[Exfoliative cytology for diagnosis of Helicobacter pylori].

The development of more reliable diagnostic methods has undoubtedly contributed to a better knowledge of the epidemiology and role of Helicobacter pylori (Hp) in the physiopathology of upper digestive tract diseases. Non abrasive and abrasive diagnostic methods have been used; among the latter, the exfoliating of abrasive cytology has been very popular during the last years. This fact motivated us to do this investigation. A descriptive-prospective study was developed in which the sample was composed by 235 patients of both sexes. The patients underwent gastro-duodenoscopy on account of dyspeptic symptoms and in order to determine the presence of Hp and its association with endoscopically diagnosed gastritis, with or without duodeno-gastric reflux, using the SYDNEY classification system. The frequency of Hp was assessed and the density of this microorganism was correlated with the hystological type of chronic gastritis following the above-mentioned classification. As a method of diagnosis of this disease, cytology showed a sensibility of 72% a specificity of 100% and an efficiency of 77% compared with histology, that was considered the "golden test". It was histologically proved that there exists a close relationship between chronic gastritis and the presence of Hp in the gastric antrum and that when the density of the bacterial population increases, the severity of chronic gastritis increases too. This germ may show up in histologically normal mucose, being able to provoke dyspeptic symptoms and colonizing the gastric antrum independently of the presence of duodeno-gastric reflux.

[Study of lymphocyte subpopulations with monoclonal antibodies in acute hepatitis A and B].

Using monoclonal antibodies, it was determined the T3, T4 and T8 1ym phocyte populations in the blood samples of 40 patients with acute viral hepatitis (20 type A and 20 type B). al patients showed complete remission from it. The results of this study were compared with those of a control group of 20 healthy subjects. There was a non significant decreased of the total number of lymphocytes measured with the T3 monoclonal antibody in both types of hepatitis. The number of T8 lymphocytes was significantly increased (p less than 0.05) when the results were compared with those of the control group. The quotient T4/T8 was diminished in the initial phase of the disease (p less than 0.05). There were no other differences between hepatitis A and hepatitis B. In the follow up of the disease, the results tend to normal values in the whole group of patients.

[Study of lymphocyte subpopulations by means of monoclonal antibodies in acute hepatitis A and B].

We studied T3, T4 and T8 lymphocyte populations in peripheral blood by monoclonal antibodies in 40 patients with acute viral hepatitis (Type A 20; Type B 20) who underwent outcome to complete recovery. We compared the results with 20 healthy subjects (control group). We found a decrease in total lymphocytes measured by T3 monoclonal antibodies and a significant increase in T8 lymphocyte populations compared with control groups. In the early stage the T4/T8 ratio was decreased. Lymphocyte populations and T4/T8 ratio was normal in 3-6 month follow-up for the acute A hepatitis group and 3.6 month to a year follow-up for the B hepatitis group. Results were related to the humoral and clinical outcome.

[Treatment with Savary-Gilliard bougies in esophageal stenosis in children].

30 patients (less than 15 years old) were admitted for esophageal strictures, 16 of them secondary to corrosive injury. All the patients were treated with endoscopic dilatation with Savary-Gilliard bougie. The dilations were done with general anesthesia using an Olympus GIF-XP10 endoscope and with fluoroscopic control. In the esophageal stenosis secondary to caustic ingestion endoscopic injection with Betamethasone was also used. The most frequent site of the stenosis was the upper third of the esophagus, and the main type of stenosis was tubular in the secondary to caustic burns and annular in the other group. In the post-caustic group 385 dilations were performed in 115 sessions. In the other group 159 dilations were done in 51 sessions. Two perforations and one sepsis were reported in patients with corrosive stenosis. There was no mortality. 43.7% of the patients with corrosive stenosis and 85.7% with stenosis secondary to other causes obtained complete healing. Esophageal dilation with Savary-Gilliard bougies represents a safe and reliable method for the treatment of esophageal strictures.
