Buspirone augmentation of antidepressant therapy.

Thirty outpatients meeting DSM-III-R or DSM-IV criteria for major depression, single or recurrent episode, and failing to respond to an adequate trial of an antidepressant (>6 weeks at recommended dosage) received buspirone (20-30 mg/day) for 4 or 5 weeks in addition to their existing antidepressant. Of the 22 patients who had buspirone added to their selective serotonin reuptake inhibitor antidepressant regimen (fluoxetine, paroxetine, or citalopram), 59% (13/22) showed complete or partial remission of their depressive symptomatology. Similarly, 63% (5/8) of patients treated with buspirone in addition to clomipramine showed complete or partial remission. The mean score on the Clinical Global Impressions Scale fell by 64% (from 4.7 to 1.7; p < 0.0001) in treatment responders (complete and partial). No serious side effects were observed during combination therapy. Seventy-nine percent (11/14) of initial responders (both complete and partial) who remained on augmentation therapy for at least 4 months were symptom-free at follow-up. Buspirone augmentation may produce marked clinical improvement in depressed patients who are initially unresponsive to standard antidepressant therapy.

A behavioral approach to marital therapy. A Greek experiment.

In this paper we refer to some problems encountered and techniques used in treating 14 couples with behavioral psychotherapy. In all 14 cases neurotic symptomatology was present, at least in the spouse who first sought help. Patients also complained of a lack of between-couple communication and understanding. 12 spouses complained of an unsatisfactory sex life. Treatment was planned in coordination with the couple and tasks were set for both of them. The main target was the improvement of communication in every aspect of marital life. Treatment lasted for about 3-6 months, and every couple had about 8-12 sessions. The outcome was considered as excellent in 4 couples and satisfactory in 7. In 3 cases no improvement was observed. A year later no recurrence was reported.

A double blind comparative study of mianserin and a fixed combination of amitriptyline plus chlordiazepoxide.

A double-blind comparative trial was carried out in 64 depressed out-patients, comparing mianserin with a fixed combination of amitriptyline + chlordiazepoxide. Both preparations proved to be equally effective at the end of the trial, but the mianserin group showed a considerable therapeutic improvement at the end of the second week, in comparison to the other group. With regard to side-effects, mianserin produced by far less side-effects than the combination of amitriptyline + chlordiazepoxide. It was concluded that mianserin should be considered as one of the most suitable drugs for the treatment of depressed out-patients.

Resocialization problems in a chronic psychiatric department.

In this paper we discuss the problems we encountered in our effort of resocialization in a chronic psychiatric Department. Resocialization of chronic mental patients is a complicated and time-consuming task. Good staff relationships are absolutely necessary, otherwise the whole resocialization program could be jeopardized. Changes must take place slowly and be fully absorbed by the ward's life before new ones are introduced. There is always the possibility that the nursing staff will resist any change. This is due to the notion that the more independent a patient is the more dangerous he becomes, and to the very fact that resocialization means some loss of the nurses' authority.
