[Heart surgery with extracorporeal circulation in pregnant women. Analysis of materno-fetal outcome].

PURPOSE: To analyze maternal and fetal outcome in pregnant undergone to cardiac surgery. METHODS: We studied the evolution of 30 pregnant women submitted to cardiac surgery at the Hospital São Paulo, between Jan/81 and Dec/92 and, further, attended at this Hospital till the parturition. The following variables were analyzed: cardiopulmonary bypass, time of the procedure and time of the anoxia, patient temperature, surgical complications, and neonatal, maternal and fetal outcomes. RESULTS: All patients had rheumatic heart disease and, in 17, mitral stenosis was the main anatomic abnormality. Mitral commissurotomy was performed in 24 patients, double comissurotomy (mitral and aortic) in 1 patient and valve replacement was performed in 5. Cardiopulmonary bypass was utilized in all procedure; occurrence of surgical complications (p < 0.001) and the prolonged surgical time (p = 0.009) were related to the fetal mortality. There was 4 (13.3%) maternal deaths and 10 (33.3%) fetal deaths related to the surgery. CONCLUSION: The indication of cardiac surgery in pregnant women is heart failure, refractory to conventional therapy; cardiopulmonary bypass is associated with high fetal mortality.

[Pregnancy after heart transplantation].

A 25-year old female patient, with peripartum cardiomyopathy was submitted to heart transplantation and became pregnant two years after. Pregnancy was uneventful and a baby-girl was born completely healthy. Six years after pregnancy, the mother remains well with preserved ventricular function and the girl has had a normal development. Although considered a high-risk situation and not indicated, pregnancy after heart transplantation is possible, without any apparent deterioration of the transplanted graft and with healthy babies being delivered.

ATP reduces blood loss produced by heparin in cardiopulmonary bypass operations.

It was previously shown that topical application of heparin produces enhanced bleeding from small vessels and capillaries. Adenosine triphosphate at low concentrations is able to dislodge heparin bound to a receptor, counteracting its antihemostatic activity. These results led us to measure the amounts of heparin remaining in the blood after protamine neutralization of the patients subjected to cardiopulmonary bypass operation and to test the topical application of the nucleotide. Adenosine triphosphate at a concentration of 10(-4) mol/L significantly reduces the blood volume (p < 0.005) oozed from the thoracic cavity of the patients (mean, 288 +/- 188 mL) when compared with controls (mean, 564 +/- 288 mL). Adenosine triphosphate at 5 x 10(-5) mol/L reduces the blood loss to a mean of 370 +/- 155 mL in the patients tested (p < 0.08). About 10% of heparin of low molecular weight (< or = 6.0 Kda), which is also found in the oozed blood, is not neutralized by protamine. We suggest that the excessive blood loss of the patients is probably produced by low molecular weight heparins in the commercial preparations that are not neutralized by protamine.

Successful use of deep hypothermic circulatory arrest in pregnancy.

A woman pregnant for 21 weeks underwent an emergency operation because of an aortic arch aneurysm that had ruptured into her left lung. Cardiopulmonary bypass and deep hypothermia were used and she needed 37 minutes of circulatory arrest at a core temperature of 19 degrees C. Both the mother and fetus survived, and a normal baby was delivered by cesarean section at the 39th week of gestation.

[Prolonged respiratory support with extracorporeal membrane oxygenation in lung transplantation].

A 42 year-old woman with terminal chronic lung disease underwent to left lung transplantation. Extracorporeal membrane oxigenation (ECMO) was required because dysfunction of transplanted organ occurred and was non-responsive to conventional therapy. The time of assistance was 47 hours and after this, the dysfunction of the transplanted lung reversed and the patient was weaned from the oxigenator. During hospital stay, she developed sepsis and died. In conclusion, ECMO was decisive to the treatment of pulmonary dysfunction, allowing time to the resolution of lung lesion.

[Late development of congestive heart failure in interventricular communication caused by penetrating wound, with pulmonary hypertension resolution after surgical correction].

Male, 33 years-old, with onset of heart failure and edema; he used to be completely asymptomatic but mentioned he underwent previously a pleural drainage, after a hemothorax provoked by a stab wound. His physical examination confirmed he was in heart failure and showed a harsh holosystolic murmur between mitral and tricuspid areas. Cardiac catheterization demonstrated a large apical ventricular septal defect (VSD) and systemic pulmonary arterial pressures. With oxygen there was an increase in the left to right shunt and normal pulmonary vascular resistance. The clinical diagnosis was that of a traumatic VSD provoking pulmonary hypertension although a congenital etiology could not be completely ruled out. At surgery there was a sharp cut at the pericardium and an apical epicardial lesion; the VSD was patch sutured and the patient did well with control of his heart failure. A control hemodynamic study performed within the first postoperative month showed no residual shunt and complete normalization of the pulmonary pressures and resistance.

[Myocardial revascularization without extracorporeal circulation. Surgical results in 1090 patients].

