Acute rheumatic fever in the young: changing prevalence and pattern.

Acute rheumatic fever (ARF) is rare in young people. A group of 28 patients is described who were hospitalized during the years 1960-1984. Patients were divided into two groups: children who were admitted between 1960 and 1966 and those admitted between 1966 and 1984. The annual incidence of ARF declined from 1.17 to 0.44 cases per 10,000 population at risk. The incidence of arthritis declined in parallel to the decrease in number of children presenting with ARF, whereas the incidence of carditis remained the same.

Peripheral facial nerve paralysis in children: the value of routine diagnostic work-up studies.

Acute facial nerve paralysis is a relatively common pediatric disorder. Idiopathic facial nerve palsy (Bell's palsy) has to be distinguished from other etiologies. Retrospective review of the results of routine diagnostic work-up studies of our patients, failed to reveal any clinically significant abnormalities and did not provide more data as far as etiology or indication for management. Significant history and positive findings on physical examination should direct the physician towards specific diagnostic procedures.

Skin type, hair color, and freckles are predictors of decreased minimal erythema ultraviolet radiation dose.

In a group of 190 white healthy subjects the skin type classification method was found valuable for differentiating subgroups with various degrees of sun sensitivity (except for 33% with borderline or unclassifiable skin type). Sun-sensitive skin types I and II were significantly more common among persons with light hair color or freckles, or both (p less than 0.001). In each skin type category the proportion of subjects with a minimal erythema dose (MED) lower than the median MED of the entire group (%LMED) decreased significantly with increasing skin type number, and distinguished between skin types I through III better than did their mean MED values. Independent predictors of %LMED were skin type and hair color. The contribution of freckles to %LMED was skin type dependent. Age, sex, or eye color had no independent effect on %LMED. The association of skin types I and II, red or blond hair, and freckles with decreased MED may reflect genetically controlled predominance of pheomelanin (a photosensitizing molecule) in the skin of subjects with these phenotypes.

Phytobezoar following raisin ingestion in children.

A rare childhood phytobezoar is reported in a two-and-one-half-year-old male infant with bowel obstruction, following raisin ingestion. Complete recovery followed conservative treatment. It was preceded by passage of a large, bulky stool, containing numerous undigested, fluid-filled raisins.

Perturbation of epidermal Langerhans cells in basal cell carcinomas.

Epidermal Langerhans cells (LC) play a pivotal role in a variety of local immune responses. Using ATPase-stained preparations, we evaluated the density and morphologic features of these cells in the skin of 12 patients with basal cell carcinoma (BCC). In three patients, we compared the density of LC in the tumor to that in the perilesional skin. In these cases, there was no difference in the numbers of LC/mm2 in the two sites. Perturbations of ATPase-positive, dendritic LC were evident in all specimens. These perturbations included various degrees of disruption of the usually uniform LC network and alterations in the morphology of LC. Many LC had rounded, deformed cell bodies, dendrites that were shortened or completely absent, and variations in the intensity of ATPase staining. With the use of an image analyzer, a measure of the deviation from circularity--a "dendricity index" (DI)--was obtained, based on the area and perimeter of the LC. A significantly lower DI, implying a predominance of less dendritic, more rounded LC, was observed in LC at the BCC surface than in LC present in perilesional skin. This finding raises the possibility that BCC may arise in areas where the morphology, or activity, of these immune cells has been altered; alternatively, the perturbations could reflect an effect of the tumor cells on LC morphology.

Cardiac injuries caused by blunt chest trauma in children.

Two illustrative cases with different features of cardiac injury caused by blunt chest trauma are described. The first patient had mild and obscure symptoms, detected on physical examination, and required observation only. The second patient had acute pericardial tamponade, necessitating surgical treatment. We present the different medical procedures that should be taken into consideration in management of such cases, although continuous monitoring, repeated physical examination, electrocardiograms, chest x-rays, and echocardiography proved sufficient in managing our two children. It is important that physicians who provide care to children suffering from blunt chest trauma have increased awareness of possible cardiac injuries.

