Let go of Latin!

Health care practitioners have long used Latin abbreviations. Using these shorthand terms does not consistently promote patient safety. although in some instances they offer a time-saving convenience. This article illustrates hazards associated with the common Latin abbreviation "Qd" by showing copies of actual medication orders. Other dangerous abbreviations are also discussed. The use of plain English is suggested as the prescribing practice most consistent with professional values.

Finding an optimal dose: considerations in accurate opioid dispensing.

Morphine sulfate is widely prescribed for the relief of moderate to severe acute and chronic pain. Unfortunately, medication errors associated with morphine sulfate can result in patient harm; profound respiratory depression can result from excessive doses in opioid-naïve patients. An actual medication order containing a potentially toxic overdose of morphine sulfate is examined. Patients habituated to high doses of morphine sulfate are at risk from underdosing, a hazard also of concern.

Unclear dose equals toxic dose.

Medication errors are a significant public health issue. Their frequency and severity are currently under increased scrutiny from many perspectives. In 1996, the National Coordinating Council for Medication Error Reporting and Prevention (NCCMERP) was established to promote the reporting, understanding, and prevention of medication errors. NCCMERP has published prescription writing recommendations to remedy error-prone aspects of prescription writing. An example of a hazardously written medication order and its possible toxicity is provided.

Nontraditional Pharm.D. program at the University of Arkansas for Medical Sciences.

The nontraditional Pharm.D. program at the University of Arkansas for Medical Sciences is described. The nontraditional program, started in 1990, includes 21 credit hours of didactic work and 12 of clerkships. In the self-paced didactic courses, videotapes, manuals, and texts are used, along with faculty consultation in person or by telephone. Clerkships can be traditional or nontraditional. Nontraditional clerkships can be completed by self-study in the student's community after an orientation period at the preceptor's site. More than 400 pharmacists have enrolled in the program and more than 80 have entered nontraditional clerkships. Thirty-nine pharmacists have received the Pharm.D. degree. The program offers an accessible alternative for pharmacists who want to earn the Pharm.D. degree while maintaining full-time employment.

A cloned T cell line that selectively augments antibody responses of phosphorylcholine-specific B cells bearing the T15 idiotype.

Helper T (Th) cells have shown to be heterogeneous both in their function and in their specificity. Several studies have demonstrated that there exist Th cells which do not appear to be specific for antigen:Ia complexes (not H-2 restricted) but interact with the target B cell via recognition of immunoglobulin idiotypic determinants. In analyzing anti-phosphorylcholine (PC) responses, such idiotype-specific Th cells are characterized by 1) their ability to increase responses to phosphorylcholine by augmenting selectively the activation of T15-idiotype-bearing PC-specific B cells, 2) their ability to bind specifically to T15-bearing immunoglobulin and 3) the dependence of their maturation on circulating T15 idiotype rather than Ia. Our aim is to explore further the relationship between Ia-specific and T15-specific Th cells. These studies describe the first step in comparing specificity and function of idiotype-specific Th cells to other known Th cells by the cloning of a T15-specific Lyl T cell (ThId) with properties consistent with those described for uncloned populations. The cloned ThId cells generated do not activate B cells to secrete antibody but augment anti-PC plaque forming cell (PFC) responses induced by Ia-restricted Th cells. This augmentation is seen only in the T15-bearing PFC responses even though non-T15-bearing B cells are equally accessible, and it is seen only in PC responses and not responses to TNP under identical culture conditions.(ABSTRACT TRUNCATED AT 250 WORDS)
