[Chronic urticaria and autoimmune thyroid diseases].

INTRODUCTION: Chronic urticaria results from multiple interactive causes. For certain authors dythyroidism is significantly more frequent in patients with chronic urticaria, often associated with other autoimmune disorders. PATIENTS AND METHODS: This study included 45 patients with chronic urticaria (29 men and 16 women, mean age 45.6 years). All underwent clinical examination for thyroid disease with assay of free T3 and T4, ultrasensitive TSH and antimicrosome and antithyroperoxidase antithyroid antibodies. RESULTS: Among the 45 patients, 8, all women, had an autoimmune thyroid disease: Graves' disease (n = 1), juvenile chronic thyroiditis (n = 1), autoimmune disease n = 6). All had goiter but most had no clinical sign suggestive of hypothyroidism or hyperthyroidism. Other clinical features or laboratory findings suggested autoimmune disorders in 3. DISCUSSION: These results suggest that a complete thyroid examination with hormone assay, especially in women, should be performed in patients with chronic urticaria. TSH and antiperoxidase antibodies appear to be the most sensitive and specific assays in patients with autoimmune thyroid disease.

Juvenile dermatomyositis: treatment with intravenous gammaglobulin.

High-dose intravenous gammaglobulin (IVGG) has proved to be effective in the treatment of a number of immune disorders. We report two patients with juvenile dermatomyositis (DM) who improved with IVGG therapy. These patients had become refractory to corticosteroids and had developed unacceptable steroid toxicity. We suggest that IVGG can be useful in the treatment of juvenile DM, by reducing steroid requirements, and replacing immunosuppressive drugs.

[Contact allergy to gold and its alloys. Pertinence of gold salt patch tests].

Allergic contact dermatitis to gold and its alloys is a rare affection and it is difficult to interpret gold salts patch tests. We report two cases of patients with positive patch tests to 0.5% sodium aurothiosulfate discovered during a dermatology exploration of an occupational contact eczema (for the first patient) and an intolerance to gold jewelry (for the second). There is much confusion in the literature concerning the allergologic exploration of contact dermatitis to gold: no standardized test, possible cross reactions between different gold salts, the tests often irritate. The mechanism of sensitization to gold salts is unknown since pure gold is inalterable and does not contain any salts. The pertinence of a positive test to one or more gold salts must therefore be examined carefully and the diagnosis of gold allergy must not be made without sufficient evidence.

[Severe contact dermatitis caused by Parfenac cream].

Bufexamac is an anti-inflammatory agent used as topical treatment of various pruritic diseases. Since it was put on the market, this product has been blamed for the occurrence of contact dermatitis (eczema, urticaria) and, rarely, severe toxicodermia. We report a case of severe contact dermatitis developed after application of bufexamac (Parfenac) cream. Patch-tests performed two months after the acute phase were positive (+ + +) for both Parfenac cream and bufexamac.

[Melanoma and primary hyperparathyroidism].

The authors present 3 cases of melanoma associated with a parathyroid adenoma discovered by routine measurement of blood calcium levels. The melanomas were differently located and had different histological degrees. Hypercalcaemia is an infrequent complication of melanoma. According to data found in the literature, it is most often consecutive to bone metastases. The other causes of hypercalcaemia are metastatic extension of melanoma to the parathyroid glands, secretion of parathormone-related peptide (PTH-RP) and the actions of prostaglandins, vitamin D and the osteoclast-activating factor (OAF). Primary hyperparathyroidism due to adenoma has seldom been described associated with melanoma.

Photoallergy to hexamidine.

This report details the case of an atopic patient showing a photosensitivity caused by a hexamidine solution. Patch tests were negative but photopatch tests to hexamidine in water solution were positive, after irradiation by ultraviolet A (UVA); all control tests were negative. It is well-known that hexamidine can induce contact allergy but this case was the first reported in which UVA was implicated. This photoallergy can be explained by the structure of the hexamidine molecule, but the question is: why has there never been such a case reported before?

[Dilated cardiomyopathy disclosing dermatopolymyositis. Management].

Cardiac involvement in dermatopolymyositis is common but rarely symptomatic. Cardiac failure as the presentation is very rare. No correlation exists between the severity of muscular involvement and cardiac involvement. Myocarditis is not uncommon and must be borne in mind if CK MB are above 3% of total CK. It is always associated with electrocardiographic abnormalities. Corticosteroids remain first line treatment. They generally lead to regression of cardiac problems which are usually of secondary importance. If cardiac involvement is severe, the spectacular action of venoglobulins should lead to their use being envisaged from the outset, combined with corticosteroids.

[Association of sarcoidosis and Waldenström's macroglobulinemia].

Sarcoidosis is seldom associated with Waldenström's macroglobulinaemia. We report the case of a 58-year old woman, 7 years after cutaneous and pulmonary sarcoidosis had begun, developed Waldenström's macroglobulinaemia. Chlorambucil was effective on both diseases. The possibility of reactive sarcoidotic granulomas in neoplastic diseases may cast doubts on the reality of the sarcoidosis-malignant haemopathy association. However, most authors consider that such associations are not fortuitous and can be explained by the immunological abnormalities observed in patients with sarcoidosis.

[Peristomal pyoderma gangrenosum after colectomy for Crohn disease. Successful medical treatment].

A case of Pyoderma gangrenosum with two different abdominal sites in a female patient suffering from colonic Crohn's disease is presented. Local trauma on the midline scar of an incisional hernia and around the stoma were the possible triggering factors. Despite the major abdominal wall defect and an infected parietal collection, steroid therapy was very effective without the need for surgery.

Dermatitis herpetiformis associated with ulcerative colitis.

Over the past 20-years, it has been shown that the majority of patients with dermatitis herpetiformis (D.H.) suffer from coeliac disease of varying intensity. Dermatitis herpetiformis may also be associated with other autoimmune diseases but only exceptionally with chronic ulcerative colitis (U.C.).
