[Coronary surgery at the Cardiovascular Surgical Clinic (1976-1990)].

In the Cardiovascular Surgical Clinic of the Semmelweis Medical University of Budapest the first coronary artery bypass grafting (CABG) procedure was performed in 1975. Since that time coronary artery surgery has become a routine everyday practice representing more than half of the total workload of adult cardiac surgery. The analysis of 1347 operations performed between 1976-1990 on patients with coronary heart disease showed the followings: the first few years--so called learning curve of CABG operations is characterised by high mortality. With passing time the number of cases performed each year increased rapidly and the surgical technique has improved too. At the same time the operative mortality figures showed decreasing tendency--it was 2.1% for the last 609 cases. All observed parameters showed some progress: in 1990 the average number of grafts per patient was 3.09, internal mammary artery usage 15 percent and the mean aortic cross clamp time per anastomosis 24.5 minutes. Complete myocardial revascularisation is the key point of coronary artery surgery. In order to achieve this target in all operated cases further technical improvement is necessary.

Coronary Artery Bypass

[Development and results of valve replacement surgery at the Városmajor Clinic 1976-1990 (retorospective clinical analysis)].

In Hungary valve replacement is still a major indication for heart surgery in adults. In the Cardiovascular Surgical Clinic of Semmelweis Medical University of Budapest from 1976 to 1990 2435 patients were operated for valve disease. Majority of the cases had single (aortic n = 856, mitral n = 912) or double (aortic + mitral n = 513) valve replacement. Over this 15 years period there have been many alterations in patients characteristics and surgical technique as well. In spite of the increasing mean age of patients the operative mortality has decreased (in the last 5 years period it was 2.7%, 5.5% and 7.9% in the three groups respectively). At the same time the number of patients requiring valve re-replacement or combined valve + coronary procedure has increased. The use of bioprosthetic valves has fallen below 10 percent from the 60--80 percent observed between 1976--1980. The analysis showes excellent surgical results in the field of valve replacement in Hungary.

Cardiac Surgical Procedures

[Experience with the surgical management of constrictive pericarditis].

61 patients were operated on for constrictive pericarditis at the Cardiovascular Surgical Clinic of Semmelweis Medical University in the last 33 years. The average hospital mortality of the surgical pericardiectomy was 4.9%. The final conclusions of this retrospective and follow up study are as follows: pericardiectomy is the method of choice in the treatment of constrictive pericarditis, since it does not has any therapeutic alternative, its hospital mortality is low and it results excellent early and late postoperative functional effects.

Congenital complete heart block associated with QT prolongation.

The coexistence of congenital complete heart block and QT prolongation represents a special type of arrhythmia. The electrophysiological and clinical characteristics of this syndrome were studied in eight patients suffering from congenital AV block and QT prolongation. Data from 22 patients suffering from congenital complete heart block only, served as a control. In the study group, the appearance of a torsade de pointes type of ventricular tachycardia could regularly be observed and the tachycardial attack could usually be provoked by ventricular extrastimuli. The corrected QT time was markedly prolonged; on ventricular stimulation, at higher pacing rates the QT interval shortened, but remained significantly higher than in the control group. Syncopal attacks--with the character of polymorphic tachycardia--appeared in each patient of the study group while occurring in only three patients from the control group. Patients were given pacemaker implants (using a higher pacing rate) and long-term administration of beta-receptor blockers. The outcome was favourable; no ventricular tachycardia or syncopal attack was observed in the follow-up period.

Biological valve prosthesis replacement--experiences and considerations.

Between 1976 and 1991. 1.017 tissue valve prostheses were implanted in 801 patients. During the same period 1876 mechanical valve prostheses were replaced. The hospital mortality was 8.1%. Till 1991 230 bioprostheses (22.6%) in the case of 166 patients (20.7%) had to be removed and replaced by other valve prostheses. There are no significant differences concerning the mortality between the first (8.1%) and the second operations (9.1%), and the durability of the various types of bioprostheses used, however, calcification, degeneration and other complications occurred more frequently and earlier in the case of mitral (24.5%) than in aortic (18.9%) bioprostheses, and in younger patients than in the older ones, as well. The mean age of patients was 46 years at the time of the first and 49 years at the time of the second operation. The incidence of reoperations was the highest in the seventh year after the first surgical intervention. In general, one size smaller prostheses were used in valve replacements after the removal of the first bioprostheses.

Bioprosthesis

Changes in the pacemaker function of the sinus node after closure of atrial septal defect. Temporary and stable iatrogenic sick sinus syndrome.

The alterations in the clinical symptoms and the pacemaker function of the sinus node following open heart operation were studied in 26 patients suffering from atrial septal defect. The most characteristic electrophysiological change was the prolongation of the sinus node recovery time as consequence of surgical repair of the atrial septal defect. The clinical symptoms of the sick sinus syndrome could be detected in all 26 cases after the open heart surgery. In 16 patients the symptoms of iatrogenic sick sinus syndrome (SSS) were only transitory, while in 10 patients a stable iatrogenic SSS developed after the operation. In the basis of these data the pathological mechanism of the iatrogenic SSS is discussed.

[Sclerotic occlusion of venous bypass].

The case of a patient is presented in whom in the course of repeated vascular surgery a venous by-pass was prepared. Eighteen months after the operation a sclerotic plaque occluded the bypass. After successful de-obliteration the patient became free of complaints. A review of the pertaining literature is presented and the possible causal factors are discussed.

Arteriosclerosis Obliterans

Baropacing of the carotid sinus nerve for treatment of "Intractable" hypertension.

The authors applied with success the electrical stimulation of the carotid sinus nerve in the case of a hypertension of long duration which was refractory to antihypertensive drugs. For the stimulation of the carotid sinus nerve a Medtronic "Barostat" device was implanted. Since the implantation- or rather since its usage- the blood pressure of the patient decreased to a great extent, and her complaints disappeared. According to the haemodynamical investigations on the effect of the stimulation of the carotid sinus nerve, besides the diminution of the blood pressure there could be observed a decrease of the cardiac index, the total peripheral vascular resistance and the velocity of the blood circulation.

The effect of isolated perfusion on limb circulation in peripheral arterial occlusion.

The effects of isolated limb perfusion were studied in 12 patients suffering from peripheral arterial occlusion. The hemodynamic investigations show that the vascular resistance decreases considerably during isolated limb perfusion. After administration of a vasodilator cocktail, added to the perfusion, the vascular resistance continues to decrease. Angiographic records, made before, during and after the perfusion, suggest that the calibre of the collateral vessels increases significantly and besides new collaterals can also be observed. The mechanism of the effects of isolated hyperbaric limb perfusion and its clinical value are discussed.
