Mentor, mentee, mentoring: building career development relationships.

An understanding of the process of mentoring can be useful for career development in the clinical, research and educational arenas of the nursing profession. The roles of mentor and mentee/protege as guide and learner are described. Several purposes for mentoring are discussed and five stages of the mentoring relationship are delineated. Potential problems, obstacles and factors which facilitate successful mentoring are explored. Through mentoring, individual, organizational and professional development can be fostered in cost-effective and fulfilling ways.

An unusual case of dermatofibrosarcoma protuberans.

A dermatofibrosarcoma protuberans involving the thumb of a nondominant hand is reported and represents the first case reported in this location. Dermatofibrosarcoma protuberans is a soft tissue neoplasm. It is a slow growing, locally aggressive fibrous tumor that occurs with equal frequency on the chest, abdomen, back, and proximal aspect of the extremities. Only two cases involving the hand are reported in the literature.

Acquired rotational digital deformity in children as a result of finger sucking.

Although most attention to the harmful effects of finger sucking by infants has been on the potential dental deformities, finger deformities also occur. The three cases presented in this article describe radial rotation of the index finger as a result of persistent finger sucking. In the majority of cases rotational deformities spontaneously resolve once finger sucking ceases. In a few cases, particularly when the habit is unduly prolonged, deformities may persist and cause functional impairment. In these patients surgical intervention is indicated. In our cases the rotational digital deformities were treated successfully by rotational osteotomy of either the metacarpal or the proximal phalanx.

Locking of the metacarpophalangeal joint.

Intrinsic finger metacarpophalangeal (MP) joint locking is infrequent, although not rare. Our experience and the recent literature suggest that metacarpophalangeal locking is being recognized with increasing frequency. The various mechanisms responsible for intrinsic finger MP locking, as well as the diagnosis and treatment of this condition, are reviewed. Four additional cases of the condition are presented.

Periosteal chondroma with atypical behavior in a 9-year-old black female: case report.

Periosteal chondroma is an uncommon benign cartilaginous tumor of bone reported most often in children and young adults but is as of yet unreported in a black child. This is a case of periosteal chondroma in a 9-year-old black female with unusual findings of short duration of symptoms; large size; suspicious x-ray appearance and; aggressive behavior. The lesion was histologically benign and has not recurred six years post curettage.

An unusual etiology of trigger finger: a case report.

A case of triggering of the right long finger presented in a patient with obvious recurrence of a cavernous hemangioma (unrelated to the triggering). A rare cause of triggering, a giant cell tumor of tendon sheath in the carpal canal, was found at operation.

Sex-role attributes, gender, and postpartal perceptions of the marital relationship.

The relationships of sex-role attributes and gender to perceptions of changes in intimacy and in the overall marital relationship were examined, as was the quality of that relationship following the birth of an infant. For a sample of 165 couples, positive changes in intimacy and in the overall relationship following delivery were positively related to marital quality at four months postpartum. Of gender and sex-role attributes, only femininity contributed significantly to the prediction of overall change and quality. None of the variables predicted perceived change in intimacy.

Management of flexor tendon lacerations in the hand.

Flexor tendon injuries in the hand present a real challenge in treatment. Poorly chosen or poorly executed treatment may lead to a functionless finger at best, impairing total hand function. A variety of approaches to treatment of flexor tendon injuries is available to the specially trained surgeon and a particular method is selected based on the merits of the case in question. Primary tendon repair is a viable choice under the proper conditions, while delayed primary repair allows a somewhat broader application of this technique. Flexor tendon grafting remains a very useful operation for the surgeon caring for tendon injuries of the hand.

Stress fractures.

Stress fractures, well-known occurrences in military recruits, also occur in civilians. There is a gradual alteration of the bone resulting from repeated unaccustomed stresses. Pain usually precedes positive x-rays by one to two weeks. Serial films are therefore required. Differential diagnosis can be a problem but treatment is simple. Femoral neck stress fractures can become complicated.

Changes in sexual relationships in postpartum couples.

To assess perceived changes in intimacy and sexuality in postpartum couples, an Intimate Relationship Scale was developed, tested for reliability (alpha = 0.86), and administered by mail. Sixty-eight, middle-class couples responded at four months postpartum and 126 couples at 12 months postpartum. Data revealed declines in the frequency of and desire for sexual activity, especially for mothers, when compared to sexual activity before the infant's birth. At four months postpartum, most women reported physical discomfort with sexual intercourse, a decline in physical strength, dissatisfaction with bodily appearance, and fatigue which interfered with sex. Strategies and techniques to promote and maintain intimacy and sexuality in postpartum couples are suggested to assist their transition to parenthood and may positively influence parenting behaviors.