A more reliable index of sunscreen protection, based on life table analysis of individual sun protection factors.

The efficacy indices of two representative sunscreen preparations were determined using life table analysis of individual sun protection factors (SPF), obtained by exposing a group of subjects to ultraviolet radiation (UVR) doses from 4 to 20 multiples of the control minimal erythema dose (MED). The ultimate efficacy index is expressed as the UVR dose under which a selected percentage of the population is safely protected by a sunscreen. This method takes into account several sources of bias in the current test procedure, in particular the considerable variability in individual responses. It also takes into account subjects in whom the MED of the protected skin is either higher or lower than the range of challenge doses to which they were exposed, thereby increasing the effective sample size. This method provides a more precise and comprehensive index of sunscreen protection, and enables statistical comparison of different preparations, or of the effect of other factors on their efficacy.

Familial occurrence of typhoid acute glomerulonephritis.

Three cases of typhoid nephritis occurring in a single family are reported. A clinical similarity to poststreptococcal nephritis was noted, although no hypocomplementemia was observed. The same phage type occurred in other patients who did not exhibit nephritis. The possibility of a host factor was therefore considered to be involved, although no HLA haploid-type link was found in the patients. These cases illustrate that in certain geographical areas the possibility of typhoid fever should be considered in patients with nephritis and concomitant fever.

Recurrent abdominal and cervical pains. An unusual clinical presentation of acute rheumatic fever.

Most cases of acute rheumatic fever (ARF) present with arthritis, carditis or choreiform movements. However, a variety of clinical manifestations which are not included in the modified Jones criteria can be the presenting symptoms of the disease. We describe a case of a 10-year-old boy with ARF who presented with recurrent episodes of abdominal and cervical pain who later developed an active carditis which established the diagnosis of ARF. A high degree of suspicion and an awareness of the less common manifestations of ARF are necessary to make an early diagnosis and initiate appropriate treatment in certain cases of ARF.

Sandifer's syndrome--a rare complication of hiatal hernia. A case report.

A 13-year-old boy with Sandifer's syndrome whose only symptoms were abnormal posture and movements of the head and neck, underwent a repair of a hiatus hernia. The disappearance of all symptoms following the operation could be explained by the common innervation of the diaphragm and the neck.

Topical treatment of periocular basal cell epithelioma with solcoderm.

25 patients with 29 histologically confirmed periocular basal cell epitheliomas were treated by topical application of Solcoderm, a solution of organic and inorganic acids and ions. The cure rate of the 29 lesions, which were followed for a mean period of 32 months, was 80%. The overall cure rate following second Solcoderm treatment of the 6 recurrent lesions was 96.7%. The cosmetic and functional results were excellent and there were no serious complications and side effects.

Preoperative ultrasonic measurements of the thickness of cutaneous malignant melanoma.

Preoperative measurements of the thicknesses of pigmented skin lesions by high-resolution sonography have been studied. Fourteen nevi and 15 malignant melanomas were examined and their thicknesses compared with those measured on both paraffin and frozen sections. Linear regression coefficients of r2 = 9.22 and r2 = 9.23, respectively, were found. This good correlation suggests that sonography may be of value as a guide in predicting prognosis and in determining the proper course of treatment for patients with malignant melanoma.

Efficacy of topical sunscreen preparations on the human skin: combined indoor-outdoor study.

The sun protection factors (SPF) of 35 commercially available sunscreen preparations were tested on human skin under various conditions. Five distinct groups of products, ranging from minimal (SPF 2 to less than 4) to ultra sun protection (SPF 15 to greater than 19) were evaluated. Claimed SPF values were not always consistent with those obtained in our tests. Substantivity studies of a randomly selected group of sunscreen preparations during the hot, highly humid Israeli summer showed mean SPF reductions of 47 and 60%, respectively, following exercise-induced sweating or a 20-min swim. Judicious selection and use of an appropriate sunscreen preparation according to individual skin type or life-style, as well as the introduction of strict testing and labeling regulations, are essential measures for reducing the risk of sun-induced skin cancer and malignant melanoma in Israel.
