Biological modifications of the vitreous in intraocular parasitosis: preliminary study.

In intraocular parasitosis the immunologic reactions in the vitrectomy-fluids depend on the parasitic agent: there is a lymphocytical reaction without any IgE production in toxoplasmic chorioretinitis, whereas there is a strong hypersensitivity reaction with a local IgE production as well as the presence of eosinophils and an antigenic stimulation in fluids with toxocara parasitosis. The electrophoretic patterns are quite similar in both cases.

[Orbital invasiveness of Merkel cell tumor of the eyelid].

A new case of neuroendocrine carcinoma (Merkel cell tumor) of the eyelid is reported. Local recurrence, spread to the orbit, led to exenteration and death of the patient. This trabecular carcinoma can be identified by histology, immuno-histochemistry and electron structural microscopy.

Birdshot retinochoroidopathy and HLA-A29+ and HLA-A29- idiopathic retinal vasculitis: comparative study of 56 cases.

We reviewed the records of 20 patients with birdshot retinochoroidopathy and 36 patients with idiopathic retinal vasculitis. The numbers of patients with the HLA-A29 phenotype were 19 (95%) and 22 (61%) respectively. Overall, similar findings were noted in the birdshot retinochoroidopathy group and the HLA-A29+ idiopathic vasculitis group: more posterior pole involvement, more severe course and poor visual prognosis despite intense corticosteroid therapy. In contrast, the patients with HLA-A29- idiopathic vasculitis showed more peripheral involvement, the course was less severe and the prognosis was better; smaller amounts of steroids were used. Only one patient with HLA-A29+ idiopathic vasculitis eventually presented with signs of birdshot retinochoroidopathy; complete cure was obtained in other cases. Our results suggest the existence of an HLA-A29 syndrome encompassing birdshot retinochoroidopathy as well as other conditions whose expression would be determined genetically.

Ocular toxoplasmosis--clinical aspect.

Ocular toxoplasmosis in non immuno-deficients can take 3 major aspects: 1) Pseudo-acquired (congenital) forms in teenagers and young adults; 2) Congenital forms in the newborn; 3) Acquired toxoplasmosis.

The possible role of herpes viruses in multifocal choroiditis and panuveitis.

7 cases of multifocal choroiditis and panuveitis are reported here (6 females, 1 male). All clinical data were carefully considered. In all cases an aqueous sampling was made for the detection of anti-herpes virus antibodies in aqueous and serum. 3 specificities were tested: herpes simplex (HSV), herpes zoster (HVZ) and cytomegalovirus (CMV). An intraocular synthesis of specific antibodies was found against VZV in 2 cases and against HSV in 1 case. There was another presumptive case for HSV.

Nonspecific immunoglobulin E in aqueous humor: evaluation in uveitis.

The levels of immunoglobulin E (IgE) in aqueous humor (AH) and serum were determined in 74 uveitis patients and 13 control patients with senile cataract. Uveitis patients' levels ranged from less than 0.06 to 460 IU/ml according to the paper radioimmunosorbent test (PRIST; limit of sensitivity, 0.06 IU/ml). Only one control patient had an AH level higher than the sensitivity limit (0.08 IU/ml). Of the five patients with the highest levels in the AH, four had toxocaral retinitis. The ratio of AH/serum to IgE normalized to the albumin concentration was calculated in 25 uveitis patients and was greater than 0.60 in 13 cases, indicating a possible intraocular IgE synthesis.

[Introduction to the round table discussion on intermediate uveitis].

Intermediary uveitis is one of the most mysterious and difficult to treat among all inflammations of the eye. From a recent statistic stemming from 2 different series of 20 cases of our patients, the following data have been found: all cases seen after 4 years of evolution are still evolutive; with 8 years follow-up, 42% are still evolutive. Recent findings supplied with the help of angiofluorophotometry give interesting details concerning the physiopathology of the disease. So, the leakage through the hemato-ocular barriers would appear not only as the result of a defect in anterior or posterior barrier but a defect of the entire barrier. Even after the clinical healing of the disease, the barrier remains often permeable. Therefore, the prolongation of the evolution of an intermediary uveitis may be the consequence of not necessarily an immune process but of a definitive anatomic sequellae.

[Ocular toxoplasmosis in 1989].

The clinical aspect of congenital toxoplasmosis is sometimes typical, sometimes atypical. The advantage of the serological study of the aqueous is an early diagnosis and an early treatment for less expense. The classical treatment relies on the therapeutic association Pyrimethamine + Adiazine + corticosteroids which will be used during acute phases, at ordinary doses, but may be prolonged further over many months in difficult cases. Severe forms are those that are spontaneously severe or increased with the exclusive use of corticosteroids in previous attacks, noticeably when injected by local route, and in bilateral forms. Prevention of ocular toxoplasmosis is represented by the detection of sero-conversion in mother and treatment in the newborn. Due to the legal aspect of this procedure in France, the incidence of congenital toxoplasmosis has already decreased during the past years and will continue to diminish in the next future. However the possibility of acquired forms cannot be excluded, even in non immuno-depressed patients.

New approaches to the detection of locally produced antiviral antibodies in the aqueous of patients with endogenous uveitis.

A local synthesis of IgG antibodies against herpes simplex virus, cytomegalovirus, varicella-zoster virus and Chlamydia trachomatis was assayed for in aqueous humor and sera from 226 patients with various clinical types of uveitis. Comparison of the ratios of specific antibodies tested for in aqueous humor and serum (C') and the evaluation of the Goldmann-Witmer coefficient (C) to each specific antibody were simultaneously studied in each case. The combination of these two methods allowed for the diagnosis of viral uveitis in 37.7% of selected uveitis patients and in 13.27% of randomized uveitis patients.