PURPOSE: To report our experience of 12 years of myocardial revascularization without cardiopulmonary bypass (CPB) and to discuss its expanding surgical indications. METHODS: This technique was employed in 1090 patients, between September/81 and April/93, corresponding to 17.4% of all revascularized patients. Regarding gender, male patients were the majority (73.7%) and ages ranged from 28 to 86 year-old (mean 57). Chronic coronary insufficiency was the main indication (69.4%) and 18.9% of our global experience were operated on during acute ischemia. The average number of grafts per patient was 1.6 and internal mammary artery was utilized in 60.1% of patients. Left anterior descending coronary artery and right coronary artery were the coronary arteries most often treated. RESULTS: Our hospital mortality rate was 2.5% (28/1090) although it was 7% for patients over 70 years. The most common cause of death was cardiogenic shock after failed angioplasty. Postoperative complications included AMI in 4.8%, arrhythmias in 5.5% and pulmonary in 3.2%. Mean of any type of blood derivate utilization was 0.28 units/patients. CONCLUSION: Myocardial revascularization without CPB is a valid alternative in a selected cohort of patients and it is performed with low morbidity and mortality. Nowadays it represents an excellent tactical option for high-risk patients and in failed angioplasties.

[Doppler echocardiography evaluation of the hemodynamic performance of the Lillehei-Kaster and Medtronic Hall valves in aortic position].

PURPOSE: To establish the range of normal values by Doppler echocardiography of the parameters inherent to the hemodynamic performance of the Lillehei-Kaster (LK) and Medtronic Hall (MH) prosthetic valves (V), in the 23 mm (LKV-16, MHV-23) and 25 mm (LKV-18, MHV-25) outer diameter valves, in the aortic position. METHODS: Doppler echocardiography was performed in 32 asymptomatic patients, with normally functioning prosthetic aortic valves (8 of each type). RESULTS: The peak velocity of aortic jet was 3.13 +/- 0.46 m/sec for the LKV-16, 2.76 +/- 0.31 m/sec for the MHV-23, 2.82 +/- 0.48 m/sec for the LKV-18 and 2.43 +/- 0.36 m/sec for the MHV-25. The maximal pressure gradient was 39.84 +/- 12.05 mmHg for the LKV-16, 30.70 +/- 6.80 mmHg for the MHV-23, 32.60 +/- 10.75 mmHg for the LKV-18 and 24.11 +/- 6.70 mmHg for the MHV-25. The mean pressure gradient was 24.25 +/- 7.09 mmHg for the LKV-16, 18.50 +/- 4.41 mmHg for the MHV-23, 18.21 +/- 6.95 mmHg for the LKV-18 and 13.57 +/- 4.17 mmHg for the MHV-25. The valve effective orifice area, considering the left ventricle outflow area determined by two-dimensional echocardiography in the continuity equation, was 0.62 +/- 0.13 cm2 for the LKV-16, 1.05 +/- 0.21 cm2 for the MHV-23, 0.98 +/- 0.22 cm2 for the LKV-18 and 1.36 +/- 0.36 cm2 for the MHV-25. Considering the left ventricle outflow area equivalent to the valve sewing ring area, the valve effective orifice area was 1.40 +/- 0.23 cm2 for the LKV-16, 1.65 +/- 0.19 cm2 for the MHV-23, 1.91 +/- 0.43 cm2 for the LKV-18 and 2.37 +/- 0.56 cm2 for the MHV-25. The Doppler velocity index was 0.34 +/- 0.05 for the LKV-16, 0.40 +/- 0.04 for the MHV-23, 0.39 +/- 0.09 for the LKV-18 and 0.49 +/- 0.11 for the MHV-25. CONCLUSION: Significant hemodynamic performance superiority of the MHV over the LKV was registered, indicating that the small LKV (LKV-16 or smaller) should be avoided.

[Hypothermic retrograde cerebral perfusion for treatment of air embolism during extracorporeal circulation].

Air embolism during extracorporeal circulation is a life-threatening complication, requiring immediate response. During a recent case, brain damage was avoided using hypothermic retrograde cerebral perfusion, connecting the arterial line to superior vena cava cannula. Perfusion lasted 5 minutes at flow of 200ml/min. The patient had complete recovery and was discharged with normal neurologic status. The technique employed is described and steps for a face this accident are proposed.

[An unusual indication of deep hypothermia with total circulatory arrest for myocardial revascularization surgery].

The authors relate a case of 69 years old patient with unstable angina and left main coronary artery disease, who underwent aggressive mediastinal radiotherapy for lymphoproliferative syndrome 20 years ago and who needed urgent myocardial revascularization. Due to great calcification and heavily heart adherences it was performed 20 degrees C deep hypothermic circulatory arrest to avoid aortic clamping and to make the anastomosis with better exposition of the aorta and coronary arteries.

[Artificial respiration with extracorporeal membrane oxygenator in arteriovenous position without pump. Experimental study].

