Injuries of ureteral lower segment and the bladder during surgery.

The injuries of the lower ureteral segment and the bladder are relatively frequent during gynaecological and surgical interventions in the small pelvis. The cases of 25 ureteral injuries observed in the period between 1976 and 1985 are now evaluated. The majority of them are complications arising due to gynaecological operations. Attention is drawn to the rise in the number of ureteral lesions connected with the progress of colic and vascular surgery. The aim of reconstructive surgical solutions is to restitute function and to attempt to find the organ-saving possibilities. Bladder injuries occur most often during transurethral operations (TUR). The nature of artifactual injuries associated with TUR and the ways they are inflicted were analysed on the basis of a study of about 1000 cases.

[Surgical treatment of coral calculi in the renal pelvis].

In the years between 1971 and 1979 in the Department of Urological Surgery of the Municipal Weil Emil Hospital in 16 of the 72 coral calculi filling the renal pelvis primary (so-called uric acid) calculi were found to which lytholysis was applied. Fifty-six cases (secondary calculi) were operated, in the majority by conservative surgery (40 cases), 16 patients were nephrectomized. The incidence of coral stones is twice as high in women as in men. Congestion and infections play the major role in their pathogenesis, hypercalciuria is less frequent. Up-to-date possibilities of examination ensure the proper plan of the operation. Complete removal of the stones, abolishment of congestion and infection and postoperative care are very important in the prevention of relapse. In 21 of the 72 cases congenital malformation as was found as a source of congestion. There are various types of conservative operations. Quite often the renal parenchyma is incised, and in this case the peduncle has to be ligated. By cooling the kidney the hypoxic time can be prolonged quite considerably. In the course of the operation the kidney must be made free of stones as the incidence of relapses is high. Analysis of the removed coral stones showed that 68% of them consisted of carbonate-apatite-struvite.

[The changed clinical picture of urogenital tuberculosis].

Of the 4500 urological patients treated in Weil Emil Hospital in Budapest, in 5 years 140 (3.1%) cases of urogenital tuberculosis were detected. In these cases earlier examinations failed to reveal the process which manifested itself one to six months. The number of those needing surgical intervention decreased considerably, 34 (24.2%) of the patients were operated. The clinical pattern has changed in our days and the possibility of an urogenital tuberculosis must be considered whenever no explanation can be found for the complaints and when even a single one of the characteristic symptoms of urogenital tuberculosis is observed.

Immune-indian ink method for detection of hepatitis A associated antigen and antibody.

TIndian-ink grains coated with commercial gamma globulin (immune-Indian-ink) were agglutinated by 3 percent of sera from healthy volunteer blood donors; by 4 percent of those from hospital staff in contact with patients suffering from hepatitis; and by 10 percent of those from patients with viral diseases other than hepatitis, In contrast, the rate of positive reactions was 86 percent in the case of sera taken from patients in the acute phase of an illness diagnosed as hepatitis A on the basis of epidemiological and clinical data. Investigation of serum samples taken serially from patients positive in the acute phase of illness revealed that the immune-Indian-ink agglutinating factor does not persist for long in majority of cases. Two months after discharge from the hospital it was present in 18 percent of the patients only. The reaction proved negative when a limited number of cases diagnosed as hepatitis B were investigated. The immune-Indian-ink agglutinating factor was inhibited by all but one of 36 sera taken in the convalescent phase from patients with a diagnosis of hepatitis A. Some sera displaying agglutination with immune-Indian-ink gave a reaction with uncoated Indian-ink, too. Efforts to free the sera from non-specific agglutinating factor by starch-block electrophoresis have led to partial success. Fractionation on Sephadex G-200 columns suggested that in molecular weight (or particle size) the immune-Indian-ink agglutinating factor is smaller than HBsAg and larger than the non-specific agglutinating factor. On the basis of these results it is assumed that the immune-tindian-ink reaction is suitable for detecting an antigen tentatively called IH chi Ag and its antibody (IH chi Ab) specific to hepatitis A.

The effect of unilateral papillectomy on the juxtaglomerular granulated cells.

The changes of the juxtaglomerular index values in the superficial, deep and whole cortical areas of rat kidneys after unilateral papillectomy were analyzed. On the papillectomized side the JGI values showed an increase in the deep, and a decrease in the superficial cortical zone. The changes proved to be significant during the whole experimental period in the juxtamedullary index and between the 4--14 postoperative days in the superficial index values. The tendency of the changes of JGI in the contralateral kidneys was similar to those of the papillectomized kidneys, but significant differences were registered less constantly. These changes are probably the consequences of haemodynamic factors. Papillectomy is considered an incompetent model for studying the regulatory function of macula densa in the synthesis of renin.
