nearest to Chernobyl. Because of the very short time interval between the accident and the tumour occurrence an independent review of the available histopathological material was done. Out of 101 cases diagnosed as thyroid cancers, we reviewed slides of 93 cases and agreed the diagnosis of malignancy in 92.5%. Of these tumours 96.5% were papillary carcinomas, 61.5% were moderately or poorly differentiated. Extrathyroidal extension was observed in 60.5%, regional lymph node metastases in 74% and distant metastases in 7%. One of the patients died from lung metastases. Our results confirm that the neoplasms increasingly diagnosed between 1986 and 1991 among children of this region are thyroid carcinomas. In addition, we correlate several histopathological findings with sex and age of the patients and other parameters, and compare the results with data from other studies.

[The plastic repair of extensive wound defects with vascularized flaps in treating tumors of the skin and soft tissues].

The study was concerned with comparison of the results of application of two procedures of plastic surgery using vascularized flaps for treatment of localized recurrent malignant tumors of the skin, soft tissues of the head, arms, legs and the trunk. According to one procedure (28 patients), large wound defects caused by extensive excision of tumor were treated using tissue flaps and microvascular anastomosis. In the other group of 10 patients, large defects were treated with skin-muscular flaps sitting on a fixed vascular-neural pedicle. But for these two procedures, some cases would not have been operated on at all due to considerable local extension of tumor.

[Vibration sensitivity in traumatic lesions of peripheral nerves].

Using a tonal audiometer, the parameters of vibrational sensitivity (in decibels) were investigated in 50 patients with traumatic damage to the peripheral nerves of the extremities. The results reflected the severity of neural damage and also the degree of the restoration of neural functions following surgical or conservative treatment.

[Pressure in intracranial saccular aneurysms in the presence of hypotension].

The pressure in saccular aneurysms of various localizations was studied in 6 patients during neurosurgical operations. The systolic, diastolic, mean, and pulse pressure in aneurysms with a wide neck corresponded to systemic pressure; in aneurysms with a narrow neck (3 patients) they were lower. In occlusion of the carotid artery on the neck, the pressure in the anurysms decreased. Pulsations disappeared in 3 patients. Fourteen patients with aneurysm of the internal carotid artery were followed-up for 2--17 years, after ligation of the carotid artery on the neck. There were no recurrent hemorrhages. Despite the clinical effect, however, due to possible complications this operation has limited indications (subclinoid aneurysms, refusal of patients to undergo intracranial operations).

[A comparative assessment of the results of plastic surgery on the facial nerve following total removal of neurinomas of the acoustic nerve].

The study was conducted in 3 patients following the application of intracranial facial nerve anastomoses, in 25 patients with the facial nerve anastomosed with the accessory nerve, and in 17 patients with facial-hypoglossal nerve anastomoses. Apart from the clinical observations of the facial muscle contractions, the comparative evaluation of the plastic operations was conducted with the aid of electromyography, electrogustometry and salivary glands secretion studies. The authors concluded that the intracranial anastomosis results in better restoration of the motor function of the facial muscles, the gustation being also partially restored on the side on the intervention. Among the anastomoses of the VII nerve with the XII and XI that with the XII is preferred.

[Various problems in autotransplantation after injuries to peripheral nerves of the extremities].

From clinical experience and the results of anatomical study, the authors suggest repairing defects in the large nerve trunks with segments of cutaneous nerves found within the boundaries of the main operative wound. As a result the commonly performed additional intervention on the healthy extremity for obtaining the graft is avoided. After the ends of the injured nerve are separated into bundles, the donor nerves are sutured to the groups of bundles so that the defect is repaired along the perimeter. Knowledge of the specific features of the fascicular structure of nerves allows the possibilities of microsurgery to be used more rationally neuroplasty.

[Nerve regeneration after defect replacement by fresh, degenerated, frozen and irradiated allotransplants (experimental study)].

The authors studied the results of allotransplantation of fresh, frozen, preliminarily degenerated and irradiated, and stored in a nutrient medium under deep-freeze conditions nerve segments. Satisfactory results were registered only in transplantation of irradiated transplants. Evidence of this was provided by morphologic and electrophysiologic data and by microangiograms of their arterial network.

[Pericranial sinus (sinus pericranial)].

The authors had under the their care 6 patients with sinus pericranii. Sinusography with puncture of the sac of the sinus and administration of a contrast medium was performed in all of them. In 4 patients the sinus pericranii was not connected with the extracranial vessels, in 2 retrograde filling of the intracranial venous sinuses and the veins of the face occurred. All the patients were operated on. The sac of the sinus was totally removed in 5 patients and opened and tamponed with muscles in one pateint. The etiology, differential diagnosis, and surgical management of the disease are discussed.
