[Course and diagnosis of pulmonary Kaposi sarcoma in patients with AIDS].

Diagnosis of pulmonary Kaposi's sarcoma can be difficult. Clinical findings and chest X-rays are non-specific and endobronchial diagnostic in about 50% of cases. Additional concomitant infectious complications are frequent. Symptomatic pulmonary Kaposi's sarcoma most oftenly is progressive and has a poor prognosis. Preliminary data suggest effective palliation with radio- and chemotherapy and an improved survival time in selected cases. We present six cases of proven pulmonary Kaposi's sarcoma and discuss clinical course and diagnostic difficulties. Based on this view we propose a diagnostic approach to warrant therapeutic success.

[Cardiac involvement in non-Hodgkin's lymphoma].

In a 47-year-old man with non-Hodgkin's lymphoma which had been treated by polychemotherapy and radiation, cardiac involvement was suspected on computed tomography (CT) two years after diagnosis. The ECG demonstrated atrial flutter and a slow ventricular rate. After a further cycle of chemotherapy the patients declined further treatment. Two years later he was admitted urgently because of cardiac failure, atrial fibrillation with a slow ventricular rate, and clear signs of abnormal repolarization. CT of the thorax again provided signs of cardiac involvement with lymphoma. In addition there were extensive tumour infiltrates in the liver and paraaortic lymph-node. Despite intensive medical treatment he died shortly after admission from cardiac failure. At necropsy all four cardiac chambers and the coronary arteries were surrounded completely by tumour tissue. The membranous interventricular septum, the area of the Hiss bundle and the tricuspid and mitral valves had also been infiltrated. Histologically the infiltrates consisted of lymphoid cells of centrocytic type. Increasing tumour cell involvement went together with complete disintegration of myocardial fibres.

Diagnosis of acute and latent varicella-zoster virus infections using the polymerase chain reaction.

A simplified assay for the diagnosis of varicella-zoster virus (VZV) infections based on the polymerase chain reaction (PCR) is described. Omitting the procedures for extraction and purification of DNA, the crude vesicle fluid materials were used for PCR. Moreover, hybridization was not necessary for detection of the amplification products because they were already visible after ethidium bromide staining of the electrophoresis gel. Results could therefore be obtained within one day. In comparison to virus isolation, PCR proved much more rapid, highly sensitive, and specific. DNA extraction and a double PCR assay with nested primers were necessary for detection of latent VZV infections in trigeminal and thoracic ganglia. The data suggest that the procedures described are universally applicable to several types of specimens dependent on the calculated amount of target DNA.

[Epidemiological and pathomorphological autopsy findings in AIDS].

The autopsy findings in 41 AIDS patients (1982-1991), including a rather large group of 8 patients without one of the classical risk factors for HIV, showed a much higher rate of opportunistic infections, especially CMV and fungal infections, as well as Kaposi's sarcoma in homosexuals than in the others. There was no decline in the incidence of Kaposi's sarcoma over the last few years. The bone marrow findings, which presented a remarkable high cellularity in these patients, well correlated with the so-called HIV-myelopathy, which is highly suggestive to represent a direct HIV-related damage of the bone marrow.

Hemangiopericytoma of the lung--a rare differential diagnosis of lung tumors.

The authors report of a 50 year old woman who was operated on because of a rectum carcinoma 5 years ago. At a follow-up examination a 2 x 2 x 1 cm sized tumor was found in the lower lobe of the right lung. Under the diagnosis of lung metastasis after rectum carcinoma resection, operation (wedge resection) was carried out. The histological examination revealed the rare tumor hemangiopericytoma. Based on this case, frequency of the tumor, clinical symptoms, surgical treatment, prognosis as well as recurrence rate and formation of metastases are discussed.

Association of macrophages detected with monoclonal antibody 25 F 9 with progression and pathobiological classification of gastric carcinoma.

Using monoclonal antibody 25 F 9, which reacts with a determinant of mature macrophages, the inflammatory infiltrate of 66 gastric carcinomas was evaluated using a counting grid. The ratio tumor cells/macrophages was determined for every tumor. For a threshold value of 5, carcinomas with a better prognosis, such as the intestinal type according to Lauré, the expanding type according to Ming and the differentiated carcinomas according to the WHO had a significantly smaller relative content of 25 F 9-positive macrophages (a minimum of P less than 0.05) than the diffuse type, infiltrative type, and undifferentiated carcinomas. Furthermore, the relative macrophage content tended to increase with the stage of carcinomas spread (P less than 0.1). The results suggested that 25 F 9-positive macrophages in gastric carcinoma are of greater significance in tumor spread than in any defensive reaction against the tumor.

Idiopathic calcification of the mitral valve.

The localization of primary calcified deposits in cases of idiopathic calcification of the mitralis was studied microscopically in the mitral tissue of 18 autopsy cases of women over 60. Macroscopic inspection revealed small calcified foci in 12 cases, none in the remaining 6. Microscopically, calcified foci were found in all cases, localized at the insertion of the mitral cusp. The annulus was involved in 8 cases only. At the site of cuspal insertion these foci were situated in fibrous thickenings towards the endothelial surface of the ventricularis. According to these findings, calcified deposits accumulate primarily at the cuspal insertion site, rather than in the annulus. Age-related fibrosis of the mitral cusp appears as a predisposing factor for dystrophic calcification.

[Tumor invasion of the vena cava inferior in adrenal carcinomas. The preoperative diagnosis].

Tumour invasion of the inferior vena cava from an adrenal carcinoma is not a contraindication to surgical treatment. Pre-operative recognition of the situation is of considerable importance for planning the surgical procedure. Three patients with primary adrenal carcinomas are reported, in whom tumour in the cava was demonstrated by real time and duplex sonography. Sonography proved superior to CT in these cases.

Necrotizing fasciitis caused by Vibrio vulnificus: first published infection acquired in Turkey is the second time a strain is isolated in Germany.

Vibrio vulnificus, a marine vibrio, has recently been recognized as a potential human pathogen. It causes human infections with mortality rates up to 60%. Until 1991, most human isolations were reported from the USA, Japan and Taiwan. The second strain isolated in Germany is documented and a significant case of V. vulnificus infection acquired in Turkey is published for the first time.
