[The treatment of patients with benign prostatic hyperplasia using the selective alpha-blocker alfuzosin].

40 patients with benign prostatic hyperplasia (BPH) were treated with the alpha-blocker alfuzosin which was administered per os twice a day in a dose 5 mg. The treatment brought about a decrease of the mean symptom score from 19.6 to 12.2, of quality of life from 5.2 to 2.1, of residual urine from 152 to 82 ml. Mean and maximal urinary flow rates rose by 35 and 45%, respectively. The level of prostate-specific antigen over the treatment course did not change. The findings of the trial evidence for high efficacy of alfuzosin in the treatment of urination disorders in BPH patients.

Adrenergic alpha-Antagonists

[The use of serpens for treating patients with benign prostatic hyperplasia].

320 mg of serpense was given daily to 24 patients with benign prostatic hyperplasia (BPH) for 2 months. Control check-ups were performed each 30 days. The effect was judged by the patient's condition (I-PSS), quality of life, residual urine, the gland size, urodynamic picture and level of prostatic specific antigen (PSA). The response of BPH to serpense was noticeable, especially at the first stage of the disease (66.7%). Reduced serum PSA in serpense-treated patients may be explained by peripheral selective inhibition of 5 alpha-reductase or, rather, by anti-inflammatory action of the drug. Antidropsical and antiinflammatory actions of serpense suggest its possible efficacy in chronic prostatitis.

[Left-side varicocele and infertility: diagnosis and treatment].

Eighty-four patients with left varicocele were subjected to complex examination to identify the causes of disturbed spermatogenesis. It was found that not only the testis but the adrenal also is involved in the pathological process in varicocele caused by venous renal hypertension. Changes of venous hemodynamics in the left adrenal lead to activation of steroidogenesis in its cortex and, consequently, to impairment of spermatogenesis in both testes. As the result of the study a new method for the treatment of sterility in patients with left varicocele was suggested, namely, X-ray-guided endovascular occlusion of the central vein of the left adrenal. Tentative evaluation of the results of the intervention gives hope of its efficacy.

Adolescent

[Diagnosis and roentgeno-endovascular treatment of patients with renin-dependent arterial hypertension and secondary aldosteronism without damage to the main renal arteries].

Bilateral electrocoagulation of the central veins of the adrenals (ECVA) was performed in 13 patients with stable and malignant arterial hypertension (AH). The renin-dependent character of AH was supported by positive reaction of arterial pressure (AP) to the test dose of captopril (25 mg). The vasorenal origin of the impairment was excluded on the basis of the peripheral captopril test findings, pharmacorenography with captopril. Bilateral ECVA was done during angiography. The manipulation turned out technically successful in 11 left (85%) and 9 right (70%) adrenals. The AP began lowering from the first days of the intervention and got stabilized by day 4 to 5. There was a significant decline of AP within the observation period up to 1 year and a reduction of the elevated aldosterone content and plasma renin activity with the content of ACTH being unchanged. After the manipulation 2 patients could fully discontinue the intake of hypotensive drugs. In 11 patients, the dose of the drugs could be reduced.

The role of impairment of adrenal mineraloglucocorticoid function in the development of infertility in varicocele patients.

The study was aimed at the understanding of pathophysiological mechanisms of the impairment of spermatogenesis in varicocele patients. The crucial role of tension increase in the venous plexus of the spermatic cord in spermatogenesis damage in the testis on the varicocele side and absence of any effect of haemodynamic abnormalities on spermatogenesis in the complementary testis has been determined. Retrograde blood flow through the central vein of the left adrenal gland in varicocele has been evidenced by X-ray examination. The role of this phenomenon in the changes of functional activity of the adrenal gland is discussed. Results of the study of functional status of adrenal glands revealed their tendency to provide hyperactivity in synthesis of mineraloglucocorticoids. Correlation between cortisol level in peripheral blood and percentage of abnormal sperm in ejaculate was shown. This fact supported the idea about the existence of a causal interrelationship between abnormalities in the functional status of adrenal glands and development of infertility in varicocele patients.

Method of choice for urinary diversion in surgical treatment of bladder carcinoma.

Evidence is presented concerning 94 patients with malignancy of bladder in whom diversion of urine was performed after cystectomy for exclusion of the bladder. There were 80 men and 14 women, ages ranging from 30 to 81 years. In 35 of these 94 cases ureterosigmoidoanastomosis, in 38 cases ureterocutaneostomy, in 15 Portilla's operation, in 2 Bricker's method and in 4 nephro-pyelostomy were performed. For the prevention of bleeding cystectomy was preceded by bilateral ligation of the internal iliac artery in 14 cases. Continuous postoperative follow-up of the patients (from a few months to 14 years) and analysis of the early and late results allow to regard the combined technique of Coffey II-Nesbit-Goodwin as the method of choice having the slightest risk of peritonitis, intestinoureteral reflux and other complications inherent in other procedures. Of the existing methods of ureterocutaneostomy the authors have chosen Le Dentu's method with some modifications. They consider ureterostomy to be the simplest and quickest method owing to the higher location of ureterostoma. Portilla's method is no longer used because of its unfavourable long-term results. Of 94 patients with cancer of the bladder, who had been subjected to different methods of urinary diversion in the years 1954-1974, 16 have survived up till now. There are 10 survivors after ureterosigmoidoanastomosis, 5 after ureterocutaneostomy and 1 after Bricker's operation.
