[Chronic idiopathic gastric volvulus in children].

OBJECTIVE: We report clinical, radiograpic and instrumental data about 18 cases of gastric chronic idiopathic volvulus observed since 1997 to june 2000. METHOD: All cases diagnosed by barium meal, performed for vomiting or after a pHmetric esophageal monitoring, in cases of atipical symptoms of gastroesophageal reflux disease. RESULTS: All patients underwent a conservative treatment (antireflux position and procinectics): 5 recovered after 1 year of treatment and 13 were operated on because they did not improve during medical therapy or relapsed after the end of conservative treatment. CONCLUSION: Abnormal position of the stomach is not an absolute indication for surgical treatment. When medical treatment does not obtain improvement in symptoms, surgical option has to be discussed.

A case of Langerhans histiocytosis with HIV-like immunodeficiency.

A case of histiocytosis X (Langerhans type) associated with bullous pulmonary emphysema and acquired immune deficiency, regarding CD4 positive cells, is described. Previous history was remarkable for skin lesions which first appeared in 1981 and progressively worsened, diabetes insipidus diagnosed in 1986, and bullous pulmonary emphysema detected in 1988. Biopsy results of skin lesions were consistent with histiocytosis X. Thyroid gland involvement was found by means of cytological examination. The search for HIV infection (also performed by means of immunoblotting and PCR) was negative. To our knowledge the immunodeficiency detected in histiocytosis X affects the T suppressor lymphocyte subset, so we thought this peculiar case was worth describing.

[Intravenous leiomyomatosis of the uterus. Report of a case].

A clinicopathological study of a case of intravascular leiomyomatosis was performed, recalling the rarity of this pathology and the necessity of a careful follow-up. Microscopically the intravascular tumor resembles the benign leiomyoma. Authors have taken into consideration the most likely differential diagnoses: endolymphatic stromal myosis and low-grade leiomyosarcoma, they study the problem of the so-called "benign metastasizing leiomyoma" and emphasize that intravenous leiomyomatosis is frequently underdiagnosed.

[A case of desmoplastic malignant melanoma].

Desmoplastic malignant melanoma: report of a case. Authors report a case of desmoplastic malignant melanoma, a variant of the common primary cutaneous malignant melanoma in a 42 years old man, who was admitted into Hospital for metastatic node disease. Differential diagnoses on the primary, eventually discovered on the right knee, are discussed.

Hippocampal infolding angle changes during brain development assessed by prenatal MR imaging.

BACKGROUND AND PURPOSE: Epileptic syndromes or neurodevelopmental delay may be associated with congenital anomalies of the shape or the orientation of the hippocampus. Scarce data are available about quantitative hippocampal developmental changes during fetal life, in particular about the progressive rotational changes of the hippocampal infolding angle (HIA), which can be considered a hallmark of hippocampal development. We hypothesized that prenatal MR imaging could demonstrate the progressive rotation of the hippocampus, providing quantitative data by means of the HIA determination. METHODS: We retrospectively selected 62 fetal MR imaging cases with normal brain at prenatal and postnatal imaging. The gestational age ranged from 20 to 37 weeks. The coronal section encompassing the pons was used to perform the measurement of HIA. HIA was defined as the angle between the line connecting the lateral margin of the cornu ammonis with the medial superior margin of the subiculum and the line passing through the midline structures. RESULTS: A significant positive correlation was found between the HIA value and the gestational age. The HIA was generally below 70 degrees before the gestational week 25 and above 70 degrees after week 30. CONCLUSION: Prenatal MR imaging allowed the progressive rotation of hippocampus to be detected during fetal life, providing normative data about HIA changes. These data could support further investigations to assess how fetal HIA anomalies might affect postnatal neurologic outcome.

[Multicenter study of serum ferritin assay for the surveillance of subjects at risk of iron deficiency].

A multicentre study was carried out in a sample population of healthy volunteers in order to assess the usefulness of assaying serum ferritin to monitor the extent of reserves in subjects with a risk of iron deficiency. A total of 317 subjects were included in the study. Ninety-nine were children with a mean age of 19 months, 121 were adolescents with a mean age of 18 years and 7 months, and 97 were women with a mean age of 28 years and 9 months. Levels of serum ferritin below the normal minimum levels for each age bracket, an indication of the exhaustion of the body's reserves, were found in 29% of children, 32.2% of adolescents and 27.8% of women. This study therefore confirm the frequent onset, even in our modern society, of iron-deficient states at particular times of life and that these are easily overlooked. The measurements of serum ferritin levels, in addition to being a confirmatory test in cases of suspected sideropenic anemia, has the peculiar characteristic of being the only test able to identify risk subjects before they become symptomatic. This enables rapid treatment to be commenced or better, efficient prevention.

[Benign intraparotid schwannoma].

Benign schwannoma in the parotid gland. Authors report a case of a benign schwannoma of the parotid gland, whose correct diagnosis had been suggested after the FNAB. They check the literature for similar cases since the sixties and discuss the differential diagnoses in the cytologic specimens obtained by FNAB.

[Intravenous leiomyomatosis. Presentation of 5 cases and a review of the literature].

Authors report five cases of intravenous leiomyomatosis, one of which had colonized the inferior vena cava and the right atrium, and make a review of the literature. The morphology of the lesions is in keeping with that previously reported. Moreover the immunostainings confirm the nature and the intravascular location of the neoplastic tissue. Authors taking into consideration the two most likely differential diagnoses afford the problem of the so-called "metastasizing leiomyoma". Eventually they agree that intravenous leiomyomatosis is an underdiagnosed entity that possibly will be more easily recognized due to the modern non-invasive diagnostic procedures.
