[Clinical results with a complex conservative treatment method in infiltrative ophthalmopathy (preliminary report)].

The present methods for the treatment of infiltrative ophthalmopathy are still unsatisfactory, some of them associated with considerable and lasting side effects. Clinical results are present in the communication, from the application of an original therapeutic complex, consisting in the parallel administration of ACTH, intravenous infusions, moderate doses cortison, diuretics and sanation via antibiotics and eventual surgical intervention of the existing focal infectious agents. The complex is applied in the course of 4 weeks. Regression of the clinical symptomatics is observed and assessed by a particular method, it reaches an average of about 50 per cent of the initial manifestations. In 45 per cent of the patients the result was assessed as very good and excellent. The subjective complaints, congestion, chemosis and edema are best and most rapidly influenced, whereas exophthalmos and the eventual ophthalmoplegic phenomena showed no essential changes. The correlations between the results obtained, accompanying the disease (Basedow disease, focal infections) and the severity of ophthalmopathy were discussed. The method was admitted to be perspective, harmless and could ensure good and lasting therapeutic results of properly and persistently applied.

[Therapeutic qualities of a lente preparation of swine insulins manufactured by Pharmachem].

The therapeutic effect of swine insulin preparation Lente, "Pharmachim" production was studied in 50 diabetics. Its hypoglycemizing activitis compared with that of standardized insulin Lente--"Novo", extract of bovine pancreas. The Bulgarian preparation was found to be very similar to the preparation Lente-Novo in its pharma-codynamic properties and could successfully be used in the diabetes melitus treatment. Insulin Lente Pharmachim is presumed to have lower antigenicity as an extract of swine pancreas, which is actually its great advantage for the therapeutic practice.

[Itsenko-Cusing syndrome in hypohyseal adenoma].

A case observed at the Insitute of Endocrinology, Gerontology, and Geriatry is reported with Icenko--Cushing syndrome developed in consequence of hypophyseal adenoma. The telegammatherapy (total focal dose 4760 rads) carried out in the course of one year led to a complete regression of the developed alterations-- elimination of the redistribution of fats, restoration of the face normal colour and oval, nails and hair normalization as well as of cortisolemia and decrease in the osteoporosis degree with consolidation of the pathological fractures. Blood pressure (without hypotensive treatment) within normal values--140/90.

[Observation of a male patient with the empty sella turcica syndrome].

One case with secondary developed syndrome of sell turcica is described, after hypophysis radiation on the occasion of acromegaly. Of special interest is the preserved tropic hypophysis secretion with a clinical picture of hypopituitarism (secondary developed hypogonadism, hypothyroidism and hypocorticism) as well as the combination with another brain neoplasm--astrocytoma.

[The functional state of the hypothalamo-hypophyseal system in tumors of the pituitary and diencephalic area].

A study was made of 38 persons, including 25--with acromegaly, 5--with chromophobic adenomas of the hypophysis, 3--with diencephalic tumours and 5 healthy individuals. STH, TTH and ACTH were determined in the blood plasma with the aid of radioimmunological methods under basic conditions and L-DOPA loading (10 mg per 1 kg of weight) with insulin (0.10 U/kg). There was an increase in the plasma content of STH, TTH and ACTH with a maximum 60 min after the administration of L-DOPA. In the patients with acromegaly STH increased considerably, and in cases with chromophobic adenomas and diencephalic tumours--less. ACTH level was also elevated. STH and ACTH level rose after insulin hypoglycemia. Possible mechanisms of a stimulating action of L-DOPA and insulin are discussed.
