Newer drug therapy for congestive heart failure.

BACKGROUND: The management of congestive heart failure has undergone a number of modifications over the past 5 to 10 years. METHODS: These include assaying the role of inotropic drugs, evaluating the role of phosphodiesterase inhibitors, considering the role of intermittent inotropic infusion in ambulatory patients, and recognizing the importance of angiotensin-converting enzyme inhibitors. Very recently, the important role of angiotensin II receptor blocking agents and the use of beta blockade have provided additional modalities for the control of congestive heart failure. The relative usefulness of such therapy has been reviewed in this article. CONCLUSION: The management of congestive heart failure has undergone considerable change with the use of newer and more effective drugs.

Evaluation of quinidine short and long-acting in control of arrhythmias.

Twenty-one patients with arrhythmias responsive to quinidine were studied both with regard to consistency of response as determined by repeat Holter Monitoring and in half of these patients a comparison of short and long-acting quinidine was made. Holter Monitoring demonstrated greater than 60% reduction of ectopic activity in all but one patient and reproducibility varied by less than 10% in nineteen of twenty-one patients. Nine of eleven patients showed the same response to long-acting quinidine as compared to short-acting. Eight of nine patients having significant numbers of ectopic pairs showed a significant reduction with quinidine therapy, both short-acting and long-acting.

I-131 total-body scan: localization of disseminated gastric adenocarcinoma. Case report and survey of the literature.

This is a case of striking radioiodine and [99mTc]pertechnetate uptake by disseminated nonthyroidal (gastric) adenocarcinoma. A 65-yr-old man was euthyroid and serum thyroglobulin concentration was normal at 11 ng/ml. Bone-marrow biopsy showed that the metastatic tumor cells were negative for thyroglobulin on immunoperoxidase stain and the secretory product was mucicarmine-positive. We estimate that radioiodine uptake in the normal thyroid gland was less than 10% of total tumor uptake. At autopsy, the stomach was the site of the primary tumor, which had the same cellular and histochemical characteristics as the metastatic lesions in bone and liver. It is emphasized that the use of pertechnetate for screening patients with gastric adenocarcinoma may be clinically useful in the early detection of metastatic lesions.