PURPOSE: To test the hypothesis of respiratory support and to study the consequent hemodynamic alterations, we performed pumpless A-V ECMO using a 1.2m2 polipropylene hollow fiber membrane oxygenator, specially adapted from conventional model utilized in routine cardiac surgery. METHODS: Two canine groups of six dogs each were studied. Group 1 had normal lungs and group 2 acute respiratory failure induced by 0.035ml/kg intravenous oleic acid. The dogs were anesthetized and maintained in apnea with curare. Heparin was given in a single dose of 400U/kg. Gasimetric and hemodynamic parameters were monitored each 30 minutes during a 3 hour period. Then the oxygenator was discontinued and after 15 minutes a final arterial blood gas sample was taken for analysis. RESULTS: Blood samples at 3 hours have shown a mean PaO2 of 260.6mmHg in group 1 and 114.4mmHg in group 2 and PaCO2 of 54.3 and 56.2mmHg, respectively. After the oxygenator was discontinued PaCO2 levels increased to 161.6 and 193.7mmHg, respectively. The hemodynamic parameters shown few alterations. CONCLUSION: In both groups pumpless A-V ECMO was able to maintain gasimetric parameters within acceptable patterns and compatible with life, with few hemodynamic alterations. However, an adequate mean arterial blood pressure is needed to provide flow through membrane oxygenator.

[Early and late results of myocardial revascularization after coronary thrombolysis with intravenous streptokinase in the treatment of acute myocardial infarction].

PURPOSE: To study the early and late results of patients treated initially intravenous streptokinase and then with coronary artery bypass surgery. METHODS: One hundred and twenty one patients with acute myocardial infarction less than 6 hours duration were treated initially with intravenous streptokinase, and 1-38 days after (median 8.80 days) coronary artery bypass was undertaken. Ninety six patients were operated with extracorporeal circulation and 25 without it. RESULTS: Overall operative mortality was 3.30% (4/121). Reoperations due to bleeding was necessary in 3.30% (4/121). Late mortality was 5.40% with survival probability of 94.60% after 36 months and 92.30% after 70 months. Late morbidity events in terms of angina and cardiac insufficiency demonstrated that 7 patients had angina, 4 cardiac insufficiency and two angina and cardiac insufficiency; probability to be free from these events was 88.20% after 46 months and 66.40% after 70 months. CONCLUSION: Coronary artery bypass surgery after intravenous streptokinase can be undertaken with security and excellent early and late results.

[Systemic-pulmonary shunts in congenital heart diseases with decreased pulmonary blood flow: critical analysis of the surgical techniques and immediate results].

PURPOSE: To evaluate immediate postoperative results in children with congenital heart disease and decreased pulmonary blood flow who underwent a systemic to pulmonary shunt. PATIENTS AND METHODS: Sixty-four patients underwent surgery, 46.8% (30) of them males with ages from 1 day to 17 years old. They were divided in three groups: I--13 pts (20.3%) who underwent classical Blalock-Taussig (BT) shunt; II--46 pts (71.8%) who underwent modified BT shunts, 34 of them with polytetrafluoroetylene (PTFE) and 12 of them with umbilical vein shunts; III--5 pts (7.8%) with central anastomosis that were made with three different types of graft; 3 PTFE, umbilical vein and bovine mammary artery in one each. The simultaneous procedures were: section and suture of PDA--2, closure of systemic-pulmonary collaterals--3, pulmonary valvotomy--3, right ventricular outflow patch--3, pulmonary branch stenosis (enlargement)--2. RESULTS: In group I there were 4 (30.7%) closures, with two immediate reoperation and a total mortality of 30.7%. In group II there were 4 closures (8.6%) and two reoperations with a mortality of 15.2% (7 cases). In group III there was one graft closure and an overall mortality of 80% (4 pts). When analysing deaths due to the shunt itself the mortality rate was respectively 15.3%, 8.6% and 40.0%. CONCLUSION: Modified BT operation was performed most frequently in our service; it was associated with less closure and mortality than the other types of shunt. It is considered our first alternative for a systemic-pulmonary shunt.

Myocardial revascularization without extracorporeal circulation. Seven-year experience in 593 cases.

The authors present the results of 593 consecutive and non-selected patients who underwent direct myocardial revascularization without extracorporeal circulation in the period September 1981 to December 1988. The results are compared with 3086 patients who underwent conventional bypass surgery over the same period of time. The ages varied from 33 to 80 years (mean = 56) with 40 patients older than 70. The overall mortality in the group without cardiopulmonary bypass (CPB) was 1.7% (10/593) compared with 3.8% for conventional revascularization. Our mortality in the last 3 years has been 0.5% and 0% in the last 203 patients revascularized without CPB. The number of grafts varied from 1 to 5 (average 1.6). The treated arteries were: anterior descending (557), right (282) marginal (5) and circumflex marginal (4). Immediate postoperative complications were fewer and hospital discharge was earlier in the group treated without CPB. The authors conclude that this tactical alternative has the advantages of: lower morbidity and mortality, lower cost and no need for blood transfusion. Drawbacks are the need for greater technical expertise and it seems to be possible in about 19% of all patients who undergo myocardial revascularization.
