Subacute necrotizing lymphadenitis --a clinicopathologic study.

This report describes the clinical and pathologic findings of 21 patients with subacute necrotizing lymphadenitis (N.L.). Clinical features of all but 2 patients were as typically described in the literature; young age, enlargement of cervical or axillary lymph nodes, slight to moderate fever with poor response to antibiotics, and invariable spontaneous resolution. Two patients showed atypical clinical findings; remittent high fever with profound prostration in both patients, generalized lymphadenopathy in one, and being a Caucasian in the other. Systemic symptoms resolved gradually, and both patients are now doing well. One patient, however, underwent a staging laparotomy due to a misdiagnosis as unclassifiable lymphoma of the lymph node biopsy. Biopsies of the lymph nodes from all patients demonstrated the typical pathologic features of N.L.; i.e., an intensive proliferation of leukocytes were characteristically absent. Bacteriologic and serological studies were negative. We conclude that some patients with N.L. run a protracted course with remittent high fever before spontaneous resolution and that a correct pathologic diagnosis helps precluding unnecessary diagnostic or therapeutic measures on such patients.U

Malignant lymphoma of the testis: a clinicopathologic study of 37 cases.

The clinicopathologic findings in 37 cases of malignant lymphoma with primary clinical manifestation in the testis were analyzed. Of these cases 34 were classified as histiocytic lymphoma and 3 as lymphocytic lymphoma. Patients usually presented with a painless small testicular mass of short duration. The peak incidence occurred in the fifth through eighth decades. Metachronous testicular involvement was noted in 35 per cent of the cases and simultaneous bilateral involvement in 3 per cent. In 25 per cent of the cases the first manifestation of lymphoma after orchiectomy was in the opposite testis. Regional and distant nodes were equally common sites of lymphoma after orchiectomy. The long-term survival of a few patients after orchiectomy may suggest that some malignant lymphomas are truly primary in the testis. However, the short interval free of disease and the rapid death of most patients imply that most testicular lymphomas are a manifestation of multicentric origin of systemic malignant lymphoma. The over-all 2 and 5-year survival rates were 30 and 20 per cent, respectively. Prospective studies are needed to define the optimal modality of treatment.

Primary hepatic leiomyosarcoma: A report of two cases.

Two cases of primary hepatic leiomyosarcoma are reported. Review of the literature reveals only seven previous reports. Cases presented predominantly with either hepatomegaly or ascites. Neither history nor physical and laboratory examination distinguished these patients with primary leiomyosarcomas from those with other hepatic tumors. Analysis of the cases reviewed suggests that aggressive hepatic surgery may improve prognosis.

Hereditary hemorrhagic telangiectasia. A case with hepatocellular carcinoma and acquired hepatocerebral degeneration.

A patient with hereditary hemorrhagic telangiectasia (Osler-Weber-Rendu disease), treated with ethinyl estradiol, multiple blood transfusions, and iron-dextran, developed hepatocellular carcinoma and acquired hepatocerebral degeneration. In addition to the carcinoma, the liver contained extensive arteriovenous maliformations, telangiectasis, and changes of Osler atypical cirrhosis. The carcinoma possibly had its genesis in the presence of an ocongenic serum hepatitis virus, or the cirrhosis, or both.
