[Prognosis in vesicoureteral reflux].

OBJECTIVES: To emphasise the importance of early diagnosis of Vesicoureteric Reflux (VUR) in prevention of renal failure. METHODS: General testing of children was followed by micturating cystourethrography (MCGU) for those with the diagnosis of urinary-tract infection. In cases where high level of VUR was determined, satic nuclear medicine scanning was performed using an isotope-labeled substance (e.g. DMSA). RESULTS: 31 nephrology department patients were followed: 29 girls, 2 boys ages 0-15. MCGU showed 33 kidneys with reflux. 13 of the patients had reflux in the first or second degree. 7 patients showed reflux of the 2-3 degree while 13 had a high degree of reflux. Among the children under 7 years of age, total of 16 kidneys, DMSA showed various degrees of damage to the paranhaim and the development of scars. CONCLUSION: After the VUR has been diagnosed, it is difficult to predict whether scarring will take place. The possibility of scarring is higher among younger children when appropriate therapeutic modalities are not identified.

[Acute renal insufficiency in children].

Acute renal failure (ARF) is not rare in hospitalized children, especially in newborns and infants. Sudden disorganization of a glomerular and tubular function is the most often transitory and require a quick make of diagnosis and early therapy in order to obtain a adequate kidney function recovery and to reduce mortality. In this article 51 children with ARF were included from newborn period to 16 years of age (25 female: 26 male). We described the most often causes of ARF, diagnosis procedures and nondialytic treatments. The diagnosis was made by history, physical examination, laboratory and radiology findings. Causes of ARF were divided on prerenal, renal and postrenal. The treatment of ARF in all cases was conservative with good success in 45 patients and in 6 children followed death because of the nature of primary diseases. The results of study show that the accent is on the prevention, early diagnosis and early therapy of diseases which lead to ARF. We conclude that a multidisciplinary therapeutic approach is imperative for successful management of ARF.

[Ultrasonography in examinations for urinary tract anomalies].

Using the ultrasound grey scale and real-time method at the Nephrology Department of Sarajevo's Children's Clinic, 90 children with different kidney diseases were checked. Among these patients, there were 22, i.e. 24.4% children with anomalies of urinoexcretory system as follows: 10 i.e. 45.46% with double or cleft excretory system, 6 i.e. 27.27% with vesicourethral reflux, 5 i.e. 22.73% with unilateral obstruction of pyelouretheral junction and 1 i.e. 4.54% with uni/lateral obstruction of vesicourethral junction. Ultrasonography has become a significant technique in diagnosing and monitoring both kidney structures and excretory system. In these war circumstances, by applying this method we have succeeded in simplifying and speeding up the diagnosing procedure as well as deciding on adequate therapy for our patients.

[The complement activation system in acute poststreptococcal glomerulonephritis].

The activation of the complement system is very important part of the immunologic process. Measuring levels of products of complement activation is a useful diagnostic and prognostic procedure. This report presents the results of a follow-up of 54 children with acute poststreptococcal glomerulonephritis. At the beginning of the disease, all patients (i. e. 100%) had decreased levels of C3 component of the complement system, and 72.2% patients had decreased levels of the haemolytic complement. The mean value of the control measurements increased significantly from one measurement to another (with p < 0.01, and within 6 weeks after the beginning of the disease, it reached the normal range. According to our results, the activation of the complement system was induced by the classical pathway. We cannot explain why the component C4 did not take part in it. According to the results of the haemolytic assay, the alternative pathway of activation was used too. We found that there was a correlation between improvement of clinical features and decrease of metabolisation of complement components.
