Acute unilateral obstruction of ureter. Disparity in divided renal function calculated from 131I-hippuran and 99mTc-DTPA renography.

Fifteen patients with acute unilateral obstruction of ureter and 30 control patients were studied with simultaneous 131I-Hippuran and 99mTc-DTPA gamma camera renography. In severe outflow obstruction the relative function of the obstructed kidney was higher when calculated with Hippuran than with DTPA. No difference was found in the control subjects. This means that the glomerular filtration rate of an obstructed kidney may be relatively more reduced than the renal blood flow. Therefore, measurement of the single kidney fraction of total glomerular filtration rate has to be calculated with a glomerular filtered tracer, e.g., 99mTc-DTPA.

Subclavian vein catheterization for acute and chronic haemodialysis. A safe temporary vascular access.

The technique and use of a percutaneous subclavian vein catheter for haemodialysis in 20 patients are reported. The catheterization procedure carried a very low morbidity, and blood flow rates of 200-250 ml/min were achieved through the catheters. Use of this angioaccess saves future possible sites for permanent vascular access. Infectious complications were not encountered. Subclavian vein catheterization is a favourable alternative to external Silastic Teflon shunt.

Plasma exchange in the treatment of propoxyphene intoxications.

Six patients heavily intoxicated by propoxyphene were treated with plasma exchange. Only one patient died because of cardiac dysrhythmia, the other five patients survived, probably because the plasma exchange removed the free plasma fraction of propoxyphene. Plasma exchange also proved to be effective in improving the consciousness of the patients.

Kidney function and quantitative histological changes in patients on long-term lithium therapy.

Renal function and structure were studied in 57 patients on long-term lithium treatment. Nine per cent had a moderate decrease in glomerular filtration rate (GFR), 75% had decreased concentrating ability. No correlation between GFR and the duration of the lithium treatment was found. Twenty-four renal biopsy specimens were studied by light microscopy and compared with kidney biopsies from an age-matched control group of patients without arterial hypertension and who had never been treated with lithium or neuroleptics, but who suffered from acute oliguria or slight proteinuria. The number of sclerotic glomeruli and the focal distribution of intestitial fibrosis was significantly increased in the group of patients treated with lithium. No difference in the total amount of fibrous tissue or the number of atrophic tubules was demonstrated. There was no correlation between the duration of lithium treatment and the structural changes. The biopsy material originated from patients treated with a single and reduced dose regimen only. This regimen might explain the few and slight structural changes found, in contrast to other investigations in which patients were treated with lithium in divided doses.

Serum 1,25-dihydroxycholecalciferol in anephric, haemodialyzed and kidney-transplanted patients. Effect of vitamin D3 supplement.

The serum levels of 25-hydroxycholecalciferol (25-OHD3) and 1,25-dihydroxycholecalciferol[1,25-(OH)2D3] were measured simultaneously in nephrectomized patients on maintenance haemodialysis, in haemodialyzed patients with preserved kidneys who were receiving different vitamin D supplement, and in patients who had undergone renal transplantation. The results indicate that the production of 1,25-(OH)2D3 can be stimulated in patients with minimal residual renal excretory function by increasing the serum levels of 25-OHD3. Successful renal transplantation was followed by a rise in serum 1,25-(OH)2D3 concentrations.

Precipitating antibodies against pseudomonas aeruginosa in serum from dialysis patients.

Hemodialysate in our facility was found contaminated with Pseudomonas aeruginosa. By means of crossed immunoelectrophoresis, series of serum samples from 20 patients in regular dialysis treatment have been investigated for development of precipitating antibodies against P. aeruginosa during dialysis treatment. In 6 patients antibacterial antibodies occurred during the observation period but were, in all 6 occasions, in close relation to acute intercurrent bacterial infections and not related to the dialysis treatment. It is therefore concluded that P. aeruginosa antigens from the dialysate do not pass the dialysis membranes in quantities sufficient to evoke a humoral immune response.
