[The Aulhorn flicker test. I: Concomitant neuritis in malignant non-Hodgkin's lymphoma and leukodystrophy].

Cases of optic nerve involvement in malignant non-Hodgkin lymphoma and multifocal progressive leukodystrophy are reported. In both patients an optic neuropathy was diagnosed secondary to infiltration of the optic nerve and the Aulhorn flicker test was positive. Pathological and neurophysiological conditions which yield a positive flicker test are discussed in the light of these findings.

[Contributions to the Aulhorn flicker test. II: The flicker test and the Pulfrich phenomenon in clinical diagnosis].

Fourteen patients with symptoms of acute unilateral optic neuritis were examined with the Pulfrich test and the Aulhorn flicker test. It was found that the positive Pulfrich response and the depth of the apparent orbit depends on the ocular or neural diseases causing a difference in neural conduction from the two eyes. The Pulfrich test is positive for considerably longer than the flicker test. It may be concluded from this that the Pulfrich test is more sensitive to minor residual dysfunction of the affected optic nerve.

[Correction of unilateral aphakia in children, adolescents, and adults at an age when accommodation is still possible].

Twenty-one patients, aged 4-42 years, with unilateral aphakia after perforating injury, were corrected with a combination of spectacles and contact lenses. While taking stereopsis and aniseikonia into consideration, each patient was corrected with the combination of contact lens and spectacles to achieve the best stereoscopic vision. Two of the five children (aged 4 and 7) achieved positive results on stereoscopic acuity. 15 of 16 patients (10-42 years old) showed good results on the TNO table test. The influence of visual acuity and age as well as the influence of the principles of the stereoacuity test are discussed.

[Random dot stereograms and the limits of their detectability].

The influence of contrast reduction combined with aniseikonia on disparities in the TNO test was investigated with monocular induced refractive errors by spherical convex and concave lenses. Single contrast reduction was produced by Bangerter foils. Unilateral near vision of 0.1-0.2 is compatible with a positive TNO test. The average tolerance limit of the TNO test was 6% when produced by monocular induced refractive error. Otherwise, pure aniseikonia values of 5% to 9% produced by overall-size lenses are compatible with the TNO test.

The proximal and distal click syndrome of the superior oblique tendon.

In the following case report we describe a healthy 25-year-old woman suffering from the "click" syndrome with pseudoparesis of the superior oblique muscle and a concomitant inflammatory ocular disorder. No etiology of the chorioretinitic changes or click syndrome could be ascertained. Based on the different topographic conditions present, we suggest the introduction of two new subgroups involving a proximal and a distal click syndrome.

[Principles in correcting unilateral aphakia to preserve binocular functions].

A new concept for correcting unilateral aphakia has been developed for patients able to accommodate. This concept is based on the investigations of binocular tolerance observed in aniseikonia. The test method for binocularity should have a retinal image of adequate size and should respond sensitively to limitation of vertical disparity. These criteria are satisfied by the TNO test for stereopsis. The retinal image size can be assessed using a spectacle-contact lens combination similar to an inverted Galilean telescope system. The image size can be reduced as long as the patient perceives positive stereopsis by the TNO test. Some typical cases are discussed. In patients who had had conventional contact lenses fitted more than 2 years previously it was no longer possible to obtain positive TNO values.

Aniseikonia↗

[Orbital and ocular mucormycosis].

A case of mucormycosis in a 46-year-old woman with acute lymphoblastic leukemia is described. The typical clinical findings, diagnostic procedures, and therapeutic considerations are discussed. Mucormycosis is a rare complication in immunocompromised patients. Early ophthalmological diagnosis and rigorous therapy may improve the course of this otherwise fatal disease.

Endophthalmitis↗

[Gas gangrene of the eyelids and orbits].

Infections with Clostridia are very rare in the orbital region. On the basis of a case report of such an infection in the orbit the authors discuss the clinical behavior, diagnosis, and therapy of this infection with anaerobic bacteria. Computerized tomography was been very helpful in the diagnostic procedure.

[Fusion amplitude and aniseikonia. Experimental studies of aniseikonia tolerance in unilateral aphakia].

The fusion amplitude of image pairs with aniseikonia was measured using the synoptophore. Aniseikonia was produced in five subjects by alteration of slide drawings. The subject's right eye was in cycloplegia after paralysis with cycloplegic eye drops. To compare these measurements with aniseikonic pictures to those using the same image pairs without aniseikonia the fusion amplitude achieved was expressed as a percentage of the aniseikonia-free fusion amplitude. The relation between aniseikonia, eccentricity of the image borders and percent fusion amplitude was described using a mathematical approach. The experimental results reveal that a minimal increase in overall aniseikonia with greater eccentricities (greater than 5 degrees) leads to a rapid decrease in the fusion amplitude and the occurrence of diplopia. The aniseikonia tolerance level of images with greater eccentricities achieved experimentally and theoretically are in good agreement with results found in unilaterally aphakic patients.

[Visual disorders as key symptoms of ocular migraine].

Vascular disturbance of the anterior visual pathways characterized by monocular visual experiences are sometimes caused by migraine. Retinal or ophthalmic migraine, a variant of complicated migraine, is distinctly rare. Two cases of female patients with ophthalmic migraine are reported. The relationships between the use of oral contraceptives and pregnancy are discussed.

Cartilage choristoma of the eye.

A young girl had a rigid, white tumor that penetrated the sclera. It was partially resected, and histopathologic study demonstrated choristoma of elastic cartilage.

Cartilage↗

[A contribution on Crouzon's disease].

Four generations of a family with craniofacial dysostosis were studied. All the members of the family affected by the disease had craniofacial abnormalities combined with ptosis on the left and some members also had convergence strabismus. A strong genetic correlation between ptosis and squint was found.
