Long-term results of cemented and screwed acetabular components.

The authors report their experience in the use of various types of acetabular components (cemented and screwed) in 253 hip prostheses with "Biodirect" femoral component and ceramic femoral head. A long-term follow-up of 148 hip prostheses (58.5% of the total) was carried out, 36 Müller type polyethylene and 61 Biolox ceramic acetabular components, and 51 screwed sockets of various models were used. From a clinical and radiographical standpoint, the ceramic acetabular component proved the most valid in the short and medium term, confirming the favourable biomechanical conditions which derive from ceramic/ceramic coupling.

Total hip replacement using the cementless "Bio-direct" femoral component and the "CAB 1" self-threading acetabular component.

The special characteristics of the "Bio-direct" femoral component, which has been used in total hip replacement for approximately 7 years, are described. Although the current trend is to pair a cemented acetabular component with the Bio-direct femoral component, a new self-threading acetabulum (CAB 1) has recently been implanted in 12 patients. This new acetabular component is fully illustrated.

Total hip replacement with the "Biodirect" cementless prosthesis.

The authors discuss the biomechanical features of the "Biodirect" cementless prosthesis, which has been used for over 6 years in the treatment of displaced femoral neck fractures. The long-term clinical and radiographic results of this study were generally favorable and seemed directly proportional to the accuracy of the implantation technique.

A new self-threading acetabulum (CAB 1) in total hip replacement.

After several disappointing experiences related to the positioning of self-threading acetabulae, a new model is presented. This consists of a Titanium conical external ring and a conical insert in Biolox bioceramic. The design of the threading produces considerable stability of implantation.

Fracture of the shaft of the femur at the apex of the stem of a femoral prosthesis.

Five cases are described of fracture of the femoral shaft at the apex of the prosthetic stem of a femoral prosthesis. Four were treated by compression fixation using a specially modified plate. In the fifth case the prosthesis was loose so it was replaced using a femoral component with an elongated stem to fix the fracture. The author suggests a hypothesis to explain such fractures, which are by no means a rare complication of arthroprosthesis of the hip.
