Combined approach (stereotactic-microsurgical) to a paraventricular arteriovenous malformation. Case report.

The authors present one case of a 42 years old male patient who had a small right paraventricular arteriovenous malformation. At operation the patient was placed in the Talairach's stereotactic frame and, after performing a right carotid angiogram to locate the lesion exactly, a flexible cannula was inserted stereotactically, through a previous trephine hole, until the tip reached the AVM. Using a microsurgical technique and through a 1-2 cm corticotomy the cannula pathway was followed easily reaching the AVM. Total removal of the AVM was confirmed by a carotid angiogram, the patient was discharged without symptoms 7 days later. The authors propose that due to the special design of the Talairach's stereotactic frame, its application as a locating system for small and deep AVM's would greatly facilitate their removal using microsurgical techniques.

Accidental stab wound of the cervical spinal cord from in front.

An unusual case of damage to the cervical spinal cord due to a non-missile penetrating injury through an anterior route in a nine-year-old boy who fell from a bicycle is reported. There was a cerebrospinal fluid leak that lasted for two days. Clinical picture was that of incomplete Brown-Séquard syndrome and flaccid paresis of the extensor muscles of the left elbow and wrist. Treatment was conservative, with a very good functional recovery. To our best knowledge there is no myelographically documented case report of such a lesion in the literature.

Hematomas of the pons.

Two cases of hematomas located in the latral portion of the pons that were erroneously diagnosed preoperatively as cerebellopontine angle tumors are presented. In both cases, the hematomas were evacuated successfully. Lateropontine hematomas must be included in the differential diagnosis of the cerebellopontine angle syndrome.

[Urologic aspects of Hippel-Lindau disease].

We report on a patient with von Hippel-Lindau disease with special reference to the urological features. Several members of the patient's family had also presented this hereditary disease. Careful follow-up of these patients permits early diagnosis of some of the characteristic tumors. Because it has a tendency to be multifocal and bilateral, surgical treatment of renal carcinoma must be as conservative as possible. For this reason, early tumor excision constitutes the ideal treatment. The importance of family screening and genetic counselling is underscored.
